Steve played with Frank Zappa as a teenager and was considered a musical genius then. He has had a long solo career too. When he recorded this song in 1990...it didn't have an orchestra originally...he fasted for 4 days beforehand to get into an almost trance-like state.

One of my favorite pieces .. so awesome.
This solo is basically note for note the same , all those fast tapping sequences and everything. He sometimes changes it a little between tours… but this song Steve composed the score for the orchestra too. So everything is 100% written as amazing as it sounds.

For the confusion on how everything is coordinated, the conductor very likely had worked out certain queues with Steve beforehand. For example, at the end they could have decided which note Steve would end on and Steve could have given him the signal to queue his musicians to hit their final notes. That way there is room for Steve to have some freedom to improv while the orchestra would know what they're doing.
