The hospital scene was never improvised. The detonation is on a timer and once triggered, it blows up. All of it blows up. If there was a pause that nobody knew about, that would be putting others lives in danger and impossible to fix. It was a delay, not an accident.
What about DiCaprio cutting his hand on the broken glass and then covering said hand over Kerry Washington's face in Django Unchained? Washington let out a cry and everyone else was baffled by the performance; so much so that DiCaprio was given a standing ovation after the director said "cut".
Another favorite of mine is in the Pirates of the Caribbean series, when Hector Barbossa comes back from the dead and none of the cast was expecting him to come down those stairs. All of their shocked reactions were genuine.
I also heard that margot from wolf on wall street really slapped the shit out of him because of that scene and was actually scared he was going to sue her, but he just rolled with it.
In Captain America: Civil War, when Tony is recruiting Peter, Robert was going to sit on Peter's bed, but Tom hadn't moved, so Robert said his 'I'm gonna sit' line. Unscripted but fit both of the characters perfectly.

He kept in character the whole time and Keri's reaction to the blood smear is genuine. He needed several stitches in his hand almost surgery.
Harry Potter and the Deatlhy Hallows part 2: If you watch the movie when that improvised hug happened, you can see Helena Bonham Carter actually looking at the camera.
