When looking in the ear with the otoscope it's better to brace your hand that holds the otoscope against their face as a stabilizer in case they move suddenly (esp. with kids) or you/your arm is bumped, to prevent puncturing the TM, or scraping the ear canal; also pull up and back on the auricle (for adults, down and back in kids) to straighten out the EAM so you can look straight in there :)

And you don't do the pulmonary auscultation and percurssion all the way down the back, just until you reach the diafragm, and in the lower thorax you should percute and auscultate the lateral areas also.

If this were a real exam she would of failed 10 times. You can't pickup heartbeat irregularities when you put the stethoscope in a spot for .2 of a second.
I used to use this video as a drinking game and take a shot everytime Kaylee said "umm" i'm now living with chronic liver faliure.

Some people have random muscle twitches. Like today my leg totally has a reflex like random movement. Didn’t even try.
Maybe her body reacted in other words was scared and bracing the reflex hammer that she had.

And you say you felt the aorta but you didn't preform the correct techique for palpating it, in a real assesment you would miss it!
