Pancreas: Redemonstrated heterogeneously enhancing ill-defined mass centered within the pancreatic head measuring approximately 2.3 x 3.0 x 3.2 cm (AP x TV x CC) (series 3, image 261; series 601, image 50). There is resultant dilation of the pancreatic duct and minimal surrounding fat stranding. The mass extends along the right superior mesenteric artery with greater than 180 degree encasement (series 2, image 70). There is narrowing of the proximal portal vein at the SMV/splenic confluence (series 5, image 151). This narrowing is less than 1 cm from the drainage of gastrocolic trunk. QUESTION: Does this encasement of the right sma make it unresectable necessarily?

Not necessarily but this patient needs neoadj chemotherapy 3-4 months. Artery first is the way to go plus periarterial divestment.
