PRI Skill #1: Using Right Glute Max to Unload Right Side

  • Learning PRI effectively requires skill acquisition.
    If you know anything about PRI, you understand the emphasis on strengthening the left side of the pelvis with the left hamstring, adductor, glute medius, and interal obliques.
    What people often don't realize is that the ability to actually shift your center of mass to the left requires activity of the right glute to actually get you there.
    If you can't "get off" your right side, you can't effectively "get to" your left side.

    Brother, this is EXTREMELY helpful. I've been getting assessed by a PRC for 9 years and your two PRI skills SIGNIFICANTLY helped me to understand PRI concepts better than I did before. You truly are a master teacher. PLEASE PLEASE PLEASE keep doing what you're doing.

    Neal, for me the issue is reversed, but when I put the closest comb I had between my right teeth for more contact, the problem was immediately 80% gone. And the part about loading the glute on the dominant side to 'get off' that side is unintuitive but so relieving. I've had countless chronic pains from head to toe for many many years and I'm only 25 - NOTHING has helped, including quite a bit of physio, and now I know why, you are literally the ONLY person to ever talk about this to the point where I was convinced nobody was in the same position as me. This is really unreal, thank you.

    It is amazing how oblivious i was to my lower posture. It is only yesterday i saw how my right leg was out of alignment with outward tibia torsion and inner hip pronation. This is the problem , the source of my bum knee . I am now more aware of how tight and high my arch is, and how unstable my foot is.
    Thanks for imparting your wisdom.

    Your videos have given me some hope, been with a regular PT for 8 weeks now and my pain has not improved. Found a PRI person on the page you recommended in another video. Have an appt with a therapist in 2 weeks. Very hopeful they will be able to help.

    Hello Neal, since i watched your video explaining the left AIC left pattern i've been interested in what you have to say on PRI techniques. I can feel that my pelvis is not positioned correctly. But the thing is i can't seem to shift to the right side. My left adductor is the one that's tight and i have a hard time abducting my left leg (the opposite thing occurs for my right leg). And having a chronic knee pain in both my legs, plus lower back pain, SI joint pain, i decided to try the program on your blog, and from the first go i could feel my left knee repositionning (my femur was actually rotated internally and my tibia externally). I can't thank you enough for sharing these techniques with the world. I'm truly amazed !

      Glad to hear it!
      The "tight left adductor" phenomena is an interesting one. The left leg is *generally* resting in an abducted and externally rotated position, so the adductor is probably not tight. Perhaps "tense and stressed out". The right adductor is usually tight. It all depends on the compensations occurring with the femurs.
      Anyway, just keep in mind that the beginner program is just that, a beginner program. Most people need more than that.

    Since I have been more mindful of making sure my right foot goes all the way through pronation, I can feel it in my glutes and in my anterior lower leg muscles. I also have a bunion on my right foot and I was avoiding stepping on it and so that was causing me even more to not go into pronation. But I find if I do the heel strike on the absolute back of the heel and roll through the step, my foot doesn't hurt.

    "Not sensing or detecting" pronation or supination... 🤔 This brought back to mind a recent experience in roller skating, after a few years of not-skating. I put my roller skates on at the rink for the first time since pre-pandemic -- the skates may be too small for me, I've had them since late teens and by now, they're quite snug; I had the bearings replaced a few years ago -- as i stepped and rolled onto the smooth rink floor, i physically felt my right foot change or shift its "shape" inside my skate. The medial metatarsal of my right foot voluntarily pressed downward independently of the rest of my foot. I can intentionally replicate the motion with both feet by selectively pressing the metatarsal-hallux joint toward the floor without rotating my ankle or the rest of my foot inward. I can also use my toes to pick up small items off the floor -- it's a good exercise for foot and toe mobility. Being aware of your own body's ins-and-outs is important for the healing process.
    This also reminds me: Rollerskating, in particular, has made me notice mechanical imbalances more often than most other activities.

    Would it also apply to people who had a right ankle injury? My right foot is actually flatter than the left, hence longer. I feel tension on the outside of my leg from the bottom to the top of my hip. Would it be still lack of pronation Neal?

    I have a supinated left foot and a pronated right foot. Sometimes my right foot feels over supinated. Compensation is a pain

    Neal with the left aic and the right leg, is the leg/femur actually internally rotated where the foot could still be externally rotated or a duck foot? I truly believe I have left aic but my right foot tends to externally rotate vs my left foot which doesnt. Maybe there is more going on at knee or ant/pos tibialis and with that mot sure. Great video thanks.

      Yes, the right tibia often ends up externally rotating compared to the internally rotating femur, and thus the right foot may turn out to align with the ERd tibia.

    So what are the exercises i should do? Do i look up side lying exercise for right gluteus maximus and do that? And are there exercises to help sense right arch? Or is it just about putting mental focus on pronating the right foot when i do things?

    Hey Neal, I think I have figured out the way to untangle the left AIC pattern. I think the left AIC pattern often comes as a byproduct of a dysfunctional SI Joint. PRI technique and glute exercises can reactivate the SI joint

      I think you're right about this. For me, I have so much pain in the left si area where the QL attaches to the illiac bone. Were you able to figure out how to fix it

    is a consistently externally rotated right leg tibia indicative of being stuck in a right aic right bc pattern?
    when i walk i feel my left leg internally rotated and "reaching" across to the right side while my right leg is externally rotated and abducting then going forward in a curved path

    Neil, I also emailed, I really would like to come see you things are getting worse. My rib age is the main issue but it is affecting everything. At any rate, if you see this, my question is if I strengthen all the things you mention on the left doing your suggested exercises, can I still use other (not pri) exercises in addition to strengthening my glute med, adductor and hamstring. Also do I try and strengthen the right glute? I’m not understanding that? Thank you

    I can’t stand on right - that leg is weak because side I had sciatica and disc pressing on nerve root and in spinal canal.
    Whole right side contracted and twisted to right.
    But right leg internally rotated..
    When I try to lie down on floor with knees bent right buttocks are forced into floor as is right foot.
    Contracted in right neck as well.
    Had flat arch on right but now both arches have collapsed because muscles atrophied from severe Benzodiazepine Withdrawal and Neuro trying other strong muscle relaxants none of which work.

    Hey Neal, just came across your videos and they're very interesting. My right foot pronates a bit due to chronic ankle sprains/instability, this issue ended up causing a posture misalignment on my right side. I know you recommend orthotics but I was wondering what you think about barefoot shoes and proprioceptive insoles?

      In general I'm not a big fan of barefoot shoes unless you spend a lot of time on natural earth. If you spend most of your time on concrete and flat floors, they don't work well.

    I've been trying to fix what I think is a hamstring tendon issue with classic eccentric exercises, however it's been months and still it isn't quite right. I wonder how PRI takes this issue because this is one thing I haven't seen you make a video on. I never remember ripping my right hamstring so I don't really know how I got it

      The right hamstring is being overused. If you lose your left ZOA and right glute, you never "unload" the right side of your body, and your right glute is too weak to ease the burden.

    What if you're better on the right? Yet higher arch on right than left, yet right shoulder and neck pain, but always pull both calves when running. Long time runner here, but whatever imbalances i can't figure out are certainly causing me to no be able to run free

    my right foot points outwards when i walk (duck feet ), When I lie down my whole right leg seems super rotated to the right, while the left one seems normal.
    The problem is that I do not know if I have a tight right hip external rotation or a left interior pelvic tilt !?

      Unfortunately I have no answer. Unless I'm working with someone in person it's hard to know what someone is dealing with. The exercises I have on my site can still work regardless because you'll still have a left AIC pattern underneath whatever else is going on.

    Neal with the left AIC and the right stance where the right femur would internally rotate with tight adductor is there a good chance that I would have a tibial torsion what would then externally rotate my right foot? This right duck foot is certainly a visible thing I notice. It seems to be getting better as I've been focusing on PRI exercises but really just wanted your input if you see that as a fairly standard dysfunction. Any help is appreciated.

    Hey what do you think about squatting on a plate under your left leg.. I go straight to the right the second I start squatting and my right shoulder is rotated in and up.. All the left glute work in the world feels great and I can feel my glute working but it completely switches of again as I work out.. I cannot get into my left side with exercises, only when I stand on the plate.. My left zoa is just completely inactive.. I only feel something squatting on the plate .. I find side planks useless because its just obliques and I have real trouble breathing into my left side no matter what position I take up.. Sorry for rambling but I'm 6 months in and had just a little improvement and it's so frustrating.. Right shoulder still winged and internally rotated no matter what I do

    Interesting...I have known for a long time about my right arch issue. I have been in minimalist shoes for 4 years. My right foot has spread wider nicely but my left arch is higher.

    This is me, my pelvis very high on right as is my shoulder. The pinch and pain with every step on the right feels like a dagger.

    I was wondering, as a pri professional, what you thought about zhan zhuang or tree standing meditation? Basically holding a symmetrical, structurally solid posture for extended periods of time while consciously relaxing the soft tissue the entire time. P.s didn't mean to over explain in case you've already heard of it.

    Neal can the PRI orthotics really help the right foot with pronation and help it stay that way so its not a 2 step forward 1 step back kind of progress? I ordered mine and i feel like it will help immensely because i feel like after i do the exercises my body reverts back to the left aic pattern

    Thank you Neil. Every single time I've practiced a sport I've hurt myself irreparably. I even managed to get a permanent bursitis to my left shoulder after one month of swimming! My left shoulder is useless. Two months ago I picked up playing darts and here I am today with a severe inflammation of my top right shoulder tendons. Some specific moves are now impossible. Playing soccer 20 years ago got me a hernia in the lower back that to this day forces me to bed for two to three weeks straight every few months. The only helpful people I've seen are osteopaths. They're magicians. But the shoulder issues seem beyond their ability, at least in my case.
    All this to say: do you know of PRI qualified practitioners in Montreal, Canada? If not, do you offer Zoom consultations and training?

      Are you hyper mobile? Obviously just stab in the dark but general bodily injuries from sports could indicate some hyper mobility spectrum disorder

      @@TT-id3dp I just did a self-test with the Brighton scoring system and it's not positive except for the pinky finger. The real issue is inflammation. Runs in the family. Maybe related to gluten. Jacqueline Lagacée in Québec did an extensive research showing that years of eating wheat affects the permeability of our small intestine, which leads to prolonged absorption of wheat in our bodies where it shouldn't go. Some people end up with issues early on and for others it can take years, but eventually wheat/gluten causes this according to her. Could also be other causes. It's been 7 months now with a very painful frozen shoulder (capsulitis), which is a different problem than the one I listed a year ago here - and much more painful. Three cortisone injections had almost no effect. I'm thinking of going to Columbia (the country) for a stem cell treatment. Quite epensive - 30K, but something needs to be done!

    This information is like the holy grail of relics 🙏 any chance you help me understand why I have all the symptoms mentioned within your video but for me... my right foot is pronated and my left foot is supernated, wich is the opposite to the diagram in this video?? Again your information Is a rarity in his day in age 👌

      From my experience, a right foot that is in a more "pronated" position, and a left foot that is in a more "supinated" position, is likely due to compensations for the underlying pattern. Sometimes people are so far to the right, that their upper body has to try and "fall" back to the left to prevent themselves from falling further to the right. This can lead to the "pronated" look of the right foot, but it's not really "pronating". You're just looking at a compensation.

    Neal, I believe I have lost the ability to stand properly on my right foot. Could be several reasons. Right leg is shorter by at least an inch and I also had a Compound fracture of the tibia in this leg when I was 9. Currently 39. To the eye, you can see my hips off.

      Being at least an inch difference is really significant. A doctor would know best but you may need something more extreme like limb shortening on your left, platform shoe for your right, etc.

      Because i dont think we can feel right posterior glute med in left sidelying right glute max exercise.

      @@khachaturdavtyan5144 the posterior glute medius works during the glute max exercise bc it helps with abduction and external rotation, same as glute max

    I have a higher left hip and cannot stand on my right leg at all. I load everything on my left side. I have so many twists and turns in my pelvis and dealing with constipation because of it. Even bladder issues.

      Some people do stand with their weight on their left side, but their pelvis is still oriented to the right. I made a few videos about this confusion. You aren't on your left leg in a "true" biomechanical way. It's all compensatory. I was one of those people, too.

      I think that is an indication of compensatory activity/instability higher up the chain at the pelvis. It's not normal from a PRI perspective.

    Why do i lose right glute so fast? After I do the exercises I can feel my left glute is working because I get thrown to the left. I lose this very quickly. By the end of the day I can feel I've almost or have lost the pattern. Also I can sense my right glute throwing me to the left but then coming back to the right it's almost if I kinda fall to the right again. Something is missing. ?? I had no right arch until I did the paper towel trick for a few weeks, that got me pronating on my right foot so I can sense the ground now. Have you seen this?

      dude i relate so much, i go out walking and suddenly it clicks and my body walks normally. then i go back to a weird walking pattern the next day

    i have this EXACT condition.
    it's gotten so bad that i developed "duck foot" on my right side.
    I guess its my brain wanting to feel that pronation and also adapting to my twisted pelvis.
    sitting is a nightmare for the human body!

    My Pelvis is clearly orientaed to the right side. i Can see it at the mirror. my right adductor is tight too, but my right foot is too much pronated.. when we compare woth the left leg, why is this so.. I can also feel that my right glute ist smaller than my left glute..

    if you have SIJ dysfunction and an anteriorly inferiorly rotated RHS pelvis do you simply do the exercises to the Right side instead of the left to restore balance? I have chronically tight L Adductors /Pectineus/Pelvic Floor/Psoas with compensations on the RHS to confuse matters LOL

    I’ve just started watching your videos and for some reason I can’t figure out which side of my body is causing my issues-right shoulder issues and left periformus pain. I’m a Pilates instructor so I’ve got the mind body connection down, but when it comes to this I seem to be out of touch with what’s happening in my body. I also have hyper mobility in my shoulders, elbows and wrists and when I turned 40 almost 2yrs ago I began to have issues stemming from my abnormal “flexibility”.
    I’m right handed but my left side has always been stronger and far more flexible-I cartwheel & do splits to the left and recently I’ve noticed that I have far better balance when standing on my left foot. I almost can’t stand on my right foot without wobbling so I know something is up.
    My question to you(if you have the time to answer:)is what side should I focus on…my left pelvic mobility or the right? Thanks so much and I’m loving your videos🙏🏻

    I hope this is my answer. My right eye has always been 20/20. My left eye is nearly blind. I walked at seven months old and in the pictures of me walking at nine months shows me loaded onto my right side. At three years old the doctors told my working poor parents that I needed corrective shoes and braces on my feet and legs. Since I was active and agile they decided to not address the problem that they couldn’t see. I’m 62 now with debilitating tension in the back of my head and after the right side was done 2 years ago, I’m waiting for my second hip replacement, this time the lazy left side.

    so if i usually stand on my left leg, would the solution just be the inverse? I've visited a chiropractor who said my left leg is "shorter" than my right because of the position of my spine and pelvis, but adjustments didnt fix the problem long-term. it isnt actually shorter, its just the way my spine and pelvis are situated. is focusing on left foot pronation a good place to start with fixing that?

      Anything that isn't too soft. Other than that, I don't know. I haven't found anything that has made a difference in my sleep. Sleeping is definitely the position that is most stressful to my body ironically enough. My back is tight every morning I wake up until I re-ground myself.

      @@Sunshine.500 thanks. It's a small price to pay if I feel good the rest of the day. It is ironic, though: in the old days my whole body hurt but sleeping was fine. Now my body feels good, but sleeping is problematic!

      That can happen, it's just not the most normal situation. I don't know what you should do since I'm not examining you. You may have to seek help.

    Its very difficult when I wake up. WHEN I STAND UP FROM MY BED, OOOOH😨
    I can't walk at all or walk heavy or walk ok.

    Almost all older men develop varus knees. That pushes your feet into supination which is bad for right side but okay for left in a Left AIC pattern. Having to push the right knee takes a lot of effort to overcome the varus angle. Correct?

    Very odd situation: Its very hard for me to walk. Finally, I found here all causes of my problems.
    But, I must say...WHEN I'M SITTING IN THE BUS, ON HIGH CHAIRS, I GET OUT FROM THE BUS AS ABSOLUTE NORMAL PERSON!?! I WALK AS ANYONE! BUT, IT ENDURES SHORTLY. TILL I PUT ANYTHING IN ANY OF MY HANDS (BAG) OR FEEL TIRED.

      Yes. However, you could be a PEC, which just means both sides of your pelvis could be forward, instead of just the left side.

    4:20 in this example you gave, if that person is tested with right hip IR in seated position, he would have less IR because hip is already internally rotated, correct?

    Great Video Neal, So many things you describe I have. I cannot properly adduct my right hip and i tested for the left AIC pattern and tested positive for it ( left leg/knee does not adduct, and right knee, does not abduct) but for example, my right Foot is the one that is stuck in pronation. Right foot pronates, and right knee is caved inwards?? what does this mean.... Thank YOU

      @@johanesterhuizen1842 you can see as people walk that their center of mass never moves all the way to the left. Another give away is lack of right arm swing during walking. Also if you ask someone to put their weight on their left foot (but still have their right foot on the ground) their upper body is still to the right. Plus there are PRI tests that indicate the inability to shift to the left.

    When you're on your left foot the pelvis is supposed to rotate to the left and so is the upper body? Upper body supposed to rotate to the right you're staying?

    Keep the tangents so we know what to listen for if we work with other practitioners and understand the difference from what you are explaining . In cycling there is no ground or pronation......and look at pelvic position in a time trial. Hmmmm. Orthotics in cycling shoes...

      The ground isnt even the ground so to speak. You just need something for the feeet to sense so the parasympathetic kicks on and your nervous system calms down. The pedals would be the "ground" when cycling

    I understand a concept and it’s pretty clear. I just don’t know why you repeated the same things 3-4x times and not getting to solution through the video.. and not even at next one..:)
    and then everyone have to wear running shoes all the time in their life to be aligned?! With medial support? To have the sense of pronation on the right side?! You said that while giving the exercise is easy to fix this pattern.. but they usually get back to the old pattern while standing. So what is the solution for that?!
    In your next video you also demonstrated a side lying exercise, and it’s obvious you have to be aligned at the torso to have the benefit.. but if the person does like that, the old pattern won’t come back while standing?! Cause it isn’t revealed.. and the title of your video were focusing on solving this problem… but then it didn’t get solved.
