A lady had Asian eyelid surgery one month prior with dissolvable stitches used to form her eyelid crease. The dissolvable stitches show a prominent bump when she closes her eyes, so she asks if it is expected after surgery. She doesn’t live in the same state as her surgeon, so she is hoping it will resolve by itself.
New York Oculofacial plastic surgeon Amiya Prasad, MD reviews her question and photos in this video, and explains the Asian eyelid surgery process and use of different stitches in this type of procedure:
1:20 - Dr. Prasad’s background and training, and his expertise in Asian eyelid surgery
1:47 - How this is an incisional technique of Asian eyelid surgery, and asks if there were non-dissolvable sutures that were previously removed, or if the whole operation was done with dissolvable sutures since she lives out of state
2:32 - How it is important to see if the surgeon needed to engage the inner part of the eyelid with muscle to create the crease, which needs deeper stitches
2:51 - How the goal of incisional or non-incisional Asian eyelid surgery is to create a crease using a long-term connection between the eyelid and the muscle that lifts the eyelid (levator muscle)
3:13 - How dissolvable stitches are used to engage the eyelid skin to the backbone of the eyelid and the levator muscle
3:26 - The different types of dissolvable stitches, which dissolve at different rates can cause bumpiness
3:44 - How the first month of surgical healing called the proliferative phase generates a lot of collagen, blood supply, and activity
4:00 - How the incision alone will flatten and smooth out with time
4:10 - How the thickening on this lady’s eyelid is hard to determine with a picture alone if it is due to stitches, or the normal wound healing process
4:28 - How each doctor uses their own routine practices, and a doctor who uses dissolvable stitches will know the healing time and process associated with them, and how individual patients respond to them
4:50 - Why it is possible that the thickening is due to dissolvable stitches, only the doctor who performed the surgery knows the details of anatomy and the surgical process behind them
5:08 - How her doctor, and Dr. Prasad have specific processes for patients who come from out of town for different types of surgeries and treatments, so patients know what to expect during the healing process
5:40 - How speaking to her original doctor and showing them her pictures will give a clearer understanding than any other doctor who didn’t perform the surgery
