My theory is that writers were going for the reveal that House himself is a medical phenomenon(as is most of his cases), basically saying that he shares a DNA of a few man(cause how else would you have same two birthmarks as a man who isn't even your father, and yes it was then revealed it was two birthmarks that they share)

Same with the cashier. They’re just regular people trying to have fun at work, not performers. - Wilson starts laughing with Chase, but with the anxiousness and confusion of someone who’s joining in late on a conversation and trying to enjoy a joke without understanding it, and so on.
All these years later.. I still cannot watch clips of this episode without laughing hard.... The irony is that I can see Wilson and Thirteen actually doing something. _"From this moment.. my lips are sealed!"_ Translation: he already told every woman in the hospital before that!
