Is his concoction something like “extract of ghoul liver”? Is that why they harvest ghoul organs at Super Duper? I think there must also be a non-ghoul-harvesting means of manufacturing the vials that existed before anyone became a ghoul, maybe something to do with how they became ghouls in the first place. Do massive doses of rad away turn one into a ghoul? Perhaps the treatment for massive radiation exposure involves a cocktail of chemicals, including rad away. That treatment keeps you alive but turns you into a ghoul, and turns you into a feral ghoul if you don’t get daily maintenance doses.
In my mind, Lucy’s use of force against Martha is justified because it’s clearly self-defense. On the other hand, it’s a lesson that good intentions are not enough, that good intentions can lead to very bad results. One must be intelligent when implementing one’s good intentions. Sometimes our first impulse to action is good, other times it can be very bad even when we intend good. (And if we compare and contrast Lucy killing Martha with The Ghoul killing Rodger, we can see how a bad intention-killing someone for their teeth and meat-can lead to a good result, the mercy killing of someone who is suffering.) This show is super nuanced and subtle.
Yes, ghouls are heavily irradiated people, who "survived". There are two types. "Normal", still sane, who trying to join society and to cooperate with people and "feral", whos irradiation sickness/decay caused them to be mad animals. If I am not wrong, in most (or all) cases its one way road and probably all ghouls will eventually get feral, except few. Show gave more insight in to whole "ghoulification" process and themes, games are more obscure and open about this.
The rules are different in the game universe. Ghouls go feral based on both genetics and radiation exposure. Basically if a ghoul has good genes, and keeps the rads down, they are effectively immortal, while others with bad genes turned feral from the jump and stayed that way, with room between the two. The "Anti-Feral Chem" in the show is unique to the show, as is the idea that all ghouls are fighting off the feral.

At the lower end of high levels, you might even survive for ten years but suffer from cancers and organ failure. People in real life don’t become ghouls from radiation exposure. You’re either dead or you’re sick and you die. Exposure is also cumulative. If you got a chest X-ray everyday, you’d eventually have accumulated enough exposure to get fatally sick. We are exposed to radiation every day. But don’t worry! In most cases the daily exposure is low enough that the accumulation doesn’t get us before we die of natural causes. Just in case, though, you should use sunscreen. 😅 Solar radiation is radiation. But back to the ghouls. I think ghoul-ification has more to do with the treatment for massive radiation poisoning than the radiation. It’s the treatment (rad away or something else) that keeps the victims of massive radiation alive that turns them into ghouls. Maybe it’s the massive amounts of rad away that they need to take, or something else in the treatment regimen, but not smaller doses one takes from lesser exposure (like you’d get from drinking or eating substances that were still radioactive). I suspect that the vials contain a cocktail of rad away and other stuff. Does my theory comport with game lore? If so, could you expand on my speculation? Or maybe radiation works completely differently in the game than in real life. 🤷🏻♀️
You describe the Vault dwellers really well. Awkward and puppet-like. I feel like the awkwardness comes from never having to deal with the outside world til this situation and now they don't know how to be normal around each other. Also you probably noticed this when you were editing but Lucy mentions how her mom died in the scene when they were butchering The Ghoul's friend. It was during the Great Plague of 77. They couldn't work the farm so people starved including her mom.
And this is the part where The Ghoul starts to question himself because, despite everything, Lucy still refuses to compromise her core principles for the sake of survival.
