For the last question, I am kind of confused, I thought the 2 option needed further teaching (DI pt shouldn't restrict fluids) while the 3rd option was correct (so no further teaching is required) since the pt had DI @simple Nursing
Thank you, Nurse Mike, love your videos. I am taking my NCLEX in 9 days! I really enjoy the videos and they are really easy to remember. I have a quick question at the end of the video 15:24 minutes the question about Desmorpressin choice 2 is not right either and we don't need to restrict our fluids since the pt is losing water and they will be dehydrated. we encourage them to drink plenty of fluids. Just wanted to make sure I am not missing anything. Thank you again!!
Yep option number 2 is a little bit confusing. In the question it didn't say if it is SIADH or DI but in the option No. 4 it is indicated that it is SIADH. It means if it is SIADH he need to restrict his water intake bcoz of the water retention.
@@montecarlocreencia1343 but it said the pt was starting on desmopressin , so that indicated that they have DI not SIADH we do not want to stop fluid output on pts with SIADH since they already have so much. In the last question you skipped going over the second option "I will make sure to restrict my water intake" love the videos! great tips on how to memorize the information
@@bodonap02 The answer is that they need further education if they believe they need to restrict their water intake. Patients with DI are extremely dehydrated and their output can often be greater than their input. You want to make sure they have access to fluids at all times unless contraindicated and that you are recording their I&O and daily weight.
Water intake needs to be restricted while taking Desmopressin because your body is beginning to retain fluids and if you continue drinking water like you did before getting the Desmopressin, you are at risk for fluid overload.
Holly and Lauren who is (NCLEX) right. I see both of your points. The client is dehydrated so fluids are needed. However, the drug can cause fluid overload. So would the most correct answer be that fluids should be monitored and adjusted as needed based on I/O's? Thanks

Thank You Mike... You explain things so well. But I have a question about the last Question regarding desmopressin. #1 Why is number one correct? I thought the desmopressin causes Headache because it decreases the Na. #2 - I thought number 2 was correct cause why would the client want to restrict the water intake if they need it? is it because of what the drug does which is to hold the water in? Thank You!

@@boomboom04201 I believe it is because the patient is urinating and losing all of the water they are taking in. If they continue to take in the fluid, it will just continue this issue.
I thought this at first too. But if you think about it, during DI, you are drinking a lot of water because you are losing so much from the lack of ADH. When you start taking desmopressin, you are gaining that ADH which means your body is going to start retaining fluids. It is important to restrict your fluid intake while taking the medication to prevent fluid overload.

Hi Mike! For the question "A client is newly prescribed desmopressin nasal spray, which statement by the client indicates further education is needed"? (Select all that apply) 1. Frequent headaches are normal 2. I will make sure to restrict my water intake 3. This drug is used to decrease my frequent urination 4. I am glad this drug is able to treat my syndrome of inappropriate ADH 5. I will record my UOP closely while taking this drug 6. I will make sure to weigh myself weekly In the video, the answers were 1,3,4,6 I was thinking 1,2,4,6. Could you please explain this? ** Thank you so much. All your videos have been super helpful!! :) **
Hi Mike! I'm a nursing professor with students who been love your videos so I am offering the following feedback wrapped in kudos for the great job you all at Simple Nursing are doing: At 15:05 if option # 3 "This drug is used to decrease my frequent urination" is a true statement then it should Not be selected as one of the correct answers because the question is interested in statements that indicate a need for further education. Please fact check me. Also, option # 2 "I will make sure to restrict my fluid intake" should be selected as one of the correct answers because there is no need to restrict fluid intake in patients with DI who have been prescribed desmopressin. Please fact check me. Blessings
I think there is need to restrict fluid intake. He is already on desmopressin which is now helping to retain fluid, adding more fluid will cause more harm. So that answer doesn't need further education.
@@odunzejoy3719 They are not to decrease fluids but they do not need excessive fluids. They are dehydrated so are in need of fluids when on desmopressin.
Great vid. However I think the last slide should be fixed, since desmopressin decreases urinary frequency and the question is about the client needing further education.
Omg thank you for mentioning this. I thought I was going crazy lol. I kept replaying the beginning of the video to see if I missed something. #3 is not a need for further education because it is correct. Its used for DI to retain fluids and decrease output. Should not have been selected.

for the hypertension and edema though, I found other sources that says there is no Hypertension and edema cause it says when there is increased in blood volume and decreased in osmolarity, it actually promotes the ANP and BNP, and inhibits aldosterone. Therefore, promoting sodium secretion and water secretion although there is still excess water, so that means is patient will have euvolemic hyponatremia which there is no edema and hypertension. So, I'm confused now which is which
I was just checking to same thing. My Prof said they don’t have hypertension (SIADH) and sometimes even have low BP, and when I checked online it said the same. This was the only confusing part.

The focus/stem of the question is patient teaching about Desmopressin therapy, not regarding the disease. Since Desmopressin works as our natural ADH, it will retain water within the body, so the client will be teaching to restrict fluids, since the medication can cause fluid overload and hyponatremia.

Wouldn't the answer to the last question be 1,2,4,6?
Yeah, I think so too
Yes, he should not have checked #3, and he skipped #2 but they would want to drink water since they are "dry inside."
that's what I thought. Thank God you caught that too.
I thought that too
My exact thinking @ Bullsforever, since it depress urine output( is not the same as decrease frequency of urination?) & increases BP. Pls more clarification, thanks @ Mike
I just came back to say Thank you Nurse Mike. I passed MedSurg bc of you😚

Thank You!

For the select all that apply for desmopressin isn't # 2 incorrect?
I was thinking the same thing cuz if you have di and need desmopressin to treat it why would you restrict fluid intake?
@@boomboom04201 I believe it is because the patient is urinating and losing all of the water they are taking in. If they continue to take in the fluid, it will just continue this issue.
I thought this at first too. But if you think about it, during DI, you are drinking a lot of water because you are losing so much from the lack of ADH. When you start taking desmopressin, you are gaining that ADH which means your body is going to start retaining fluids. It is important to restrict your fluid intake while taking the medication to prevent fluid overload.
@@hollywilson2699 oh ok... That kinda makes sense cause I was wonder the same thing.

Hi Mike! For the question "A client is newly prescribed desmopressin nasal spray, which statement by the client indicates further education is needed"? (Select all that apply)
1. Frequent headaches are normal
2. I will make sure to restrict my water intake
3. This drug is used to decrease my frequent urination
4. I am glad this drug is able to treat my syndrome of inappropriate ADH
5. I will record my UOP closely while taking this drug
6. I will make sure to weigh myself weekly
In the video, the answers were 1,3,4,6
I was thinking 1,2,4,6.
Could you please explain this?
** Thank you so much. All your videos have been super helpful!! :) **
1,4,6 are my answer. 2 is incorrected because desmopressin hold the water that why the PT needs to restrict the fluid intake.
Thanks sir
Good explanation
Keep doing
Hi Mike! I'm a nursing professor with students who been love your videos so I am offering the following feedback wrapped in kudos for the great job you all at Simple Nursing are doing:
At 15:05 if option # 3 "This drug is used to decrease my frequent urination" is a true statement then it should Not be selected as one of the correct answers because the question is interested in statements that indicate a need for further education. Please fact check me.
Also, option # 2 "I will make sure to restrict my fluid intake" should be selected as one of the correct answers because there is no need to restrict fluid intake in patients with DI who have been prescribed desmopressin. Please fact check me.
Blessings
You are right! I found this odd too. In the video that is not the question he reads tho.
I think there is need to restrict fluid intake. He is already on desmopressin which is now helping to retain fluid, adding more fluid will cause more harm. So that answer doesn't need further education.
@@odunzejoy3719 They are not to decrease fluids but they do not need excessive fluids. They are dehydrated so are in need of fluids when on desmopressin.
thats why i confused
option 2 and option 3
Hi Mike, thank you so much for your videos! They are so helpful! I could pass NCLEX RN in the first attempt! 😊😊

I am wondering the same ? number 3 is not completely right

how was it
For SIADH won't the client remain euvolemic?? Meaning they won't have HTN??
Thank you finally someone makes sense!

Very important information thank you
for the hypertension and edema though, I found other sources that says there is no Hypertension and edema cause it says when there is increased in blood volume and decreased in osmolarity, it actually promotes the ANP and BNP, and inhibits aldosterone.
Easy to understand and remember! Thank you!

@@SimpleNursing 👍🙏
For the last question, why was "i will make sure to restrict my water intake" not selected?
He never mentioned the option. But I think it should be selected since DI = Dry Inside so the body needs more fluids NOT restrictions.
I think 2 should be selected because water needs to be increased
The focus/stem of the question is patient teaching about Desmopressin therapy, not regarding the disease. Since Desmopressin works as our natural ADH, it will retain water within the body, so the client will be teaching to restrict fluids, since the medication can cause fluid overload and hyponatremia.
