Actually, in the book, the illusion disc fell off of her when she slipped, revealing parts from the junkyard (a bike as her body, for example) and as she hobbles home, she disintegrates to junk and finds Eleanor in the garage, where she finds a bunch of trash bags with Sara's body parts inside them. Eleanor takes the illusion disc and puts it on - she is now the old version of Sara. And Sara dies, realizing how beautiful she actually was.
