Total Knee Surgery - 4 Simple Ways to Reduce Swelling

  • If you've just had Total Knee Replacement (TKR) surgery, you're probably dealing with a lot of swelling surrounding your knee, and possibly swelling into your lower leg, upper thigh and/or your abdomen...
    And this swelling can be the biggest barrier between you achieving full range of motion (ROM) in your knee, especially as the swelling can make your knee feel tight and painful.
    With that in mind, reducing your swelling as early as possible in your recovery is of vital importance.
    In this video, I'll share 4 simple and effective ways you can reduce your knee swelling at home during your recovery from total knee replacement surgery.
    And I'll also share 3 other tips to help you manage any long-term swelling that might be affecting your knee.

    I had tkr on 7/27/2022. First, people seem to be using oxi for pain. Not the best for me. I use Norco, 6 a.m., noon and 6 p.m. I wake up stiff, have my Norco and workout 40 minutes later. I thought the pain was going to be 10x more than I have. Yes, it hurts. No joke. Pain tolerances are different for everyone but you need the right meds. Stay on schedule and sleep as much as possible. I'm doing basic flexion and extension exercises 2-3 times daily. I walk 3 walks total of 1000 ft./day. Don't be a super hero. Consistency is the key.

    I had TKR on 9/29/22 and it had been a game changer. I love Shawna and Joe on my PT team at your office. They have been compassionate, professional and given me the support and guidance for ongoing healing.     Hello everyone, I had surgery 07/11/22, it's been a week exactly and I keep hearing" it's going to get better, just do the work. I am a praying, believing, trusting in God person, but right now I am miserable. My leg is so swollen 😫 and I'm doing my exercise bending, all of those things. Sometimes I want to cry about what I have done. I was in pain before surgery and now I'm still in pain. I know I must be patient and obedient. Slow and study but I just had to get this out. You pray for me and I will do the same for you. Thanks for listening.

      You're right, Miriam: it will get better! Do your best to keep the swelling down, and if you're taking pain meds, do your best to take them as prescribed and stay ahead of the pain. You'll get through this :-)

      Surgery 8/4/22, hurts beyond anything I’ve ever experienced (74 yrs). Swelling is tremendous, though I’ve used ice water circulating. 🥴🤕🙏🏻🙏🏻🙏🏻🙏🏻

    Im only 59. Had Right TKR last Tuesday. 5 days post op back home. Yes painful but taking it day by day. Listen to your body. It knows when your tired

    I had my left knee done yesterday. The pain is ridiculous. Unfortunately the oxy they gave me isn’t helping all. Good luck to everyone on your recovery

      Hello, I'm 12 days post surgery on my left knee. I invested in a Cryotherapy machine along with pain management. Staying on a schedule with the pain meds really helps. However I have felt that horrible pain. Hope your doing better now.

      Lucky... My surgeon would only prescribe hydrocodone. What a joke!! The pain was terrible.

      I’m in the same boat…agonizing pain. Why won’t our surgeons deliver better pain management? What would be wrong with a pain pump for first day after surgery?

    I got my knee replacement on Wednesday! This pain is excruciating but I've been doing the ankle pumps!

      I just got a tkr almost 3 weeks ago and the swelling is awful 😖 but I know the more I move the better I feel

      I am suffering with my 2nd knee replacement. It is very uncomfortable but having had the other leg done a year ago, I know it will improve but slowly. Do the exercises but don't over do it.

      That was the worst surgery ever! I even stopped going to physical therapy and started rehabbing myself at home... The therapist was hurting me too bad!

      Some great advice here . I have the Berg machine for iceing of knee . Can not imagine trying to manage swelling with out it . Keeping swelling at bay is critical to reducing pain. Combined with PT .

    One month out from a TKR and going along good. I am using a CTC-7 Cold Therapy Machine and two PlasmaFlow compression sleeves which both work great. I use a 1 1/2”-2” paint roller to massage my leg which works a lot better than a kitchen roller pin. I highly recommend doing all your exercises before and after your TKR and staying on top of your meditations. Stay ahead of the pain !!!! Thanks for the informative video.

    Hello ! thanks for your information. I got ACL problem in football 7 months ago. The doctor told me it's half tear so you don't need for surgery. I went through all advice of doctor (not walking, cover it for 20 days, and more.). Now I walk, cycling and doing its exercises to strengthen muscles, but I still have little swelling in my knee without pain. As per your information I should not worry about it. it may take weeks, months or years.

      Hi, Peer. Yes, you'll probably have some minor swelling for several months. However, I'd still do you everything you can to reduce it, including the tips in the video. There are also some good k-taping techniques for swelling.

      Again, sorry to hear this! How's your PT going so far? You might want to find some lymphedema treatment if your leg is really swollen.

    I had my Left Knee replaced (Conformis) 10/12/23, I just had my Right knee replaced(same) 4/12/24. I purchased a Icing machine off of Amazon that I use frozen bottles a water. My main issue with both knees has been spasming when trying to sleep and the swelling. I purchased an exercise ball and put a towel over it. Used it to elevate my leg 2-3 times a day. After 6 months on the Left Knee only residual is tightness. ROM is 138 degrees.

    Needed this. 12 days post op, had chronic knee pain last night, woke up to swollen ankles, never had b4. Think I overdid physio exercises yesterday, makes sense. The four major tips are v helpful. Thank you kindly.

    I'm about to go through partial knee replacement, would you recommend the same wedge pillow for that? It could be full replacement, they aren't sure yet; won't know until they get in there, I'm hoping for the best.
    Any advice for partial replacement?

    I made some of my own ice bags with rubbing alcohol and water placed in a zip loc bag! I also like that they add additional weight to press on my knee to help with the flexing!

    I just had a TKR a week ago and did really well on my 1st PT session. On my 2nd session, at 4 days after surgery, I did 10 complete reverse revolutions on a stationary bicycle. After I got home, my entire leg swelled so bad and was so painful I could not bend my knee at all! That was 2 days ago and it's gone down a little but it feels worse than the day after surgery.

      I'm sorry to hear that your swelling got out of control! Have you been back to your surgeon since your swelling worsened, or are you still doing physical therapy?

      @@GordonPhysicalTherapy I did go back to the Dr and he determined that I tore the muscle. I have not returned to the center but I have been doing a bit less intense exercises from home, as instructed. . It has gotten a little better every day since. It definitely threw my progress back somewhat. I know now that I have to listen to my own body and know when I can and cannot push. Thank you for your response 😌

      Gosh it’s early to be going to the gym - maybe too much too soon? I’m dreading the harder stuff - a week after surgery I’m still trying to bend/straighten my legs

    How high does the wedge need to be for proper elevation. An Amazon link would be great. For best results, should you lie flat without a pillow when using the wedge? Just over a month following TKR and my knee is about 2" larger than the other. I am using ice, doing my execises, but my range did not improve this week and my therapist had to "work on my knee", causing me a great deal of pain. I am beside myself with pain. On a sidenote, I can't take pain meds due to nausea.

      The higher above your heart you can get your leg, the better. Lie flat on your back, with your knee as straight as you can tolerate comfortably.
      When your PT "works on your knee", do they help massage out the swelling, or are they just stretching you?

    I had my TKR 5/19/23. I'm dealing with stiffness when i get up from lying down or sitting. I know it is a process but my swelling is what stops me from bending. I try not to over-do exercises but I'm just tired of being stiff. Especially in the back of my knee

      What you're describing is completely normal at this stage in your recovery! Just working on your stretches, getting a good balance of activity/walking, and trying to keep that swelling down!

    Hi, liked your video. i just had knee replacement surgery on 11/30 and I started PT 12/1. It was a little tough, but not too bad. I am elevated that leg but not enough. I di better with that today (Saturday). Everything feels pretty good believe or not. I actually have very little direct pain with the pain, it will however, ache sometimes and I have medication for that.
    I would like to ask a couple of questions, my pain to me is similar to having had a rigorous leg workout with leg extensions. I would like to know what (along with everything else that you mentioned in your video that I might be able to do for the quad soreness?
    Also I have some pretty extensive bruising on the inside of that thigh and on the inside of the same ankle. I am doing my exercises at home, but I am not overdoing it. so what can I do for the bruising?

      I had total knee replacement on 12/14/2021 and I listened to your 4 steps and it made more since to me. I have knee swelling and can’t bend my knee much, in fact my PT therapist had me on the bike doing back and forth pumps which I believe was to get the flow going. It hurt like heck, and I’m only taking Tylenol for my pain and was wondering if maybe I weaned off my pain meds to soon?

    Very helpful. I had knee replacement 10 days ago and I'm active and doing everything I should -- icing, heating on the back of my knee, walking, etc. I have a good PT, but she's tough on pushing me through the stiffness. It's hard to keep my leg straightened and I find I stiffen up fairly quickly. I've had two hip replacements and recovered quickly. Before my surgery I rode a bike 20 miles every other day, skied (even with my bum knee), hiked, swam, and am in good shape for a 73 year old. Still, I'm confused by the stiffness that just seems to invade my leg the minute I quit moving it. Am I just being impatient?

      Hi, Mark! Everything you said sounds completely normal. At 10 days out, that stiffness that settles in so rapidly is completely expected, and it can be very difficult to get your knee to straighten out at first. So just keep plugging away! And listen to your PT's advice about finding the right balance between pushing your stretches/activity level and allowing for adequate rest/recovery time. Best of luck!

    Was doing great, but 4 days post surgery tripped 4 days on my walker and strained/tore MCL. Knee and entire leg swelled 6 centimeters and extensive bruising resulted. Now in brace for 2 weeks to be worn whenever I am out of bed. Balancing PT without hurting that part of my knee an obstacle. I am 13 days out from surgery now. It is resolving, but all this has has prevented me from consistent attempts at bending. So wish I would have seen this before hand and done a lot more research. Thanks for video. If you have any advice would love to hear.

      No pain, but apparently now the Dr has said the plastic plate between is a 12 and should have been 14? Knee is unstable

    I had 2 knee replacements one about 3 yrs ago and the other a year ago..Both knees are still swollen and do hurt some..I had fallen twice and landed very hard on both knees..I landed on cement both times and had 4 fractured ribs and bruising and a cut on one knee and small cuts on my leg and hand...My left hand was swollen for short time..And I had x-rays of both knees and it didn't show any damage to my new knees..Couldthe swelling have come from the fall and how long will thus swelling last?

      Hi, Pamela. It's possible that the swelling came from your fall, although swelling after TKR surgery is pretty common. Either way, do everything you can to reduce it. I hope this video helps!

    I had TKR 4 years ago and I’m still having bad swelling all round the patella after walking about 1 mile on the flat. The swelling is all around the patella.

      Sorry for the late reply... your surgery is coming right up! My advice before surgery was basically to follow the 4 tips and also to work with a lymphedema-certified (CLT) PT as well. I hope your surgery goes well!

    Gordon done my surgery 7 weeks ago on patella tendon rupture for 6 weeks i hav just sit not making any movement fo 6weeks....on th 7week im trying to bend my knee and its very hard and i can only 10 degree

      Yes, after being immobilized for that long, it's going to be a slow and gradual process to get your knee to bend again. Hang in there, and you should gradually improve.

      Hey, Camy! Lots of ice and elevation, as well as compression and massage. After your manipulation, make sure you stretch your knee AGGRESSIVELY!

      As long as you have swelling, I would continue to elevate and do everything you can to reduce the swelling.

    I had both knees replaced last summer, 8weeks apart. The L knee was done exactly one year ago yesterday. I’ve done beautifully. However, today about 3pm it began to feel stiff and an hour later I couldn’t bend it enough to getup the steps to my porch!!!I iced and elevated it, I assuaged from my ankle to my groin. It’s a bit better but oh my this is miserable!!!!

    My knee & ankle are still swollen twenty months after I fractured my patella, comminuted fracture, trauma surgeon, P.T for 8 months post op, 3X per week.

    I can't wear the compression hose because of damage to the shin just above the ankle either by something done during the surgery or by the hose. I had to quit wearing them two weeks out because the pain in my shin was greater than the pain in my knee. Do they make a hose that starts half way up the shin? Thanks for the video.

      I'm sorry to hear that! No, I don't believe they make a compression garment that starts above the shin, as that would essentially trap the fluid in your foot and ankle. They do make velcro wraps from a company called Juzo that you might look up though...

    Post op 1 wk. Im a "young TKR patient. I had fractured tibia, st 4 osteoarthritis and a knee that would subluxate itself.
    I have been in terrible pain . Swelling is terrible. I have a cold therapy ice machine. I switched the attachment for the cold therapy machine to the pad designed for shoulder instead of knee. I did this because the shoulder one is large enough to wrap all the way around my leg. It cools the leg all the way around as opposed to just cooling the top. I elevate on a wedge pillow. I have greatly decreased the PT walking and other things because i was doing too much and now im paying for it.
    My question is.
    How do we sleep?! I will get my cold therapy going and it will help reduce the pain and swelling. But. I cant sleep w my leg elevated. I take it down from the wedge to sleep.. i wake up in pain because i moved in my sleep and all the swelling is back.
    What position can we sleep in? What alternatives are there for positioning for comfort ?

    Did you ever come up with an ice machine you like that we could buy to help with swelling after knee surgery? I did not see one listed, only the wedge.

      I received one at my surgery at University of Florida's Shands Hospital for TKR. It is a Donjoy. Put ice and water in and it pumps to a pad you use which circulates cold water.

    Hi Gordan,
    My mother's had total knee replacement surgery past one year. However, still knee is still swollen and she is facing pain near the left and right side of knee.. Dr. Said it might be because of bone weakness and said to walk more and do exercise.. Please can you assit if this is related to nerves or something or we can take private PT or any exercise massage can reduce pain and swelling..

      If your mom still has swelling and pain, I'd look into having someone help break up the scar tissue around her knee. I'd also consider aggressive stretching if she doesn't have full range of motion. Do you have access to someone like a PT who can help?

    It is now just over 18 months since I had my full knee replacement operation. My ROM is good but walking up and down steps is painful. I have done all the exercises etc during my recovery and can bend the operated knee 130 degrees. I can even walk the golf course without too much trouble. When I put my knees side by side the operated knee is slightly larger than the non operated knee. The condition is improving but it is very very slow. Walking up steps is easier than walking down steps. What is the probable cause and are there any exercises that I can do to help ease the problem.

      Thanks for getting back to me.
      The pain is just below the knee . Mainly on the right and left hand side.
      When walking up steps the pain is present when I put my weight on the leg.
      When walking down steps the pain is present (right knee) when I am lowering my left (non operated) leg. Hope that makes sense. On a scale of 1 to 10 with 10 being the highest degree of pain, I am experiencing a pain level of about 4. It's not excruciating pain. I suppose I could live with this pain. The more steps I climb/descend the pain gradually increases.

      No. Ultimately, you should try to get to 125-135 degrees of flexion (knee bend).

    My surgery was 2/22/22 ...I honestly can say I've had near ZERO pain after my surgery ... the arthritic pain I had prior to my surgery is for sure gone. Biggest issue is swelling & stiffness 😫 now using a CPM machine so let's see how this goes lol

      You're still early in your rehab, so hopefully your swelling and stiffness will continue to improve. And if you feel like you've plateaued, consider working with a "lymphedema certified" PT to help with that swelling. Good luck with your recovery!

    They do say that knee surgery is one of the most painful. I had partial double knee replacement 8 days ago and the pain is excruciating so I can’t imagine the pain from a full one!

      It's normal to still have some amount of swelling for 6-12 months. As long as you're still making progress with your pain, ROM and overall activity level, you should be on the right track :-)

    All these pain comments freaking me out a bit. My surgery is in two days in my left knee. I invested in a cryotherapy ice machine and surgeon applies it immediately after surgery. Hope it helps with the meds. Day surgery and a three hour drive home. I’m nervous.

    I had my left knee replacement surgery 1month ago ,but my pain is unbelievable 😭,i continue my PT everyday after PT session my pain become more .and also swelling.i bend my knee almost 90 degree .
    Plz suggest me some good tips to reduce pain😭.

      I'm sorry to hear that! I have some suggestions for you: the first is to ask your doctor if you're in need of a manipulation (aka "manipulation under anesthesia"). I have a video about that topic if you want to check it out. My other thought is that if your swelling is really bad, you might want to find a PT who specializes in swelling and lymphedema. They're called CLTs (certified lymphedema therapists).
      One last thought: if you're cleared for it, you might consider aquatic therapy for the pain and swelling. I hope this helps!

    I had a total knee replacement 0n Nov, 2…on Nov, 3 it dislocated….2 nd surgery nov. 3…in brace for 6 weeks….first knee no problem…. Any one else have this wierd thing happen…

    I had a tkr and it has been 12 weeks out an I am having swelling and stiffness in my knee. It feel like a band across my knee were it want let me bend it. Can you wear the compression to bed at night.

      Yes, you can wear compression at night, and you can also elevate your leg, as long as you can sleep.

    I had knee replacement 4 weeks ago and l m having so much muscle pain. No swelling but this muscle pain is not let me sleep.l take oxicodone and just takes half of the pain ..is there any Exercice or stretch l can do?? My pt is making me do the normal exer.but not working .tia.

      Where is your muscle pain located? It sounds like you might need someone to massage your sore muscles to help them calm down. Does your PT do any hands-on work with you, or only exercises and stretches?

    Could this be Lymphedema?

      Yes, this could be lymphedema, or it could just be residual swelling and inflammation. Is it just your knee that's swollen or your entire leg?

    I’m almost on my six month, and my thigh still feels stiff and atrophied, knee is still a little bit, swollen, warm, and tender to the touch.
    I can’t believe the stories I’m reading about people during the first week or two of post surgery! I guess I was really lucky because I had no problem sleeping and took Percocet every four hours the first 10 days. Was also prescribed flexeril, Valium (never did anything for me), stool softener, and blood thinner. Guess I was lucky

    My doc say I was one in a million that did not have any pain after 6 visit i was at 120 range of motion thank god for my son he gave me my med on time and we kept icing my knee
