    Episode 60 is a thoughtful conversation with my good friend Nadir Ali MD, recorded last October in his Houston home. Nadir is a cardiologist who is highly motivated to achieve the best outcomes for his patients. He also manages to carry out an extraordinary level of research into all aspects of health. Nadir has released many excellent videos where he delivers detailed lectures on lipids, glucose/insulin dynamics, atherosclerosis drivers, drug interactions and much, much more. He runs a busy cardiology practice in Houston, TX - and also created and led the Low Carb Houston conferences in 2018/2019.

    Hi Ivor, An excellent presentation by two extremely logical and compassionate men who will change the trajectory of heart disease. I have a question in regards to my situation. I have reduced my CAC score by 100 points, down to 335 from 435, by doing time restricted feeding and keto. I basically eat OMAD with a snack before dinner. Fasting from 18-20 hours per day. I feel that I am one of those lean hyper responders that you speak of in your discussion. I am almost 60 years old, my lean BF % of 7, 155 pounds at 5’9”, weight train 5 days per week and sprint and walk a couple of times per week as well. My LDL is 173, HDL 94, triglycerides 63. Total cholesterol 283, but all ratios are excellent. I have noticed however, my fasting glucose has been increasing. I used to be between 85-95, now when I check it it is between 110-118. Am I becoming insulin resistant by eating a higher fat low carb diet for over a year now, and should I be concerned. I am feeling great and alert and stronger than ever. What is your take on all of this? I appreciate your time in advance for your thoughts on this. Oh, I had my fasting insulin tested last year and it was a 3.0. I have scheduled some more blood work and should have results in a few days. Thanks again.

      Hey Brian great job - this glucose happens commonly - need use HOMA calculator not just glucose - target is to be below 1.2 , and you are

      Hey Brian, some studies are suggesting the body intentionally becomes insulin resistance when eating ultra low carb so as to intentionally spare glucose in the blood. It might not be pathological at all. It's worth doing some research on.

      Brian Shultes just a thought, OCCASIONALLY, cycle out of ketosis. Maybe once every 10 days. I believe we need to eat like our ancestors. Which included an occasional high carb day. I doubt very much they walked right by berries and/or fruit cause “that wanted to stay in ketosis”. They would have gorged on them. Also, this teaches your body on how to switch between fuel sources.

    55:53 “The power of an intervention to improve all of these significant variables (overweight, pre-diabetes, hypertension) in one individual is so high. So my point that I’m trying to make is that if a treatment works, you don’t need twenty-eight thousand patients to show a difference in outcome….If a pharmaceutical intervention needs 30,000 patients to show a half a percent difference in an all-cause mortality outcome, they should not even do that clinical trial, because it could just be noise."     I got a semi fit friend of mine started with keto. Her fitness improved markedly. She went to the doctor who ran a cholesterol test which found her LDL had gone "too high". She has since discontinued her keto diet.
    I'm thankful I've found keto, IF, CAC, etc. But I wish I would have found it 15 years ago.

      Get her to get a CAC score, then she can see that she doesn't have to worry about her LDL!

    Nadir is one of the great ones, while being extremely humble and cognizant of the need to adjust his assumptions as new evidence comes in. And Ivor is an effective popularizer, which every discipline needs.

    I love the point you made about how an engineer thinks vs an MD. If more MDs used thought processes like engineers, the trajectory of “sick-care” may actually change to true “health-care” as with functional medicine MDs.

    I am so looking forward to learning more. My CAC was 755 April 2019; diagnosed T2D 11.9 Sep 2019; started intermittent fastiing under Jason Fung and many others....A1C is now 7.6.....yet my PCP doesn't give credit to the follwing results in lipids: TC 281/281/300; HDL 46/62/62; LDL 208/163/199 and hold on to this; triglycerides 133/281/194. S0 in 3 sets of sets since Oct 2019 to 28 May 2020...I have a 87 point drop in Tryg....my A1c realized 7.7 to 7.6 ..weight is down over 20 lbs.... ; haven't felt this great in over 25 years. And I am eating carnivore (Dr ken Berry and Dr Shawn Baker) and many more, keto/veg....and taking supplements that help insulin, etc. BP by the way was 120/80!! Next CAC is scheduled Sep 2020; will ask cardiologist to run C Peptide, C reactive and full lipid panel after review of CAC. I suspect it will drop from 755. I am a happy gal trying to improve her health and reverse type 2!

      Kim P you can also try using vitamin K2-7 and D3 as a way to reduce calcium score

      @Tony L I have worsened but Dr Nadir Ali doinf nuclear stress test in 2 weeks but till me to back off bacon and hwc and butter...eat more leaner meats and walk 5 times a week with green tea and being fasted. My a1c is 7 now...and not 1 year yet with way of life of fasting or ketovore. So I think one more year a1c will be below 5.7 and fat burning will increase

      Not good...k2 m7 and d3 didn't reduce the plaque but I did increase it double. Working with cardiologist Dr ali in keto world as to why.

    So much knowledge packed into one hour. I started LCHF in August 2019, lost 32 pounds, can probably go down another 8-10. My LDL did shoot up to 224 (total 304, HDL 56, triglycerides 62 - as I recall) and the cardiologist wanted to put me on statins. These two (and some others) led me to decline the statin. Told the cardiologist we will monitor for a year or so as weight/fat loss stabilizes.oh yes, the HbA1C fell from 5.8 to 5.3 in 4 months. Changed my primary care doc who refused to listen, found a new one who is more knowledgeable. He reduced the thyroid med, which the earlier one refused to do. Also expects that the next test, in June, will show better markers.

    They told me I have familial high cholesterolenimia bc., my ldl was high and my mother always has high cholesterol but we are doing carnivore and both of us reversed heart disease. She is almost 90 and I took her off all the drugs they were giving her bf., I put her on this diet she has Alzheimer dementia and seems to be getting better except when I do let her eat bread then she goes off with only one slice. I can't get her to stop she's from Germany and always had it in the morning.

    This is pure gold. As a 57 yr old lean mass hyper-responder, it puts my mind at rest, even though my GP doesn't get it. Trigs/HDL = 0.5, but all my GP is interested in is the LDL.

      ...mine are at 0.236 with LDL close to 4 but my Doctor follows Dr Ali and so "he knows"; never mentions Statins during consultation and I have not exceeded 90g of carbs in a day since 2010.

    If I had a doctor like this, I might go see him once in a while. My doctor is a waste of time. I'm going to go get a cac test and pay for it myself.

      The only way to health is take full ownership and responsibility for your own health.
      You choose whether or not to stop eating crap foods and all sugars.
      You choose to eat only a whole food (including meat) diet.
      Your doctor can do very little, if anything, for your health. They are only useful for catastrophic problems and interventions.

      I love that RUclips lets you adjust the playback speed. I get impatient watching very long videos, but I put this one at 2x speed and it flew along at a pace I could tolerate, whilst still comprehending.
      I'm mentioning this because you can also adjust the playback speed down as low as half speed if you find some videos are moving too fast to follow. Maybe you know this already, but just in case it helps you.

    I was on Lipitor and Repatha and cholesterols were very low. Doctors were satisfied and I was miserable. Low energy, forgetfulness and getting heavier by 5-20 per year over 5 years. Stopped taking the cholesterol drugs about a month ago because I felt so bad and within several weeks felt better. I happened on keto and IF videos and started absorbing the information. One month after eliminating sugar, the most carbs I can avoid and an 18-6 to 23 fasting schedule I've dropped 22 lbs, 42 to 38 pants. Last week my GP didn't have a problem with dropping the stating and Repatha but suggested taking another med which I declined. Cholesterol had already jumped up but I don't care and this video has reinforced my decision. Thank you Dr. Nadir!

    patient with poorly controlled diabetes have high LDL-TG and once their sugar levels decrease, LDL-TG decreases and LDL-C increases. In other words, when LDL-TG is high, LDL particle is oxidized easier and it becomes small, dense and more atherogenic. When LDL-TG is low, LDL-C increases and the LDL particle is fluffy and less atherogenic.

    Heart surgeon LDL hypotheses is what’s make them money. Bypass, stems, surgery, sale of medications. Why would surgeons recommend patient to exercise , diet. Really make no sense when they might even still have debt to medical school or debt on their new Mercedes or their children education . Surgeon, need money big time. So they have to perform in someway . If they just advised patients to change life style then doctors will be in big economic chaos.

    I’ve chosen to ignore older 78 do Fitbit races 12 miles/ day resists weights upper arms. Feel great no joint problem. Some literature I read says older woman do better higher. I am a Carnivore OMAD. Also Fast to 1 GKI 72 hrs week.

    I am keen on getting your advice wrt my recent lipids test results:
    TC 273
    HDL 59
    TG 58
    LDL 208
    TG/HDL Ratio 1.0
    I take medication for hypertension and am suspected of having Familial Hypercholesterolemia (have corneal arcus, so it seems). Quit Statins 3 months ago. I do intermittent fasting 18:6 every day for 2 months. Have lost about 10kg. BMI of 20-21. Performed an Ankle Brachial Index DIY with a score of 1.5.
    I will really appreciate your advice Dr. Your lifesaving videos have served as my health guide these past months.

    My triglycerides are high and LDL high... CAC score is zero. Confused. 50 years old. On keto for 4 years! Dr's think I am about to die.

    I don't know if I'm a hyper Responder but my ldl went up from 93 to 153 on a low carb diet. I also do an 18/6 eating plan. My cardiologist wanted me to get off the diet. My trigs were 49 and my HDl 66. My weight went from 148 to 136 at five foot 7and a half. I went from lean to really lean. He put me on a statin for a short time but I couldn't tolerate them so i stopped. I suggested another calcium score. my score 5 years ago was 28. the score came back at 14. My Dad had his first heart attack at 51. I just turned 60 and have had no events. Hypertension also runs in the family dad was put on meds at 22 in the service me at 28. at one point i was on 20mg in 30's and 40's but i did way in the high 150's with a gut. now at age 60 i'm on 5mg and my weight today was 134.

    two of my favorites Dr. Ali and Ivor thank you thank you for all you have done and continue to do. I am a LMHR and all my markers are excellent and my LDL-C and LDL-P continue to go up. HDL high and trigs in the toilet :)
    I have taken care of the root causes no more prehypertension or prediabetes, my CAC = 0 . Eating keto/low carb and focusing on protein has resulted in a stronger, leaner me with tons of energy, (working on pull ups and doing P:E diet thanks to Ted Niaman) My blood glucose response to non starchy carbs is now excellent I can safely eat more sweet potatoes and other tubers.
    With Covid 19 I'm going to leave my LDL where it is in hopes that my immune system is ready. Stay safe everyone and KETO ON!

    Hello Mr Cummins, can you comment on the recent MESA studies that showed that dense coronary artery calcium may be less dangerous than plaque with less density. And the your thouths on vit K2 for lowering calcium deposits? Do you think its beneficial to reduce it or does the calcium stabilize the plaque.

    Good Evening ,
    This is so refreshing to hear interventional cardiologist discussing the pathophysiology of lipids and there vehicles , and the paradoxical roles between high cholesterol and low carb . I am a clinical pharmacist and a biochemist and have long known about this paradox , and I have instructed patients to also do a low carb , keto based diet to lose weight , decrease inflammation, and protect there cardiovascular system . I also practice this , and have seen dramatic results with these patients and myself . Bravo for not following the mainstream cardiologist school of thought , and actually cure your patients or slow down dramatically the disease progression . Excellent point about why does the body produce all these lipids , instead of just relying on old data and old cholesterol theory which those of us that have done the research is outdated and false . Excellent point on lifestyle , nutrition , exercise these are the cornerstones to health and longevity . Nutrition is key ,

    My keto-type eating plan working very well thank you. Lots of fish and meat and other good fats like avocado and olive oil, eggs. I eat a lot of greens (mixed salads), and have cut my fruit intake quite significantly. I'm 65, and have been on this plan now for about 18 months.

    Yesterday my "home" taken bloods were 79 glucose( ketones 4.0), Triglycerides 74 but my Total cholesterol 381. I need to find out at least my HDL but am not going near a doctor with a figure of 381, I'm never taking statins...- I've just bought a better meter that would give me HDL/LDL/Total....could this high total cholesterol indicate familial hypercholesterolemia? Been keto 10 weeks with IF and a few prolonged fasts but almost zero carb bordering carnivore...been eating 4 whole eggs a day on this journey could this be raising cholesterol (I know it's not meant to??) I live in Scottish Highlands and getting this CAC CT scan done privately is a task (700 mile round trip to Manchester??) let alone asking the family doc/NHS.... BTW my blood pressure has fallen massively too now 110/59 with RHR of 48bpm...

    Got my labs back from our bi yearly health fair, 25hydroxy D up from 41 to 134, just in time for coronavirus. Thanks for all your great videos, I like this heart doctor. 20000 ius of D3 per day, cutting back to 10,000iu and K2 to reverse my CAC score of 1072. Thanks for your time and great advice

      Do not take K2 if you have stable plaque in your arteries. Calcified plaque is more stable than fatty plaque.
      Over 5,000 IU’s of vitamin D3 daily can cause kidney damage.
      Stay well.

    If I am understanding, If you are not eliminating the excess cholesterol due to FH you may end up with more oxidized LDL in your blood stream?

    I have listened to all of Dr Ali’s podcasts so my comment is based on my understanding of what he has talked about. If they are predicting that you’ll take the drug for 30 to 50 years then with such a long life why are they taking it? Especially with the risks of diabetes and Alzheimer’s -all so that they live an extra week longer than those who don’t take it? It seems to me that a vast majority of people take statins because they think that they can eat whatever they like, rather than choosing to eat low carb. Thoughts?

    Your view on Repatha I have been taking it for a year my ldl is around 70 dr wants it lower after watching this I think it is fine but my sugar has gone up to 150 watching my diet if sugar doesn’t come down I am thinking of taking to dr about stopping Repatha cause I know the sugar is worse than the cholesterol

    An absolute must watch and listen. Most cardiologists in South Africa seem to pro-statin and anti-higher LDL. And the CAC test is hard ever used; I had all my days to get a CAC test done in December 2019. Even the dieticians don't have the facts.

    Sir, my triglycerides is 70, HDL is 102. But LDL is 350 and TC is 550. Is it OK?

      *difficult to answer based only on this report. Need to investigate further (NMR Lipid profile, ApoB, Coronary calcium score by CT etc) and consult an expert physician to make sure everything is fine. Follow his advice*
      1) make sure you don't have the condition called *Familial hypercholesterolemia.*
      2) visit www.cholesterolcode.com and see Dave Feldman's protocol.

    I took my 3rd CAC in less than a year. ON 7/17/19 I had a CAC score of 233. On 10/21/19 I was at 294. On 3/10/20 I was at 260. From experience I can tell you that the Lp-PLA2 blood test is an extremely good predictor of what you will see on a CAC. Lp-PLA2 tells you how much arterial inflammation you have. The CAC tells you how much calcium your body is using to protect itself against the arterial inflammation. Forget about the C-reactive protein test. It's too heavily influenced by things other than cardiovascular inflammation and is too unreliable an indicator for what you really need to find out.

    I was a bit concerned about my LDL watching this. Ivor do you have any data on LDL being too low? I'm currently on 40mg atorvastatin and 10mg Ezetimibe prescribed due to a CAC score of 89 and family history of heart disease. I also take 1000mg Niacin (nicotinic acid, not niacinamide). My diet is now very low carb.
    I was looking over some studies that said low LDL was around 50 mgdl and really low

    I'm in Canada and I have asked my doctor for a CAC since he wants me going on statins. My LDL is high, my trygs are low, DHEA is high, HDL is high. Doc didn't want to give me a CAC but I insisted. Now he can't find a place that does this test, so told me to find one. SMH.

      have been keto for 2 years and went Carnivore Feb 1st 2020. Have decided to reintroduce some veg as my stomach is having issues handling just meat. gotta do what's best for you.

    Great discussion. I'd like to hear Dr Ali speak on whether the type and number of LDL particles matter, i.e. LDL-P, LPa, ApoB; ApoA. If they do matter; what should those on lo carb or no carb who see these markers moving in a negative direction.

    So I had a CAC in mid July on my own just to check. My score was 575. Not good. Since that day I have been doing keto and intermittent fasting. I’m seeing a cardiologist on Friday. I have listened to and read a lot of your information. From my reading I didn’t think it was actually possible to reverse this score. Can you lower your CAC score?

    Could anyone here clarify a vague point for me? Dr. Kendrick says that LDL cannot pasa the endothelium, but I’ve also heard Ivor say that there are papers showing endocytosis of LDL through the artery walls.
    Those two statements appear contradictory.
    Could endocytosis be occurring in the vasa vasorum side instead of the main highway of the artery? Has LDL been actually observed to transcytose, or is it merely an inference that it takes place?

    Ivor... one question: You talked about a "get out" option for low cab for people whose LDL skyrockets doing low carb (myself). All my other blood markers improved. BUT- I am Apoe4. Since you have the Apoe4 gene what do you do? Do you do "Keto Lite" ?? Have you modified your Keto/Low carb?

      @@IvorCumminsScience Funny you should say that about cholesterol manipulation... I had my bloods done 40 hrs into a fast and my LDL jumped much higher up to 276.... that's a record for me. My personal best:) Dr. Nadir Ali mentioned LDL jumping up while fasting... that was all new to me.

    Excellent! My husband had his heart scan. No complaints, he felt good, he fasts and is 30 days carnivore and 2 months keto..... Shazam!!!! His # was 2000...... he will be seeing a Dr this week. Funny thing is the Dr he will be seeing we haven't seen before. My husband had the clinic send him the results. He emailed my husband right away and asked him to look up videos my Ivor!!!! Wow, I think we picked an ok Dr. But...... what can he do for now? What questions should we ask or testing he should have!!!! Thank you so much! Love you both and I feel blessed to have always been digging for health truths!!!!

      K2 MK7, plus vitamin D3. I take Koncentrated K. Ivor had the K guy on last year. I'm 73, pre-diabetic two years ago, but my trigs and cholesterol have always been low. Just had the scan, it was 6 in one artery, 0 in the other three.

      @@IvorCumminsScience have had 5 heart bypasses I followed esselstyns ornish and mc dougall life style could not walk on the cooled in 3 months running 5 kin 3 month between seeing my doctor hospital appointments and finally my surgeon the week before the hope he was not happy at first but when I told him I was off al 10 meds and a symptomatic he said there he would not have the op pressing on a year or so later I got divorced went on a drinking and eating crap the angina came back my mental health suffered and so I was back to see my surgeon had the OP was in a bad way for a while but went and done more research found the likes of ivor and other fantastic low carb doctors and professors I am on low carb dont take meds diabetes gone again feel Brill started NHS 5k run using weights could not life 20 kg now lifting 60kg
      People have got to listen to the likes of ivor it saves lives God bless good luck by the way the low carb is far easier than vegan lifestyle cut out the sugar veg oils all processed foods like proof Robert lustig says and prof Tim noakes the world has to wake up by the way 3 months after the op my surgeon that my arteries where blocking up and there was nothing else's they could do that was 2 and a half years ago

    I would like to see the relationship between COVID-19 deaths vs cholesterol vs statin use. If lower LDL decreases the bodies ability to fight infection, both bacterial and viral, then corresponding data would show this. This is unless the novel COVID virus mechanisms have been engineered to avoid this important biological defense. This also begs the question are antibodies dependent on the bodies production of cholesterol to be effective?
