I found this whole explanation very therapeutic. I had pain for 3 days and I'm on my first tray. But now I'm ok. This helps a lot to understand what was happening.

As someone who recently tried getting Invisalign liners and backed out because of issues starting on Day 1 (attachments placed wrong making it painful for me to chew and eat food) I really appreciate this in-depth explanation. If I try Invisalign again, I'm going to an Orthodontist instead of a dentist because I feel like my case is more complicated and needs someone who is more educated to give me the smile I want.

So how do I know if the resorption and bone restructure after tooth movement takes 7 or 14 days? Should everyone change trays after 14 days to be sure that the resorption is finished? I’m 51 and I’ve been changing trays after 7-9 days. My teeth are very sore and feel very loose, but I don’t know how sore and how loose is normal, as I can’t compare. I can’t use the chewie in the first 3 days, as it hurts too much, like I’m pushing my teeth onto the bone.

I change my aligner every 5 days. My doctor said that if you want the treatment to be completed in a shorter time, you can change it every 5 days instead of 7 days, but you should make sure that the aligner fits well.
Thanks for such an informative video! I am on week 4/23 of my trays and it has been good so far. I did a lot of research and spoke with my ortho, and decided on Invisalign. I am using TrayMinder and it really helps keep me accountable and mindful as I plan my meals and days.

I have been wearing my aligners for 6 weeks now, and I believe they are working. I usually feel tightness and soreness the first three days. Then it is back to normal.

Thank you for this video. I am someone who has boneloss and was advised by oral surgeon to not do orthodontis due to having very little bone. The orthodontis I consulted disagrees and wants to get my bite corrected. I'm 47 and honestly I prefer to not take the risk. I do need one tooth molar intruded and would do orthodontis for that particular tooth only. IStill thinking on how to proceed.
I am 56 years old. Went through periodontal surgery and treatment on 2019. Getting my teeth clean every 4 months at my periodontist. I am getting invisalign at my age, not for the look,but more for the healthier teeth and gum.

SO HELPFUL!! Thank you! 😃 Downloaded the app last night to try. I’m an older patient (39), some teeth are wobbly, one more than the rest. X-ray shows tooth and gums/bone are in good condition but one tooth in particular has more ‘space’ around it (i forget what that area is called), I guess from the movement and changing trays too quickly for some teeth (7-8 days). I’ve been wearing tray 13 for 6 weeks because I heard a loud crack early in tray 12, but that was just before new years. Waited it out as I didn’t have any real pain. Didn’t know if it was the tooth, the tray, or what. Switched to tray 13 after 14days, had more pain, particularly that tooth. It felt weird, extra sensitive to touch, when wiggle or press on it could hear a really quiet click. Had an appt but then my tonsil was sore and with covid being a thing these days I felt I needed to be responsible and reschedule. Pain was not anything unreasonable but naturally if I did have a cracked tooth/root I didn’t want to switch trays yet w/o knowing what was going on. So, another 2 wks of tray 13 to make sure I wasn’t sick. Was clear to go in (at 4wks of tray 13), got x-ray/assessed, dentist said ther’s the slightest line where there could have been a crack but it’s definitely not cracked now (I take Vit K2 in a multivitamin format, maybe in the 6wks enough repair happened, OR maybe it wasn’t cracked at all) told everything but that space looks good but lets wait 2wks longer with this tray to see if anything changes with the tooth’s mobility. Now been 2wks, saw dentist again, still wobbly (I can’t move it with my tongue, or tongue can’t perceive it). So I’m moving on to 14 but will need to have a visit with my ortho for a followup to get his opinion, if concerned or if it’s expected for what we’re doing + my age. Last visit he had said 10days per tray is good if 7 feels too fast. I am inclined to think no less than 12... I suppose it can vary one tray to the next. Also realized that around tray 12 I was taking in far less calcium. I don’t drink dairy, rarely eat it (cheese, if I eat pizza basically). I mainly get calcium from almond milk and whatever greens/veg/fruit/nuts/seeds I put in my daily smoothie and my multivitamin. Still that’s not enough. So I bought calcium supplement that contains Magnesium and D3 and hopefully this will be a help to my teeth and bones. Especially at my age. I hope there a video on the use of “chewies”... I need an explanation about whether it is purely to ensure the fit is proper or if there is also some science about what that does to the gums/bone etc and speeds things up. Been confused on that subject. The “chewies” my ortho hands out are the plastic suction sticks they use to suck your saliva out during procedures. They have a wire to hold a bend in them. So biting on a wire (though covered by the plastic) is what I thought cracked my tooth. I was chewing when I heard the crack... going to buy some ‘Y’ shaped silicone chewy tool on Amazon today. Hopefully that will ensure I don’t crack teeth/bone in the future and be much friendlier (and sanitary).
You’re welcome. We hope TrayMinder will be helpful for you. There is a video on chewing exercises. The main use is to help seat your aligners better so that your teeth move more efficiently. There is no research that shows that using chewies will speed up tooth movement in a biological manner.
What do you think about 7 day changes vs 10 day changes on someone in their late 20s? Are 10 day changes the sweet spot to wait for osteoblasts to do their job?

My dentist recommended to wear my aligners for only 10 days at a minimum of 18 hours a day. They reassured me that it's fine as long as I follow it closely, so I've been ensuring that, but is this too rapid for proper resorption/rebuilding?

I have a question! is having a midline in the middle important? or is it just for aesthetics? i got a tooth pulled out when i was a kid so my ortho had to take the tooth on the bottom out as well. I really don't want to take more teeth out just to put my midline in the middle though..
Thanks for the nice comment. Regarding midline, it depends on many factors. For some patients, having coincident midlines is not worth the increased treatment time/efforts required.
You are absolutely amazing and informative. At 30 I've started my invisalign journey. My treatment is set for a new tray every week 36 weeks . After listening to your description I worry if 7 days will be enough time with each tray ⏲️.

Thanks for your kind comment. Age is not a concern as long as your dental health is in good order. A healthy periodontium is important when moving teeth. Some of my patients are in their 70’s and they have obtained great results, both with braces as well as clear aligners. Please seek the advice of an orthodontist. Good luck.

I have a 5mm midline diastema on my upper row that I’m going to close with Invisalign. Would you recommend having buttons put on teeth 1&2. I’ve seen someone have this, but I don’t think my orthodontist is doing this for me and I’m concerned. 😢
I'm confused about the statement at 2:45, that the bone breaking cells only begin to work after 4 hours of continuous force. If less than four hours have passed since I returned the trays to my mouth after a meal, let's say 3 hours, and I take them out to eat again, does that mean that in that 3 hour segment there was no movement?
The osteoclasts begin to break down bone after about four hours of continuous light pressure (from aligners are braces). There will still be tooth movement that can occur with intermittent forces (think thumb suckers and tongue thrusters) but the tooth movement is less efficient and can take much longer. www.sciencedirect.com/science/article/pii/S1607551X17307647
Great video! Really appreciate the detail around bone remodeling. I’m starting Invisalign tomorrow. By the end of my treatment, if I’ve successfully moved teeth and the bone has remodeled appropriately, why do people need retainers? It seems there is not pressure on the teeth to move back. What causes teeth to want to move back to where they were?
Teeth, like all parts of our bodies, are prone to changes with time and aging. Teeth shift due to a thing called mesial drift, a tendency for teeth to drift towards the center/front.
I am also one of those people who need to understand the why and how behind something so that I can fully commit to getting onboard with it. You accomplished that! I do have a question too: if it takes a minimum of 4 hours for the process to start and 6 is an ideal amount of time for pressure, then isnt taking out the aligner to eat a meal causing us to lose progress between breakfast and lunch? Incentivizes eating quickly but this is hard when teeth hurt lol

Hang in there, it will get better. Tylenol (if not allergic) is good for pain management, decrease number of times you take aligners in and out, and doing chewing exercises with chewies (see chewie video) to stimulate blood flow are general recommendations. Please reach out to your orthodontist if you have any concerns or if the pain lasts more than a week. They will be the best to guide you on what to do.
Hi, Im on my 2nd tray and I have been wearing them for 22h per day. However I take them out more around every 3rd hours someone for only a few minutes to have small meals. I do heavy weight lifting and im constantly hungry, before invisalign i had aprox. 6 meals per day which im trying to change. Is this bad for my tracking that i take them out more oftan than every 4h but only for like 10min-15 min each time? Thanks for great videos!

Maybe you could teach my ortho how to turn and lean a premolar, spike on spike collision and im in retainers, he sd let my teth settle for 2 months. The retainer is not gunna let this premolar turn lol

About how long does that take? 30min? Also, if my teeth are still a little sore, is that a sign I should continue with my curent tray until a have a few days that are completely pain free? Thanks, I’m glad you found the video to be helpful. Usually I advise my patients to not leave aligners out for more than 45 minutes. However, special occasions warrant flexibility. As far as your sore teeth, please ask your orthodontist, as only they know the details of your treatment.

SO HELPFUL!! Thank you! 😃 Downloaded the app last night to try. I’m an older patient (39), some teeth are wobbly, one more than the rest. X-ray shows tooth and gums/bone are in good condition but one tooth in particular has more ‘space’ around it (i forget what that area is called), I guess from the movement and changing trays too quickly for some teeth (7-8 days). I’ve been wearing tray 13 for 6 weeks because I heard a loud crack early in tray 12, but that was just before new years. Waited it out as I didn’t have any real pain. Didn’t know if it was the tooth, the tray, or what. Switched to tray 13 after 14days, had more pain, particularly that tooth. It felt weird, extra sensitive to touch, when wiggle or press on it could hear a really quiet click. Had an appt but then my tonsil was sore and with covid being a thing these days I felt I needed to be responsible and reschedule. Pain was not anything unreasonable but naturally if I did have a cracked tooth/root I didn’t want to switch trays yet w/o knowing what was going on. So, another 2 wks of tray 13 to make sure I wasn’t sick. Was clear to go in (at 4wks of tray 13), got x-ray/assessed, dentist said ther’s the slightest line where there could have been a crack but it’s definitely not cracked now (I take Vit K2 in a multivitamin format, maybe in the 6wks enough repair happened, OR maybe it wasn’t cracked at all) told everything but that space looks good but lets wait 2wks longer with this tray to see if anything changes with the tooth’s mobility. Now been 2wks, saw dentist again, still wobbly (I can’t move it with my tongue, or tongue can’t perceive it). So I’m moving on to 14 but will need to have a visit with my ortho for a followup to get his opinion, if concerned or if it’s expected for what we’re doing + my age. Last visit he had said 10days per tray is good if 7 feels too fast. I am inclined to think no less than 12... I suppose it can vary one tray to the next.
Also realized that around tray 12 I was taking in far less calcium. I don’t drink dairy, rarely eat it (cheese, if I eat pizza basically). I mainly get calcium from almond milk and whatever greens/veg/fruit/nuts/seeds I put in my daily smoothie and my multivitamin. Still that’s not enough. So I bought calcium supplement that contains Magnesium and D3 and hopefully this will be a help to my teeth and bones. Especially at my age.
I hope there a video on the use of “chewies”... I need an explanation about whether it is purely to ensure the fit is proper or if there is also some science about what that does to the gums/bone etc and speeds things up. Been confused on that subject. The “chewies” my ortho hands out are the plastic suction sticks they use to suck your saliva out during procedures. They have a wire to hold a bend in them. So biting on a wire (though covered by the plastic) is what I thought cracked my tooth. I was chewing when I heard the crack... going to buy some ‘Y’ shaped silicone chewy tool on Amazon today. Hopefully that will ensure I don’t crack teeth/bone in the future and be much friendlier (and sanitary).
You’re welcome. We hope TrayMinder will be helpful for you. There is a video on chewing exercises. The main use is to help seat your aligners better so that your teeth move more efficiently. There is no research that shows that using chewies will speed up tooth movement in a biological manner.

Hi can you do a video on, How to know if you will need elastics with your Invisalign? because I will be getting my Invisalign at the beginning of January and I'm curious on if im going to have to have elastics. I also have a open bite so hopefully this will give you and idea. 😬
Please watch the rubber bands 101 video. It will cover most of the bite issues that patients can encounter. Your treating orthodontist will be the best person to answer any specific questions regarding elastic wear. Good luck with your treatment:)

I’m new into this, week one, but the trays are tearing up the inside of my mouth and I always feel like I’m rushing to eat. I hate that.
My dentist recommended to wear my aligners for only 10 days at a minimum of 18 hours a day. They reassured me that it's fine as long as I follow it closely, so I've been ensuring that, but is this too rapid for proper resorption/rebuilding?

I have a question! is having a midline in the middle important? or is it just for aesthetics? i got a tooth pulled out when i was a kid so my ortho had to take the tooth on the bottom out as well. I really don't want to take more teeth out just to put my midline in the middle though..
Thanks for the nice comment. Regarding midline, it depends on many factors. For some patients, having coincident midlines is not worth the increased treatment time/efforts required.
You are absolutely amazing and informative. At 30 I've started my invisalign journey. My treatment is set for a new tray every week 36 weeks . After listening to your description I worry if 7 days will be enough time with each tray ⏲️.

Thanks for your kind comment. Age is not a concern as long as your dental health is in good order. A healthy periodontium is important when moving teeth. Some of my patients are in their 70’s and they have obtained great results, both with braces as well as clear aligners. Please seek the advice of an orthodontist. Good luck.

I have a 5mm midline diastema on my upper row that I’m going to close with Invisalign. Would you recommend having buttons put on teeth 1&2. I’ve seen someone have this, but I don’t think my orthodontist is doing this for me and I’m concerned. 😢

I'm confused about the statement at 2:45, that the bone breaking cells only begin to work after 4 hours of continuous force. If less than four hours have passed since I returned the trays to my mouth after a meal, let's say 3 hours, and I take them out to eat again, does that mean that in that 3 hour segment there was no movement?
The osteoclasts begin to break down bone after about four hours of continuous light pressure (from aligners are braces). There will still be tooth movement that can occur with intermittent forces (think thumb suckers and tongue thrusters) but the tooth movement is less efficient and can take much longer. www.sciencedirect.com/science/article/pii/S1607551X17307647

Great video! Really appreciate the detail around bone remodeling. I’m starting Invisalign tomorrow. By the end of my treatment, if I’ve successfully moved teeth and the bone has remodeled appropriately, why do people need retainers? It seems there is not pressure on the teeth to move back. What causes teeth to want to move back to where they were?
Teeth, like all parts of our bodies, are prone to changes with time and aging. Teeth shift due to a thing called mesial drift, a tendency for teeth to drift towards the center/front.
I am having Invisalign lite treatment. can aligners moved teeth as well as jaw also backside like teeth.
Great question. Orthodontic treatment moves teeth within the bone, not the jaws.

I do have a question too: if it takes a minimum of 4 hours for the process to start and 6 is an ideal amount of time for pressure, then isnt taking out the aligner to eat a meal causing us to lose progress between breakfast and lunch?
Incentivizes eating quickly but this is hard when teeth hurt lol

Hang in there, it will get better. Tylenol (if not allergic) is good for pain management, decrease number of times you take aligners in and out, and doing chewing exercises with chewies (see chewie video) to stimulate blood flow are general recommendations. Please reach out to your orthodontist if you have any concerns or if the pain lasts more than a week. They will be the best to guide you on what to do.
Hi, Im on my 2nd tray and I have been wearing them for 22h per day. However I take them out more around every 3rd hours someone for only a few minutes to have small meals. I do heavy weight lifting and im constantly hungry, before invisalign i had aprox. 6 meals per day which im trying to change. Is this bad for my tracking that i take them out more oftan than every 4h but only for like 10min-15 min each time? Thanks for great videos!
Great question! It sounds like you are very conscientious about your aligner routine and fitness. It’s generally fine to take them out to eat and replace shortly after :)

Maybe you could teach my ortho how to turn and lean a premolar, spike on spike collision and im in retainers, he sd let my teth settle for 2 months. The retainer is not gunna let this premolar turn lol

Thanks, I’m glad you found the video to be helpful. Usually I advise my patients to not leave aligners out for more than 45 minutes. However, special occasions warrant flexibility. As far as your sore teeth, please ask your orthodontist, as only they know the details of your treatment.
