SLAP Tear Stretches & Exercises for Shoulder - Ask Doctor Jo

    A SLAP Tear can cause many shoulder problems including pain, decreased shoulder strength, and decreased range of motion. These stretches and exercises should help restore movement and strengthen your shoulder.
    The first stretches are called pendulums. Use a small weight in your hand to help open up the shoulder joint. Use a chair or counter top for balance, and lean over so your arm hangs down towards the ground. Move your body, not your arm in circles so your arm swings around like a pendulum. You can also rock front to back and side to side. Start off with 10 of each and work your way up to a minute of each.
    Now you are going to be in standing using a resistive band. Try to keep your arm straight the whole time and don't bend your elbows. Your thumb is going to be pointed upwards. The first two are front and back which are flexion and extension. You don't need to raise the band way above your head. You can bring it to a 90 degree angle which is where your arm is parallel to the floor for flexion. Now you are going do put your arm out at a 45 degree angle which we call scaption.
    The next stretches you can use a Swiss/therapy ball, or if you don’t have one, you can use a table or counter top. This is an active assisted stretch, which means you are moving the arm now, but the ball will support the weight. You will slide your arm forward with your thumb facing upward towards the ceiling and lean your body forward until you feel a stretch. You can stabilize the ball with your other hand. Hold it for 3-5 seconds, and start off with 10-15.
    Then you will be in the quadruped position. Get on all fours, and try to get your hands directly under you shoulders perpendicular to the floor. Trying to keep your balance and use the stability of your shoulder, lift one hand up and tap the other hand slowly and controlled. Alternate back and forth. If this becomes easy, then you can go into a plank position, and repeat the same movement.
    For the next exercise, you will do a shoulder external rotation with a resistive band. Try to keep your elbows by your sides through out the exercise. If you want to roll up a small towel and place it between your side and your elbow for each side, this will keep your arms close to your side through out the exercise. Keep your elbows at about a 90 degree angle and your thumbs up towards the ceiling. Also try to keep your wrists in a neutral position. You don’t want to over stress your wrists, and then have a wrist injury. Slowly pull both arms out away from each other keeping your elbows at your side, and then slowly come back in. Start off with 10 of these, and then work your way up to 20-25. If that becomes easy, then move up with resistive band.
    Then you are going to do rows or scapular retraction. You can use your feet as an anchor. You want to keep your elbows in close to your body and make a rowing motion. Try to squeeze your shoulder blades together when you pull back. Make sure you are using a slow controlled motion. The next exercise is going to be punches or scapular protraction. Wrap the band around you, and keep your arms straight in front of you. Punch forward, but try not to bend your elbows. Start off with 10 of these, and then work your way up to 20-25. If that becomes easy, then move up with resistive band.

    I had a slap tear two years ago on my right shoulder. Pretty severe. Now its my left shoulder. I been doing these exercises for three days now. Last night I was able to sleep flat. I'll keep doing them. THANK YOU!

      @@Jordanfiend361 much better! I started using a kettle bell. The kettle bell swings helped open me up. I've been going since August. No more pain. Make sure if the pain is too bad to check with a specialist.

      @Jordanfiend361 orthopedic surgeon said no but the first time it happened it took a year to heal and the 2nd time about six months.

    IT REALLY WORKS!!!!!
    I had hard time lifting up my arm in some postures but after following your video instruction, acute pain is much relieved and I feel my shoulder components in right position.
    I saved money and time which I was planned to use for to see a doctor.
    Tips for another slap lesion patients:
    GET YOUR ROM OF SHOULDER RECOVERED FIRST BEFORE DOING ANY EXCERCISE
    especially if your slap tear is due to repetitive stress (throwing or pitching motion)

      What if you've always had your rom and after the mri shows slap tear cyst and bone spur and it hurts but is manageable dr said won't heal without surgery but how do I have full range of motion makes no sense to have surgery???

    Thank you for this a million, zillion times over. I had a nagging pain in the labrum and my ortho recommended some rest. I started doing these exercise and feel better already. Yet to do a push up since it causes pain when coming up, but I hope doing this for a month will enable me to do that.

      3 years now for me, both shoulders labral tears. Still can't do pushups. Each time I try, have excruciating pain that is disabling. Now have stopped altogether trying.

    Thank you so much Iv lived with this for years had a scan they said it’s torn but doesn’t need surgery I can’t even put a T-shirt on without pain! I think this is why my sleep has been trash I keep waking up after a few hours consistently.. I’m really starting to think it’s my shoulder pain while I sleep 🤨 thank you so much I’m doing this right now hopefully 🙏 if I do these every every day I should start to feel relief

      You're welcome. Hopefully they will help, but unfortunately, I don't know that answer since I can't evaluate you. Everyone is different. If there is any way you can see a physical therapist in person, they should be able to help you figure out a specific program for you and help answer your questions.

    For shoulder injury MRI report
    Impression:
    x SLAP tear is seen extending into the biceps labral anchor and the posterosuperior
    labrum.
    x Partial thickness tear involving few of the bursal surface fibres of the supraspinatus
    tendon - approximately 20-30%. There is an additional small focal concealed tear at the
    supraspinatus tendon footplate involving the posterior fibres measuring 2.0 x 2.0 mm.
    No retraction of the tendon fibres is seen.
    x Mild associated fluid is seen within the subacromial-subdeltoid bursa with mild joint
    effusion.
    For knee injury MRI report
    Impression:
    x PCL appears lax with heterogeneity and sequelae of high grade near complete thickness
    mid-substance tear with partial remodelling.
    x ACL appears intact.
    x There is interval resolution of the marrow oedema seen in the previous MRI in the proximal tibial
    shaft and anterior aspect of the lateral femoral condyle. No bony marrow oedema is seen in the
    present study.
    x Focal area of full-thickness tear of the Quadriceps tendon is seen at its insertion on the superior
    patellar pole involving some of the middle and lateral fibres [approximately 10-20%] with
    associated focal intrasubstance signal. Subtle associated periosteal injury with remodelling
    involving the underlying superior and anterior patellar pole an overlying soft tissue swelling is
    seen. This appears to be relatively unchanged in morphology except for the remodelling as
    compared to the previous MRI.
    x Subtle intrasubstance signal within the posterior horn of medial meniscus with irregularity of the
    inferior articular surface - represents sequelae of subtle tear with remodelling.
    x Lateral meniscus in intact.
    x Thickening of the LCL is seen. MCL and LCL otherwise appear intact.
    x Patella and patello-femoral joint are otherwise normal. Patellar retinaculae are normal.
    x Articular cartilage is normal.
    x Hoffa's fat pad is normal.
    x Mild joint effusion is seen.
    x Muscles surrounding the knee joint are normal. Popliteal fossa is normal.
    What exercise is preferred?
    Because of the lockdown period due to COVID19 crisis, no physiotherapist is available.
    It will be a great help to me.

    I have a slap tear that was extremely painful. I could not lift my arm up at all. I did these exercises and am pretty close to pain free and my range of motion is getting close to 100 These exercises are now part of my pre workout routine. Thank you.

      Yes. These exercises strengthen the stabilizer muscles and get the shoulder to rotate correctly.

    Hello Mam
    I am a Sportsperson.
    My sport is Cricket.
    But I am Suffering from a big injury of ligament damage in shoulder which basically called labrum tear
    My rehab was completed in 5 months
    But now , after 8 months , the pain is starting again .
    Whenever I throw the ball , pain started in shoulder .
    Pls can you give me a proper advice .

      She's probably not answering soon, this video is not recent, but i recommend you going with another Orthopedist and explain the whole situation, it is just a recommendation:) I am getting my surgery on October

    Thanks for the info Doc.... Ive just seen my surgeon today with my MRI result. Everything is perfect, except a very small tear on my left Labrum at 2'clock. So that explain the pinch I feel when I lift heavy stuff or I put a shirt on. He said unlike my right one who was heavily damaged, the left doesnt need surgery this time cause its going to create more problems and unnecessary damage. I fought him a lil bit on it because I wanted to go back lifthing weights and boxing as fast as possible. He said the best you can do is find a physio and do some strengthening to protect the Labrum. I was disappointed because surgery saved my right Labrum and I wanted the same for my left. Lets hope those exercices are going to help.

      @@robertstrada2456 Pain did not go away. I did stem cells last year, it helped a lot but they is still a little bit of pain left. In a couple of months I may try another PRP shot

      @@willwise2168 Yes it's totally worth it. I did the injection and the SLAP tear exercises for 6 month. After that I did another MRI and the tear was gone

      The labrum itself won't heal, but just like other tears, you can strengthen and stretch the muscles around the joint to make it strong and stable to protect the tear.

    I've been diagnosed with a SLAP tear and have been scheduled for surgery in 4 weeks. Are these exercises more effective pre- or post-surgery? It's probably too late to see effective relief before my scheduled date.

    Hi Jo, I recently had my MRI done and my left shoulder has rotatory cuff injury. But it’s not serious and the right shoulder has partial tear of superior tendon. Do you suggest I try these exercises to help my shoulders heal? 🙏

    I have had bicep pain for 3 years now. My mri looked fine. I had a Ct arthrogram. It showed a superior labral tear. My surgeon says time for surgery since I have failed p t and injections

    Didn't think that when I went to pt and the doctor gave me these exercises that they would work so I didn't do them. He even put me in a sling but I didn't wear it. It wasn't until after I was released from pt that I received an MRI but it isn't conclusive it is a tear. It only mentions that it is possibly a slap tear but inconclusive. Dry needling helped tremendously but wasn't the cure all and they thought I was getting addicted to it. Lol. Now I am going to do them and see if I have some relief. I just wished he had explained to me what they did. Thanks for this video from someone who has recovered. In the future I may end up with surgery because it sucks when you are a bodybuilder and can't lift properly due to shoulder pain.

      If you don't know what your diagnosis is, it might make it worse if you try to push through the pain. I do have some other videos that are for passive range of motion that might be a little less painful.

    Dear Dr Joe, thank you soooo much! These exercises have worked wonders for me..I'm back playing tennis and have very little referred pain down my forearm now since starting the routine 1 month ago. I have been doing them religiously each day and it has helped me considerably at work as my job is moderately physical. Although I do the entire routine, I have found the swiss ball stretch to be very effective in releasing the tension caused by shoulder pain. Thank you

    Just had an MRI and it shows a tear from 12 o'clock to 1 with separation back to 9oclock. I'm new to this, how bad is my injury.
    I'm looking to have surgery anyways, I'm trying to get into the fire service and can't viably have a 30 year career like this.
    Also, like the vid. Very informative. And I actually chuckled when you knocked the chair with the dumbbell

      Thanks :-) Sounds like you are definitely a candidate for surgery. If you can, you should try to go to some physical therapy before your surgery. The stronger and more flexible it is before the surgery, the better your recovery will be. Good luck.

      yeah i’m 17 w a pretty messed up shoulder popped out over 50 times no doctor checkup yet, But i did notice in the beginning my throwing decreased tremendously; immediately.

    very nice and simple way to explain to make a common man understand,well done ma"m. I had a shoulder tendonitis tear surgery 4 months back. I am on a rehab. Can i do these exercises. Moreover i cant touch my back with the operated arm as i can do with my unoperated arm . Please advice.

    Dr. Jo I have a question
    I am an amateur baseball pitcher and threw at about 70 mph. And as I said right before, i really appreciate your video.
    Question: If pain becomes completely relieved, Will it be proper to do this excercise with more intensive therabands?

    Dr. Jo, Thanks so much for all you do and your attention to safe and detailed instructions. Is it possible for the Labrum to heal in some cases? I've heard it usually needs surgical repair. If not likely to heal, then is the strength training the thing that will stabilize the joint to possibly avoid surgery that way?

      You're welcome, and thanks. Unfortunately, a labrum cannot heal on its own. You can strengthen the muscles around it, and often do pretty well without surgery depending on how big the tear is and where on the labrum it is. So it's definitely worth trying a few months of consistent therapy to see if you get some relief.

    Question ? I play baseball and got diagnosed with a slap tear in 2015 and really haven’t had any serious pain , went through physical therapy and everything and my shoulder feels fine . It’s 2020 now and I still have no pain lol do u think I could’ve got misdiagnosed??? It just sucks because whenever I have a little soreness in my shoulder I always have the labrum tear in back of mind n it always scares me

    Dr. Jo, are shoulder labrum rehab exercises contraindicated if there is a rotator cuff issue present as well? What about visa versa? Are RC exercises contraindicated for labrum issues?

    Hi Dr. Jo.... Have a slap tear 1 to my right shoulder per ortho doctor and arthoscopy MRI. I am currently in physical theraphy, taking ibuprofen and using ice packs. I have a lot of discomfort to my shoulders, scapula, neck , axilla and sometimes the breast area. I am a senior with health issues and on medications. Thank you in advance for the video, I will start using today, maybe it will help me. I am so tired of pain and discomfort.

    Thank you Dr Jo, your videos have helped me so much! I...have a slap tear but I dont feel much pain, just some instability during certain movements and exercises. I will try these out, I really dont want surgery.
    Did you need surgery for your slap tear?

      You're welcome. I had to have surgery on one side, but it was more because I also had a clavicle fracture that never healed, so they had to take out a big piece of bone to open up the space. I am pretty sure I have a tear on the other side as well, but I have done fine with conservative treatment.

    I have had some bad pain in my shoulder during softball season when I throw. I spoke to an orthopedic surgeon I know and he said it sounds like it could be a tear on my labrum, or possibly not that severe. I do not want to stop playing softball, but definitely need to see some improvement in my shoulder. In your opinion, doing these twice a day, how long should it be before I feel positive results? Should I stop playing softball until the pain is gone?

    Thank you so much doc jo, my deep pain in my shoulder joint in particular movements relieved more than 80 percent👍🏻👍🏻👍🏻👍🏻👍🏻after following these exercise after 15 days along with homeo drugs.... it works well❤️❤️❤️

    i fell down three months ago.. I took medicine, but the pain continued .. so my doctor advised me to take an MRI of the shoulder.it shows the following impressions
    1. supraspinatus low-grade interstitial tear with no fatty acid change in muscle.
    2. Biceps low-grade tendinosis.
    3. partial-thickness type 2 SLAP tear.
    I have severe pain in the shoulder. can I do this exercise to recover? my doctor suggested me to take a steroid injection.

    Thank you for this video! My diagnosis is slap syndrome of right shoulder and damage of long head of biceps. Tell me please, if I have pain in my long head of the biceps when I make this exercises, should I continue? Is this pain normal?

    Has anyone done these exercises over a week and has still found relief? I too have a SLAP tear, and have physical therapy exercises that are a bit different than this with minimal improvement.. not sure if I should spend me time trying these instead. Thanks!

    most of the workouts are pretty much almost the same as shoulder impingement workouts. guess i'll continue doing so adding some i've learned here. thank you!

First it was just my right shoulder now my left follow suit. I wonder if my effort is in vain since I'm only starting to do it early august till now. I really hope all these effort pays off.     hey, Dr. Jo! future PTA here, and I can't thank you enough for your videos! question with the pendulums; I understand you want to open up that joint space with the pendulums, but will the rotator cuff muscles not want to suck the humeral head back into the glenoid fossa if your pendulums become too big?... kinda defeating the purpose of the exercise? thanks in advance for your response, and I can't thank you enough for doing what you do!

      You're welcome! It shouldn't unless you are using an extremely large weight with the pendulums. Just using gravity or a small 1-2 pound weight isn't enough to cause that response. Good luck!

      @@AskDoctorJo Injured my shoulder playing basketball years ago. Blocking motion applying force down on the ball as opponent went up with the ball which resulted in a simultaneous upward counter-force. Since the acute injury, overhead throws (football or baseball) take 15 mins of warm-up before really being able to rip. A physician diagnosed a slap tear (no MRI) years ago. My experience has been the area can strengthen on its own (minimal rehab) and can revert back to square 1 anytime after overhead throw or similar force as original injury. No problem just build it back up. About a month ago I woke up and it reverted from sleep. I'm able to stay active (mountain-biking, basketball, football, skiing) but moving on and off this shoulder while sleeping sucks. I plan to start focused exercises as described and am interested in any suggestions based on what I've described. Perhaps I'll consider a surgical repair one day.

    I've had 3 shoulder dislocations and on the 3rd, 2 years ago, my labrum was torn and surgery was recommended. Unfortunately I had other pressing life issues that I couldn't be out for a few months so will these exercises still help me today from preventing another dislocation by strengthening the stabilizers or do you have any other videos that are more for my problem? Thanks

      Unfortunately, I don't know if these will help or not since I can't evaluate you. I also have one for shoulder stability that might help, but going to see a physical therapist in person would be the best option.

    Hi doctor Jo, I have been following your rehab exercises every time I have an injury. This time it was shoulder and I have been in pain for almost an year. MRI revealed that I had a SLAP tear.
