    Very Informative. But how do we go about recording the results? Is there a PDF sheet online to print off?

      You can score based on the finest symbol which patient pointed correctly.. The values of that will be mentioned at back of the chart or it will come along with the test.

      Do I need to do the animal pictures, even when the patient gets all the fine circle one?
