Why tennis players get shoulder pain and how they can fix it

  • Tennis is a game of constant overhead motion. So if you play enough, your shoulder is vulnerable to pain and injury. Fortunately, you can stop both in its tracks if you know why it's happening, and what exercises to do to fix it.

    After some aggressive grip changes and service motion adjustments, had my first real shoulder crisis ever; took a week off tennis, first time in ten years, because pain was intense enough to interfere with my sleep and I just could not have it. Unbelievable, but after performing this simple rehab routine for five days or so, I am back in business. I am grateful you took the time to make and upload this video, dr. Boynton, keep up good work .

      @@LiamApilado Forgot about it completely, months ago; after the pain was gone, I returned to experimenting with my service motion I think I got it right, don't need to stretch the shoulder in any non-routine way at all

    Wow. This is magic. I followed along with the video, pausing it at times to do the exercises. AND now I feel almost complete relief !! (I had a sore shoulder for 2 days after practicing serving a lot)

    Thanks for the information in this video. I felt like I've had a trapped nerve in my shoulder for over a month. So I did lots of movement and swinging my arms. It relieved it temporarily but the following day the all the knotting that felt behind the rotator cuff before went to the top and was much worse. So tonight I've tried wintergreen all, finding a trigger point and pressing and IR therapy. All gave slight relief but your exercise has relieved it the most. So this info is very useful.

    Thank you so much doc. I've been playing badminton for a long time and suddenly started suffering from shoulder pain for many months now. I've tried various strengthening exercises recommended by physio on and off but none of them seemed to have worked. I did the 2 exercises you mentioned here and instantly felt relief. This seems like a miracle! I'm not gonna rush it now. I'll do these exercises consistently until there's no agitation left whatsoever. Everything you explained makes sense. And indeed I started to notice some pain in backside of my shoulder and the pain indeed presents sharply in the front when I swing motioning a smash and almost feels like the pain starts somewhere inside the front of the shoulder and is radiating towards the bicep which stumped me.. You're the best!

    Dr. Erin you honestly saved me!
    i had this pain exactly in the position of the shoulder you're mentioning and i've been having silly physio where they are using massage guns and cuping on the front part of the shoulder where i feel i weird feeling when i serve like my shoulder isnt very stable and they have neglected the back part as you mention. I will start using these exercises thank you very much

    thank you for this video, it helped me so much! I knew this stretch but it didn't work for me(feeling pain, not stretch) until I changed the position of the elbow as you suggested.

    This stretch is exactly what I was looking for. Practicing 250 serves a day - I was in dire need of a fix. This stretch is now leaving me pain free and able to practice my serves. Obviously the shoulder strengthening exercises are needed as well - but this doctor hits the nail on the head. You need to do this stretching in addition to strength training. Otherwise - like I found, you will suffer pain from performing lots of serves.

    I finally got a kick serve but my shoulder does not like it. I tried latex bands and hanging from bars and all sorts of things. But I always had pain and tightness in the back of my shoulder. This stretch followed by pushing down as you describe does exactly what you say. It rebalances my shoulder. Thank you so much.

    Hi! Thanks for this video! When is it safe to go back to playing tennis after a shoulder/arm injury? I’ve waited almost 3 weeks now, and generally haven’t felt any pain in over a week now. I can feel my arm while doing certain shoulder exercises but I wouldn’t describe it as pain, and I can tell that the arm is still just a tiny bit weaker than the other one. Am I safe to go on court and play some gentle strokes?

    Wow, this is great! Thanks! - My own physio always says to me to “stretch and strengthen”. Do you have any rotator cuff strength exercises to add to the stretching?

      Train the Infraspinatus muscle in that abducted position, both concentrically and eccentrically with resistance bands. Important for the deceleration phase of the arm on serves and topspin forehands.

    What a lovely dog, what breed is it? Counterintuitive to think you need to stretch out the back of the shoulder if the humeral head is pinching the rotator cuff tendor at the front, most vids suggest strengthening the back and stretching front to balance it out. Given your credentials, i'll give this a shot for a while and see if it helps. After a long game i get a bit of anterior shoulder ache.

      same. did you find a solution? if so let me know because I have had a pain for about a month now in the same place and it started to hurt while I was serving but now it’s so bad that I can’t even hit a forehand

      Same here. In my case it is due to the infraspinatus muscle. However this exercise is still useful.

      Strength training usually 3 times a week. I would say this is more of a preparatory exercise. Never before playing as it would fatigue the muscles. Afterwards is okay though.

    Hi Doc, I just returned from a pro tennis tournament with a shoulder injury (impingement inflammation). Your video makes total sense, you’re a god send!! One question,It’s not clear to me whether this exercise is advisable as a preventative shoulder injury or as therapy for when you have an injury (I don’t want to risk aggravating the problem). Can you clarify this for me and potentially anyone else that found you searching RUclips for info on shoulder injuries. Great video.

      It is both preventive but should also be possible when nursing an injury. Monitor your pain and back off in case.

    Hi dr. I am a rising junior in high school and I am 150 in Texas boys 16 in tennis. I fee pain whenever I switched strings to a more tighter and harder string. I just wanted to know if that had anything to do with the pain in my front shoulder all the way down bicep muscle. Just curious to what caused it. Also if I’m in a match what should I do to help it

    So doing face pulls doesnt help at all? I havent been playing for a few months and I still have pain. Working out at gym but avoiding heavy press movements.

    Great tip. That's exactly where the pain is in my shoulder. Do you know what cause the pain on tennis court? Is it because of wrong motion or to hevy racket?

      On rail technique is usually the cause. If you are technical sound, then try using a more headlight racquet

    Dr. Erin, nice video! I have a question about shoulder lack of proper mobility due to kyphosciliosis. When I lay down, the shoulder blades down and arms in L shape, triceps/biceps lay down -- but -- the forearms and hands cannot reach the floor. (By the way, I see your dog is sensing you are talking about heeling -- and wants to help -- how adorable)

      Work on inproving kyphoscoliosis and posture. Yoga is a great tool for that.
