I have always had a terrible periodontal disease, since I was a kid. I’m almost 50 now, and about 20 years ago a periodontist wanted to remove all my teeth, because it was getting worse with the pregnancies (he said that I had bone loss of an 80 year old woman at 30), but I wouldn’t let him. I still have about 14 of my own teeth, which I’m happy about. My upper teeth were the worst, and on the bottom, only the middle ones. Anyway, I’ve had bone grafts and sinus lifts on both sides on top. On my right side I’ve had implants put in, then crowns, but unfortunately it only lasted about 10 years, maybe even less, and they all fell out again. The bone was completely gone again. I’m not doing it again and going through all that. On the left side, the bone that was grafted is still there, because I never finished putting in the implants and crowns there. I’ve had a partial denture made for the left and right sides then, but it’s so extremely uncomfortable, that I never wear it. I just try not to smile too much or open my mouth wide. So far, the implants and crowns on the top front are still fine. But, it seems that if you have a bad periodontal disease, it’s really useless to go through all the implant work, it just doesn’t last. 😢
I am so sorry. I am 51 and have gum disease. My teeth are being pulled. Then I will be fitted for dentures. I am getting worried, they might not fit. There is no way I want implants. I just want my dentures to fit. I have been looking forward to finally being able to smile.
You explained gum recession, bone loss, tooth loss, bone grafting and implants in a simple, logical way (not too high-level, not too detailed) that most RUclips videos have failed to do. Well done.
Thank you Dr.! I’m currently seeking an option that will fit my situation. I know I have significant bone loss and I’ve got the ; you can’t have snap in dentures/implants. I know I won’t be able to keep any of my natural teeth and I’m a lot more comfortable with my options. I’m going to move forward and will let ya know how it goes. Thank you for using simple terminology and your outward, positive attitude.

80% Everywhere!!! That would be me. I'm doing an all on four on the bottom and 5 on the top. The maxillary failed the first time, went back in bone graft sand sinus lift augmentation and supposedly it all looks good so, we're going back in a a few months. I've had periodontal disease since I was 12. At 56 they all came out. I wish I were in London, I'd totally trust you with my mouth!
This is the first time I've heard someone say they've had periodontitis for more than 40 years so you've given me hope that I'll make it to at least 60 with diligent upkeep, thank you
Love the way he explains in more simple terms that we need to understand. Dentist don't do this for us, and we know nothing about our bone loss. The question I have most of all for the dr. here, is one bone is rebuilt, implants placed, does the grafting hold? When you see 'failures' of implants, what does this mean? What is considered failure? When the implant (metal) is exposed? Does it fall out? The is the most important question of all.
That happened to me. They told me I had periodontal disease but never explained to me the things that could happen and now supposedly I don't qualify for a bone graft.
Dr. You explained this procedure beautifully. I chose to have an implant due to tooth #13 fractured straight up. I spoke with my orthodontic surgeon. We discussed the options, I had done research so I had a bone matrix placed, came back in six months got impressions. Got my implant in a while later, I have not had any trouble. People to ask questions if they are unsure. Sorry about the ramble.
I’d been looking for a dentist in Columbia, Tennesse and found Dr. Brian Fann and staff. Amazing experience and they went the extra mile to get me in for an appointment. Treated me like family! Highly recommended!
So happy to have found your channel. I've not had teeth (any) for 10 years and I have significant bone loss all over and around my jaw and inner working of the mouth. I was told I didn't have enough bone ridge to place a working denture(s) so I have teeth I wear when I go out and cannot eat with them at all. Is there any hope for an all-in-four? My dentures don't look anything like me and I have great depression because of it. Thank you for sharing your expertise with us! God bless 🙏🏻-Tina in Kentucky
Mee too,I've lost all my confidence,I won't go for jobs,I haven't had a relationship for 8 years 💔🥺 I've accepted my fate,I'll never have the $$ to do this,,my bone is ruined ( All nearly) I accept I'll be single for the rest of my life n hate selfies, mirrors etc....good luck yo those that have a chance
My husband had working denture made. Almost year ago and he had been without teeth for 28 years. He has noticeable bone loss. Gum shrinkage and no ridge. I have gum disease and bone loss due too infection. I had my final wax try in yesterday. You could still try and get dentures that you like better,
I get it mine were pulled seven months ago. No teeth has not been fun. But I still go to family gatherings. It just means that I have to put my embarrassment aside. So that people that love me. Can see me. This is temporary eventually you. Will have teeth again. It just is going to take some time. Don’t give up. Focus on the positives.
I m wondering why are they using dental implants with screws rather than anchors. Anchors have more stability and grip than screws especially when jaw bone is a soft bone. Moreover anchors are a lot easier to install, but the only drawback is when you force one in it will never come out.
@rinzler9775 Unless thoses experts you talked to have invented new technique they can't speak, uptill now the only sure thing is the implants with screws.
Thank you so much for this really important information, it will really help me with my implant decision in the coming months ahead. thank you Doctor Gurs Sehmi.
Would love to see your evaluation on three on six dental implants vs all on four. They are claims they can install them even with severe bone loss is that true? and if so is this an all around better solution to use the patients natural gum line, have smaller bridges in the event of breakage, etc...
Hi Dr Gurs, Thanks for taking the time to educate and help all of us. Is getting a CBCT scan just as beneficial for implant planning as a CT scan? Are they the same? If not, how are they different? Which do you recommend? Thanks.
Worse come to worst, what is the most costly expense for people to expect when they loose most/all of the dental bone (both upper/lower)? Thanks for your help.
Excellent video Dr, I recently got one implant with 20% bone loss, I feel gum line is hurting, my dr said its not infection. I worried abt the implant now.
I've had a complete upper denture for a number of years now, with a partial in the low jaw. Now aged 50, it is time to remove the last of my bottom front teeth. The last 5 in my case that is now a bad shape. Like many, I keep looking into the all on four systems. Both in the UK and overseas. I worry that when I have the last of my bottom teeth removed I will be unable to eat having both full upper and lower dentures. The amount of Poligrip I use now is unreal.
Get dental implants for the remaining teeth... If at all possible.. That will be in your best favor. Then high quality dentures with the dental implants..
I found your video very informative. I like the idea that one could possibly have the all-on-4 implants, would the same hold true of the implants for snap-on dentures? Aesthetically, I'm not sure about the gap between the dental prosthetic in all-on-4, and do like the look of the gum coverage of traditional dentures, but the snap-on process seems like it would make them more stable. I do have bone loss, and there are spots where teeth have been gone for years; I have no molars on the right side. How long would it take to get the periodontal disease under control after extraction? Would it require a lot of office visits, or would it require antibiotics with more intensive cleanings at home? I do brush and floss, and my gums don't bleed from those, I don't scrape my tongue, but do use mouthwash (especially pain relieving); I imagine I'd need to continue brushing or maybe use a wash cloth to clean them. Could immediate dentures work in the meantime? I do need to work, and give my position of employ, I really do need teeth. Besides, people used to comment on how infectious my smile was, and how I smile with my whole face; I just don't smile as well anymore (only a simple upturn of the corners of my mouth, with it closed). Right now I'm trying to figure out how to come up with the money for the total extraction, and the set of temporary dentures. In the US this process is so so very expensive. There are places, like Affordable Dentures and Implants, but the reviews are mixed, so I'd really rather go to dentist or oral surgeon I've used in the past; again, that's going to be super expensive. So, I'm still in the research phase. I appreciate your videos.
i wish implants had multiple roots like real teeth. i have all my teeth but im preparing for the future. im so scared to be toothless like a homeless 😭
Great, just what I was looking for! It is neat and clear explanations of how it works, straight to the point, I really appreciate the information, thanks. I have a question. I have just lost my two top molars, one on each side, both were wobbly. One fell and the other was removed. Both had the root canal done many years ago. When I asked about implants I was told there is "not enough bone" but I don't have gum disease (no inflammation or redness but I have some gum recession). I am 68. I'm at lost about what to do, do you have an advice?
What happened? I lost a top molar during pandemic, first infection, then it broke, need implant with lots of bone loss, but they did not say they can't do it. Now other side upper molar cracked into root, got bad infection, that essentially went untreated by professional who did not advise scan to deal with it. Then paid big money for laser gum surgery only to find out the molar cannot be saved and infection caused major bone loss. Talk of grafting etc. So scared. Here he says with 80% bone loss, still things they can do.
Hi Dr Gurs, In this video, you describe the procedure for replacing all the teeth with implants and you mentioned that it’s a totally different process if you’re just replacing a few teeth. Do you have a video that describes the implant procedure for just a few teeth? I didn’t see a link in your description, but I’ll look through all your other videos myself too. I need to just remove and replace one tooth with an implant. Thanks.
They told me that I was born with my bone loss issue and nobody wants to deal with me. I can’t get implants. They won’t give me dentures. They said just go until all my teeth fall out. I mean, I cannot believe that I went to a different people and got the same basic response my teeth are not rotten. I don’t have gum disease I just, don’t have enough bone in my jaw left for implants & dentures are out due to loss of bone. I’ve seen people with gum, disease and rotten teeth and they got their teeth restored. I don’t understand any of this. I have been searching out someone for 15 years and nobody helped me this is impacted my life and this has really done damage to myself through the years, not being able to have a decent smile , this was nothing that I have done to myself. I don’t understand why I can’t be helped.
Great info thank you. What if a person has Sjögrens resulting in the perio problems. Is it worth getting an implant in front upper tooth with bone loss?
I believe he is referring also to the mere fact that many people now, than before.. Because of degeneration :of mankind to get more sick as time goes on.. The oral health regarding the needful bacterias in the mouth...
I haven't had lower molars in years. I have been turned diwn by many but today I saw a Dr that says he can shave bone to make it wider. From about 5mm up to 7mm in some areas. Would this be invasive? He says it's easier because then we don't have to do a bone graft.
Thanks for the informative video. Do you have any experience with Paget's disease of the Bone in the mandible? Specifically, how would bone grow around implants with that condition? Would the bone regrowth be too fragile to support that?
Dear doc unfortunately I made a big error by having dental implants in Turkey, now I am so unhappy my teeth are ill fitting stagging neck caps under denture , salavate when speaking, I have no confidence and don't like to look at myself . Full mouth input . Please what can you suggest. I was more comfortable with the denture. But now I have no teeth to hold them in place. I only saw you on RUclips after having implants. Thanks Angela
I had 4 implants put in for lower 4 on 4. Each implant failed. They were standard size & the reason they failed was because the bone dissolved. This was the result of me being anemic. I had gastric bypass surgery a few years prior to the implants. The gastric bypass was not for the purpose of weight loss. Instead, it was a treatment for diabetes 2. I'm curios to know if I might have better results with mini implants.
@@colourmc2437 in my particular situation this was the case. nonetheless, if you have any considerations for implants & have a history of anemia my advice would be to make certain you discuss this with dentists before hand. my anemia is due to gastric bypass surgery.'
@@colourmc2437No, anemia does not cause bone loss, in fact, red blood cells are produced by our bone (marrow), not the other way around. There is something going on with her immune system and she needs to be seen medically.
What about the lower jaw? I have bone loss on the lower part, now I got only 5 lower front teeth left on the lower jaw. Hope you can do some videos about it. Thank you very much.
Sir, I mailed you OPG film of my wife, but you rejected my request of suggesting me. If so then why do you share RUclips vids, if you are not interested in suggesting someone.
He never answers anyone either, he just wants to get likes and his face out there. He is always advising against surgery abroad etc but only because he isnt capable of doing it. Try Sofia Implant in Bulgaria. I oaid €7400 for 10 implants top and bottom. Includes everything from scans to temporary teeth.
Im getting Zirconia teeth But im only getting 12 teeth top and bottom.. why is that. My friend got 14.. Iv got my top and bottom implants in just got to wait 5 months to heal ... Can not wait and till I get my Hollywood smile ...
Yes ,Very informative. I have had the all on 4, in a Day ? .but bottom was ok. Could Not fit upper as had to do bone Grafting, 3 months after initial surgery, Still No top teeth. And have lost 23 lbs in weight,Due to soft food diet. So Not Very Happy.
Hi Dr Gurs, how do you calculate the percentage of bone loss? I understand from your video that when we are 18 the bone line is 2-3mm below neck of the tooth and with future bone loss you could measure the distance of the bone line below the neck of the tooth and then subtract the 2 to 3 mm so that computes the amount of bone loss but what do you divide that by to get the bone loss percentage. Thanks.
Do you know anything about medication, such as Buprenorphine, exacerbating serious gum disease and bone loss? Also disease called Achalasia, where lower esophageal muscle does not relax, food gets stuck, chronic acid coming up to mouth at night during sleep
I take suboxone and yes it does cause serious bone loss been taking it for 5 years 25-30 and i have the bone of a 60-80 year old sucks bc its so hard to kick
I just have, I had pterygoid implants and didn’t need a bone graft but it’s very doubtful any uk dentist is knowledgeable to do this procedure. Its very specialist so a dentist like this wdnt be able to do it
My bones receded due to teeth extraction from my childhood, all 8 front teeth are gone , now im 52 yrs old, im wearing a bridge and its not comfortable not to mention the odor no matter how much yo brush. So is there a way to have an implant with this condition?
Doctor, i have right UCLP, had alveolar bone graft > 15 years ago. I had right incisor missing and having a bridge. Now they say, the canine holding the bridge is almost near coming out. There is gum ressession. To make things worse, i have osteoporosis and on alendronate for nearly 5 years. My oral health is good. Now they suggest i should remove the right canine together with the bridge and should go for a 2 teeth denture. Have i got any other better solution please?
I was only 47 years old but the orthodontist saw my x-rays and said I have bine loss. OMG! Not sure if this condition related to clear aligners treatment or not . I have it done 2 years after x_rays done.
I pulled my teeth five years ago cause I grind and broken so much bridges etc . I got lots of bone loss and no infection with denture can they rebuild my jaw and will my face look less saggy after ? I am 38
Good question. I have similar problem for lost upper and lower molars/bone loss. Think will help somewhat for sure as I was told when teeth are replaced will fill out cheeks more.

Three years back I went to dentist for my ortho treatment. That dentist removed my front theeth from lower teeth area and after that my ortho treatment get runied and now i got to know that my theeth bone has been loss between two teeth. Now, another dentist suggested me for bome grafting and teeth implant. I lost 80℅ bone between two teeth. Is is possible to treat this by bone grafting and implant? I am 27 years old. Please reply

Hi doctor, I have extra ted my last two teeth of my lower jawline and one of them, it's been at least 5 years and 3 years so I don't know how much bone I have lost but now it's like my upper wisdom teeth is hurting my lower cheek area , doctor suggested to extracty wisdom tooth and implantation so that in a few years I don't have to remove my rest of my upper teeth , so I am having trouble in getting implantation because what if something wrong , So my question is that is it right thing to do the implantations or do we have anything that can stop my upper teeth to come down so that I won't go through implementations
Hi. I've been watching your videos and it has really helped me a lot. I just have this second thought of having my lateral incisor implanted but it has been two year since it was extracted. Is it possible that I've lost 80% of the bone? Can I have it implanted just by sticking it in the gum? Will it be much cheaper if so?
After the implant and zirconia porcelain had been placed, can we eat hard food like peanuts and meat, will it cause the implant and zirconia porcelain crown to crack when we eat hard food?

I have no upper teeth and i want implants now, but im told i lost 100% of my upper front bone, but they could put the screws in the back, i dont understand this technique? Can you help me?
I'm not sure about your exact situation, but if you have no bone at the front, usually the next options are too either consider bone grafting and sinus lifts, or implants that use other bone areas, like zygomatic implants, pterigoid implants or trans nasal implants. These are all very advanced techniques
My theory, though not perfect, is to brush thoroughly with a HARD bristle brush using a combination of Arm+Hammer+Colgate (stannous fluoride) toothpaste, brushing 2-4x per day, carefully, like floss, and chewing Xylitol chewing gum after eating. Avoiding snacks, and sugars/carbs. I'd rather brush my teeth out of my mouth than deal with dentists.
Hello doctor. One question. If I would have to remove all my teeth because of advanced gum deasese and go for bone grafting, would I have to be without teeth during the months which would take for my bone to heal or is it possible to have some sort of temporary teeth for the time being. Thanks in advance for your reply. Cheers.
I have 2/3rd bone loss around full teeth .had flap surgery with bone graft . Still most my tooth are slightly wobbly . What's the reason for it ? Is it gonna better ? And I have gum recession too which is bad on 4 upper front tooth ? Any treatment plan can you advise ? Am I able to have aligners any chance ?? Can you please reply 🙏 TIA
I need front tooth extracted and l looking to get an implant. Because its suffered from periodontal disease I've lost alot of bone and gum recession. There's no pain it's just loose and is sticking out looks ugly. My dentist has told me I cannot have a bone graft at time of extraction because of infection and he wants that to clear up first. What is your opinion I'm worried I'll just lose more bone and gum if bone graft is delayed?.
Any update how are you now im in a similar position they are insisting on the cleans but im having them every 3 months my teeth are getting worse now i need the front 1 put asap its moving but not offered bone graft just maryland bridge but will have to wear denture in the meantime and all i think is if i have 70% boneloss now when its pulled and temp denture is in its only going to be worse
Isn't it shocking that people's teeth, bone and gums get to this stage when early prevention advice might have stopped it or stopped it being so severe?
I took care of my teeth as best as I could. Went to the dentist regularly had oral surgery to control periodontal disease. I was 28 when I found out I had periodontal disease. I am 56 all but 5 teeth have either been pulled or fallen out. My parents had periodontal disease so did my uncles. Seems to be hereditary for me. I am starting the process of getting implants. What I am saying is sometimes you can do everything right and still loose your teeth.
Could I ask for your advise? I have 2 front upper dental implants with a bone graft as I had very low bone density and this was then made up with a 4 tooth bridge. Two days after the initial implant, I started to get a lot of pain above the eye brow ,location of my sinus. I have had this now just over 8 months, my doctor seems to think it is related to my implants. As I had told him when I was getting one of the two upper implants as the dentist was tightening the screw in, on one of his last turns ,I nearly left my chair as it felt he had hit a nerve. He told me not to worry , and for a few months after I told him of the pain in the eyebrow area and he said it would go and had no relation to what he had done. What do you suggest I do?
I had laser treatment done for gum disease. Now that it's all done my dentist says I need molars on top and bottom pulled. That's 4 teeth on top and 4 on bottom. I'm not sure I want to be pulling all these teeth out and I'm worried if I do what are my options. My teeth seem pretty strong and none are loose. Please help.
@marydempsey5985 I did get a second opinion, which cost me over a thousand dollars just taking xrays and pictures of all my teeth. Dentists are nothing but big rip-offs, just lying to you and taking your money. I'm so done. You dont know who to believe or trust. I'm not letting them pull any of my teeth. Just going for second opinions cost you a fortune.

This is the first time I've heard someone say they've had periodontitis for more than 40 years so you've given me hope that I'll make it to at least 60 with diligent upkeep, thank you
@@vanessaphanouvong5870 How old are you?
What is done matrix? Never heard of orthodontic surgeon, do you mean maxillofacial surgeon?
I’d been looking for a dentist in Columbia, Tennesse and found Dr. Brian Fann and staff. Amazing experience and they went the extra mile to get me in for an appointment. Treated me like family! Highly recommended!
Of course they treat you like family when u have the ability to pay 💰
I had my teeth pulled at low income dental clinic. They treated me like family with no ability to pay.

Anchors would be a nasty problem if you need to remove one. Sometimes implants fail, or get infected - having the "unscrew" option is critical.
The anchors would hurt on a sensitive tooth it seems as well..
Unless you're a great dentist with much experience in the field your theory is just internet know it all .
@islambary2menkom or, you have had this treatment before, and spoken to several surgical experts directly on the available options, like myself.
@rinzler9775 Unless thoses experts you talked to have invented new technique they can't speak, uptill now the only sure thing is the implants with screws.
This channel is really helpful. Thank you for the information
What a freaking nightmare I wouldn't want my worst enemy to go through that hell😢😢😢😢
Finally someone who explain it perfectly , thank you sir
Thank you so much for this really important information, it will really help me with my implant decision in the coming months ahead. thank you Doctor Gurs Sehmi.
Would love to see your evaluation on three on six dental implants vs all on four. They are claims they can install them even with severe bone loss is that true? and if so is this an all around better solution to use the patients natural gum line, have smaller bridges in the event of breakage, etc...
Hi Dr Gurs,
Thanks for taking the time to educate and help all of us.
Which teeth you did implant. And what is your age?
Why don't they design implants with the same roots like real teeth??j Don't blame you, do what you must.
Hi Dr Gurs,
In this video, you describe the procedure for replacing all the teeth with implants and you mentioned that it’s a totally different process if you’re just replacing a few teeth. Do you have a video that describes the implant procedure for just a few teeth? I didn’t see a link in your description, but I’ll look through all your other videos myself too. I need to just remove and replace one tooth with an implant. Thanks.
My mouth sags because I had back teeth removed . I don’t know if implants would have helped and they are very expensive .
If you have an extraction can you add the graft into that same hole or do you have to cut the surrounding gum?
Do you do Zirconium implants?
I am not sure that severe periodontitis can be eradicated. Can you elaborate on that?

Can i still do implants when my tooth was extracted 2 years ago?
Great info thank you. What if a person has Sjögrens resulting in the perio problems. Is it worth getting an implant in front upper tooth with bone loss?
Hello, what is your take about LANAP? is it really helping bone growth?
You gave me hope doctor.
Thanks for the Vedeo.
If a front upper tooth broke off at the gum line, no bone loss, does that mean an implant is the only solution?
Good info .. a little confusing, “gum disease, no implants period”
But then “most patients have gum disease”

I haven't had lower molars in years. I have been turned diwn by many but today I saw a Dr that says he can shave bone to make it wider. From about 5mm up to 7mm in some areas. Would this be invasive? He says it's easier because then we don't have to do a bone graft.

Thanks for the informative video. Do you have any experience with Paget's disease of the Bone in the mandible? Specifically, how would bone grow around implants with that condition? Would the bone regrowth be too fragile to support that?

What’s the difference between bone grafting and LANAP surgery and which is better?
Wish i could get implants but its not feasible with my sjogrens and ehlers danlos 😢
Oh! Is being anemic causes to dissolved the bone?
@@colourmc2437 in my particular situation this was the case. nonetheless, if you have any considerations for implants & have a history of anemia my advice would be to make certain you discuss this with dentists before hand. my anemia is due to gastric bypass surgery.'
But being anemic causes to recede bone teeth?
@@colourmc2437No, it does not.
@@colourmc2437No, anemia does not cause bone loss, in fact, red blood cells are produced by our bone (marrow), not the other way around. There is something going on with her immune system and she needs to be seen medically.

Thanks.
Excellent video

Get the gum disease treated first. Go to periodontist and after a few session infections should stop. Damage done already will not however.

Yes you can.
