Direct Ophthalmoscopy use of the eyes is a very challenging clinical skill, incorporating both the examiner's knowledge of the retina, but also understanding the use of the ophthalmoscope
In this clinical skills tutorial, we look at the use of the direct ophthalmoscope as part of an ophthalmic examination
it should be noted that in the ideal circumstances, the room lights will be dimmed during the examination, and dilating eye drops used to improve the visualisation of the fundus
Some people may notice an ASMR effect from this clinical examination

Theres one other doctor I know of who does these sort of things but with intention of making asmr, cant recall his name atm. But yes, as part of the asmr community, I just discovered this man ❤
@@willwalk2000SRP sounds right up your alley. He’s an optometrist who does intentional ASMR, but also speaks in a way that makes it sound unintentional so it’s that perfect mixture of the two.

As an autistic person I really appreciate the fact that you explain in detail what the examination will be like before carrying it out. I know I would feel much more at ease during medical appointments if more doctors did this

I ask every doctor, if they could explain or tell, what they are going to do. Most of them did.

@@Triattt I couldn't figure that out, because I left. 😄 Maybe they are tired of listening to problems 🤷🏼♀️ or annoyed by Doctor Internet (what is understandable in some point) or just hate to explain.

I'm not sure why but I've always loved eye exams - I'm always so fascinated by anything outside of my own research field (English Literature) so I'm that one annoying patient that will ask you a million questions about how it works, what you're looking for, etc. These videos definitely help to satiate that curiosity and, hopefully, make me a better patient! Thank you Dr Gill :)

@@DrJamesGill I fear there are some GPs that don't share your point of view. I knew one who's take on it was 'What's it got to do with you'?
I think there is a difference between the patient being engaged with their health, and turning a discussion of two experts - the patient in how their body feels and the doctor in their area of medicine, into a two person discussion with commentary from Dr Google on the side.

I know we all have our different triggers, but this extreme proximity combined with soft speaking is wildly intense. You could do hours if this and I wouldn’t tire of it.
I have zero experience in the medical field, but I’ve actually learnt a fair amount just from watching Dr. Gill for the last handful of years. It’s all so fascinating. Not to mention, these two make such a great pair.

I wish local doctors were that gentle and polite.
Honestly we try to be gentle and calm but sometimes events with another patient can be difficult to clear in the few moments between patients

We’re all still working. Just there isn’t enough capacity.
I was talking to someone today, that technically a GP surgery has to see ALL patients who request an appointment
The problem is there are not enough doctors to meet that need.
I don’t know about you, but lasts time I was seen (I needed an MRI as part of a possible cancer work up - it was negative) I don’t want the doctor who is doing that work up to have seen 60 people that morning, and is utterly shattered for the afternoon
As an American I appreciate the Dr’s vocabulary such as “brilliant” and “grand” and “are you happy to proceed”
I haven't felt the scalp tingles in a long time but this did it! ASMR effect indeed.

In watching all of the eye exam videos you remind me so much of my life long eye doctor who retired a few years ago because he had a very similar voice and it’s so calming/nostalgic to me that it always leaves me with a happy feeling after the video ends! Hopefully the new eye doctor I’m trying to set up an appointment with will have the same caring effect on me, I’m badly in need of a new prescription, lol…

I was diagnosed with Papilledema related intercranial hypertension. It wasn’t until getting my eye exam after almost a complete month of being ignored by general practitioners about my symptoms did I get told to see my Optometrist and I am so thankful for her every day because she kept me from losing my vision completely!
Hence why ALL patients which blurred vision need to be seen. Plus I always advise patients with blurred vision to see the ophthalmologist.
Many don’t like being told that, as it’s an extra step for them, but the retinal photography is much better, and thus safer, than just using the ophthalmoscope
I’ll be doing another video on hypertension related changes, as they are a little different

I'm not a doctor but the one thing I've learned from Dr. Gill is that my current doctor isn't much of doctor either.
These demos are very much driving standard in terms of “mirror, signal, manoeuvre” so in the real world things can be a bit faste r
I'm not sure how the auto-focus works on your camera, but when it's angled so that you're the one facing the camera you should try to switch the setting to "eye-tracking." It'll help to keep you in focus as you move around the frame.

Warwick takes people of all age ranges. I think the oldest student we had was ~50
I sadly had to quit med school due to monetary issues, but when I was there I had classmates above the age of 50! According to my professors, it is not uncommon to see people becoming MDs after their 60’s, they have a good decade of practice and fulfill the dream they couldn’t during younger ages. Go for it!!!

I have one of my 4th year OSCEs next week and these are great since we have gotten a lot less face-to-face teaching and practice opportunities due to coronavirus disruptions. Hey Dr G. Thanks for all your content. Hugely informative and really well presented. Is there any chance you'd do a full body check up on someone?
There isn’t really a “full body check”
We start in the first week of term by teaching the “general examination” but that really is a crutch to get the students to understand how to start examining the body.
My response to why I don’t like it is I’ve never had a patient come to me an say “doctor, my generally feels a bit sore, could you examine it”
Yes that is hyperbole, but we should be able to narrow down what we need to be looking for from the history.
Keeping in mind, 70-80% of our differentials should come from the history, and realistically the examination should be used to try and refute those differentials 😊

What I was more angling towards was something along the lines of the examinations where almost all aspects of your makeup are checked but I suppose if one was to amalgamate all the videos you have done into one, all those fields would be covered!

I see a lot of English speaking real doctors or doctor roleplay include the detailed explanation and consent before proceeding, it’s pretty re assuring and nice.
In my whole life living in France it happened to me two times only.
And I have been seeing a lot of doctors and specialist all the time :/
Apart from being polite, I think it’s useful for the patient. So they have an idea as to what is coming, but also gives them time to get their thoughts in order to be able to give the clinician adequate feedback from the examination 😊
When examining the back of the eye, what abnormalities are you looking for? Also, how different would the abnormality be compare to a healthy eye?

Quick question doc, with partial vision how does this affect the eye exam, I assume it'd be recorded in the medical history. Just curious is all, thanks in advance
Do you mean for the examiner or the patient?
@@DrJamesGill Patient, I would assume.
This exam is to check for eye health such as health of the retina and the blood supply. I’m partially blind in my right eye - it actually helps as you won’t be aggravated by the light as easily :). Sometimes partial sightedness can affect how wide the pupil opens so makes it slightly difficult for the doctor to see in but then you would likely be seeing an optometrist with better equipment :)

I have a slight issue with my right eye, so clinically I’ll do the examination with the left eye, but highlight it to the patient
