গর্ভাবস্থায় শেষের দিকে অনেক মায়েদের Intrahepatic cholestasis of pregnancy (ICP)-র কারণে সারা শরীরে প্রচণ্ড চুলকানি হতে পারে। ICP-তে, পিত্ত রস (bile) লিভার থেকে সঠিকভাবে নিষ্কাশিত হয় না। এর ফলে পিত্ত রস রক্তে জমা হতে থাকে এবং ত্বকে চুলকানি সৃষ্টি করে।
ICP-র কারণ:
গর্ভাবস্থায় হরমোনের পরিবর্তন ICP-র ঝুঁকি বাড়াতে পারে।
জিনগত কারণও ICP-র ঝুঁকি বাড়াতে পারে।
গর্ভাবস্থায় ডায়াবেটিস, উচ্চ রক্তচাপ, বা স্থূলতা থাকলে ICP-র ঝুঁকি বেশি থাকে।
ICP-র লক্ষণ:
ত্বকের তীব্র চুলকানি, বিশেষ করে হাতের তালুতে এবং পায়ের পাতায়।
চুলকানি সাধারণত রাতের বেলায় আরও খারাপ হয়।
ত্বকের রঙ হলুদ হয়ে যেতে পারে (jaundice)।
বমি বমি ভাব এবং বমি।
পেটে ব্যথা।
ডানদিকের কাঁধের ব্যথা।
ক্লান্তি।
ICP-র চিকিৎসা:
ICP-র চিকিৎসার লক্ষ্য হল পিত্ত রসের মাত্রা কমিয়ে চুলকানি থেকে মুক্তি দেওয়া এবং গর্ভবতী মা ও শিশুর স্বাস্থ্য রক্ষা করা।
চিকিৎসার জন্য ডাক্তার ঔষধ দিতে পারেন।
কিছু ক্ষেত্রে, গর্ভাবস্থা 37 সপ্তাহের আগে প্রসব করানোর প্রয়োজন হতে পারে।
ICP-র জটিলতা:
গর্ভাবস্থায় উচ্চ রক্তচাপ।
অকাল প্রসব।
গর্ভপাত।
ভ্রূণের মৃত্যু।
ICP-র প্রতিরোধ:
ICP-র কোন নির্দিষ্ট প্রতিরোধ নেই।
গর্ভাবস্থায় নিয়মিত ডাক্তারের সাথে দেখা করা এবং ICP-র লক্ষণ সম্পর্কে সচেতন থাকা গুরুত্বপূর্ণ।
Severe Itching During Late Pregnancy: Intrahepatic Cholestasis of Pregnancy (ICP) Explained
Many pregnant women experience intense itching throughout their bodies, especially towards the end of pregnancy. While there are common reasons for this, a condition called Intrahepatic Cholestasis of Pregnancy (ICP) can also be a culprit.
What is ICP?
ICP is a liver disorder specific to pregnancy. It disrupts the normal flow of bile, a fluid produced by the liver that aids digestion. In ICP, bile accumulates in the bloodstream instead of being released into the intestines.
Why Does ICP Cause Itching?
This buildup of bile salts in the blood reaches the skin, leading to severe itching. This itch is often most intense on the palms of the hands and soles of the feet, but can affect the entire body.
Causes of ICP:
Hormonal changes: Pregnancy hormones can increase the risk of ICP.
Genetics: A family history of ICP increases the chance of developing it.
Underlying health conditions: Pre-existing diabetes, high blood pressure, or obesity during pregnancy can elevate the risk.
Symptoms of ICP:
Intense itching, particularly on the palms and soles.
Worsening itch at night.
Yellowing of the skin and eyes (jaundice).
Nausea and vomiting.
Abdominal pain.
Pain in the right shoulder blade.
Fatigue.
ICP Treatment:
The primary goal is to manage bile levels and alleviate itching, ensuring the well-being of both mother and baby.
Medication prescribed by a doctor can help control bile production.
In severe cases, early delivery (before 37 weeks) might be necessary.
Complications of ICP:
High blood pressure during pregnancy.
Premature birth.
Miscarriage.
Stillbirth.
Preventing ICP:
There's no specific way to prevent ICP.
Regular prenatal checkups and awareness of ICP symptoms are crucial during pregnancy.
