Any medication can cause any side effect, so using the medication carefully and starting at a low dose will be helpful. Consulting the prescribing doctor and informing about the side effect is also very important.
Hello Doctor, very informative thank you. My dermatologist prescribed me 2,7mg sublingual minoxidil. They are like little squared jellies that dissolve under the tongue. I've been taking them in conjunction with 1ml a day of topical finasteride 0,2% + minoxidil 3% + FGF growth factor + actiana in trichosol solution.

Do you also think this topical solution of finasteride 0,2% with fgf and actiana is still as effective as taking oral finasteride but reducing also side effects? I have experience in oral minoxidil and feel that it is quite effective by itself without the need to use sublingual minoxidil. Topical finasteride is not available in the UK but is used a lot in India where practitioners feel it is very effective. A combination of oral minoxidil with topical finasteride is used in India with good success.

Hi Hasan, the tablets that we have now are make to be swallowed. The sublingual tablets (under the tongue) are make only in some countries and I am not sure if there are any where you live. You can try and crush them and use it under the tongue, but the dose needs to be lowered to 1mg as that was the dose mostly used by Dr Sinclairs team in Australia. The current tablets or oral minoxidil are best swallowed as thats what they have license for.
But doctor assuming a person non responding to topical minoxidil due to lack of sulfotransferase enzyme , sublingual will skip the liver for metabolism. so does that mean sublingual is going to be non effective for topical non responders ? As you said the activation of sublingual doesn't happen in liver unlike oral form.
Very good question. The way sublingual minoxidil works is by creating a very high concentration of minoxidil sulphate in the hair follicle, which will reach it through the blood stream. Unlike topical minoxidil, where only a small amount gets into the follicle, the sublingual reached a much higher concentration and also avoids the first pass metabolism. As there is more minoxidil is the hair follicle, even a low amount of sulphotrasferase will create a higher concentration of the active molecule in its vicinity.

From my reading of the literature the drugs used sublingually tend to have a different structure compared to that used orally. Not sure if an oral drug will the the desired efficacy compared to a sublingual one. Let me know how you get on.

At present, sublingual minoxidil is available only in selected institutions like the one in Australia where Prof Rod Sinclair works. The oral agent is unlikely to work sublingually. The dose of sublingual minoxidil is also very low compared to oral, about 0.1 to 0.5mg. It is not available in the UK, where I work.

They are different. Sublingual minoxidil is only available in some centres - in Australia and maybe the US. There are no commercial producers of this in the Uk or India yet.

As mentioned in the video, oral minoxidil tablets do not dissolve under the tongue every well. So the amount absorbed is not stable. Sublingual minoxidil is specifically made and available only in a few places. Think Prof Sinclair team is the only one who do it for their research. Hopefully it will be available soon to a wider population once the research findings are confirmed.

You are right, there may be a more beneficial effect with oral rather than topical minoxidil. It may be useful to continue with topical minoxidil for about 4 weeks and as the oral agent starts to kick it, stopping the topical minoxidil could be considered.

That is a combination many dermatologists use, so it is worthwhile continuing as to takes about 4 to 6 months to have its full effect.

Yes, we can use low dose Minoxidil with the medications that you mention. Start with a low dose and gradually titrate up, as mentioned in this video

They are different. Sublingual minoxidil is only available in some centres - in Australia and maybe the US. There are no commercial producers of this in the Uk or India yet.
Hello doctor,I have been using topical minoxidil for almost 3 year(daily 1ml dosage) because of hair thinning.However,i recently moved on to oral minoxodil 2.5mg,first started with 1.25mg for 20 days,then changed it on to 2.5mg few days ago.Sir,as you suggested to continue both oral and topical minoxidil for first 1 month,i have followed this rule, but now i am having increased shedding than before.I don't know why this is happening.What should i do? should i stop topical or is there any oral dosage problem?help me sir!😢
The times span that you mention for oral minoxidil is very short. We need to use to for at least 4 to 6 months before it fully works.

As mentioned in the video, oral minoxidil tablets do not dissolve under the tongue every well. So the amount absorbed is not stable. Sublingual minoxidil is specifically made and available only in a few places. Think Prof Sinclair team is the only one who do it for their research. Hopefully it will be available soon to a wider population once the research findings are confirmed.
why came hair end white bulb?
Regrowing hair is often white, but in many instances, like in alopecia areata, they become darker with time. The exact reason is not known.

@@srijayasrijaya9590 Sometimes there is increased shedding when we start minoxidil. Need to continue with the tablet for at least 4 months before any change is noted

You are right, there may be a more beneficial effect with oral rather than topical minoxidil. It may be useful to continue with topical minoxidil for about 4 weeks and as the oral agent starts to kick it, stopping the topical minoxidil could be considered.

To use any agent sublingually it needs to be specifically formulated for sublingual absorption. A normal tablet is unlikely to be beneficial.

Hi sir , I'm from Hyderabad India , I'm suffering with HS from past 3-4 years
First treatment of isotretinoin was discontinued in half way due to health issues in 2020
But this time
2nd treatment of isotretinoin was successfully completed a month ago - 2023 ( 150mg/kg )
But My HS still triggers in armpits as usual .
I really want to consult you , but I got to know you are living in UK .

So sorry to hear about your skin condition. There are quite a few options that you can consider for the management of your skin condition. I do not know anyone personally in Hyderabad. I do not have any clinic in India either. I enclose below a video on HS which I did last year. You can ask your own doctor to consider any of the treatments that I have mentioned in the video.
