    I just did a case similiar to this today morning. I finished in one hour and 30 minutes. I chose not to leave the roots behind . I took crown out first and roots were do-able so i removed it. Will check patient post op in 4 to 5 days. Personally i do not believe in leaving the roots behind. Unless it is a root tip and will resorb. Leaving both the roots behind seem unreasonable to me. However i respect the fact that unless we are the operator handling the case we cannot comment. Very good job dr. I loved the video and hope you can upload more cases. It will be a great platform to exchange views on minor oral surgery.
    * i packed the socket with coltene's roeke gelatamp prior suturing.

      I agree with you. On the CBCT, it actually looks quite far from the nerve-----well "far" as in not SUPER clsoe/wrapped around the nerve. I would have taken out the roots as well. But once again, who are we to judge when we were not there.

      I've never had any issues when I opted to leave roots like this rather than continue in difficult cases like this

      I would really like to know ,what are the chances of infection post such intervention...honestly with just a lil distal bone removal the roots could have been removed quite easily.

      Coronectomy is a much safer route. Paresthesia is such a headache. Also no need to make the surgery more traumatic than it needs to be for the patient. He finished in 4 min lol

    How do we know if leaving the roots will cause pain after that will not subside unless the roots are removed ?

      Complications after coronectomy is very race. I never experienced. I usually take F/U Panorama 1 year later.

    Doctor is too humble with his skills. I had a similar impacted tooth and i showed 2 surgeons. 1st one decided to extract the tooth without sectioning and only used the elevator for an hour and failed. 2nd surgeon took 20 mins to extract.

      If it was a similar case you absolutely can't remove it with just elevator and above all mind the age of the patient.
      Got a position C case for 38 in 61 year old with the similar condition of 37. IT WAS A DIFFICULT CASE.

    Would like to know the 6 months post-op to see if the coronectomy procedure left the roots intact without further infection. Also a little hard to tell, but looks like there is a PAP on #13 and recurrent decay underneath the crown on #14 with open margin on distal

    my third molar was horizontally impacted at 90 degrees as well. was a little sad because my dentist had to sacrifice my healthy 2nd molar adjacent to it today in order to extract it successfully as the dentist was having difficulty getting visibility.

    If you want to leave the root is it better if you not hecting so tight ? Just keep a small hole so the root can move to coronal

    i did this case last month in one hour, i never mind to leave rooth in ramus, its become a nerve damage or mandibular abcess or.....
    but i love your videos presentation, the patient must observe

    Wouldn't it be better to remove the tooth with visible root canal treatment and wait for the wisdom tooth to move into its place, then remove it in a more convenient position? I am asking as a person who is not a dentist.

    I have horizontal wisdom tooth half teeth is erupted sideways and broken my 2nd molar from side which resulted in tooth decay so i decided to remove the 2nd molar the only thing im curious is if i remove 2nd molar will wisdom tooth fill that space

      First of all you cannot lie to a dentist, a wisdom tooth can never broke a second molar, what happened there is that you did not care about the impactation of the wisdom tooth , accumulates food between both teeth and decay the second molar, this usually takes several months to happen which means that it was neglect on your part. To your second question, of course the wisdom teeth will not fill that space, please visit a dentist instead of doing consultation on youtube do not be cheap that already cost you a second molar.

    I had a similar case with horizontal impaction. Only half of the distal portion was erupted and remaining half was under bone. it was too hard. It took 3 hours to complete

      that's too long for the pt to undergo. Might as well have done what Dr. Cho did.

      @@shahida1520 ive seen it done, and ive done it when it would cause more harm than good if you tried to retrieve it repeatedly

      You can compress the IA if u try too much force to remove roots. In this video he decided to only do a coronectomy to avoid any IA damage. He left the roots in.

      I get this done too and I already know they’ll leave the roots behind. They are too entangled in the nerves and I might risk losing function in my face or tongue if they would take it out, so I’d rather have them leave the roots honestly 😅

      @@piasadora I get it! I was so nervous too! All four of mine were horizontal and I underwent surgery to get all four of them out. I didn’t realize how much pressure those wisdom teeth put on my other teeth until they got out! I feel so much better! The numbness lasted for a few hours, but make sure you start on painkillers early, then the pain is definitely manageable. After a week, it’s mostly back to normal. Good luck!
