Maybe going a bit bigger on the drill size while still in tolerance in the hard materials leaving less for the tap to have to cut could maybe help. If that wasn't already done, but you gotta check your run-out if you go big like that
Jeeeeesus christ that would make my blood boil. Ive had this happen many times and the only option I have is to find an old endmill and start cutting away the tap =D But 2 times out of 3? Thats almost impressive! =D

Usual causes are the drilled hole did not allow for chip clearance, the hole was undersize, tap was dull.
It can also be a machinist's fault 😁
