At the beginning of the first episode, Elsa says, "My father had three children [Elsa, John, and Spencer]. Only one would live to see their own children grown. Only one would carry the fate of this family through the depression and every other hell the 20th century hurled at them." Now that John is dead, that has to mean Spencer lives, right? Like you, I also feel like he's going to die, but if we take that line literally, and I can't see any other way to take it, he has to live for some time.
My theory is that John sees his child grow to be fairly old and dies before he can see the ranch more secure, but that Spencer carried the ranch/family through the rough patch but doesn’t get to see any kids grow up.

Likely over 18 now hir wasn’t when the war happened cauze that’s why Spencer would’ve gone instead but then again who knows. Yellowstone loves to do the opposite of your thinking some times
I am not a doctor. And have very little medical training other than the basic first aid that the military teaches people. So someone correct me if I am wrong. But I think the doctor said no aspirin because even back that aspirin was known as a blood thinner. Her being shot you want her blood to clot so it would stop going OUT of the wound. And those promote scabbing and healing.
