There is an entire episode of Hard Copy dedicated to this incident and it ended up turning into a hit piece on Trent Reznor.

Yep, true story.
Reminds me of the late 1980's, a German band was seeked by the local police because they came across a cassette of (music) video (without the music) and mistook it for a snuff film. Not because it contained violent images, just odd ones. I dont remember the band's name, and rather not give the wrong name, but they were industrial experimental as well.
There's a couple more situations like that. Charlie Sheen once called the FBI after seeing a Japanese gore movie thinking it was a real snuff film, and the special effects director for the movie Three Kings (where short scenes show injuries described with realistic looking organs being damaged by bullets) was also contacted by homicide investigation after allegations that homeless people had been killed to use the bodies as props.

In a torn down structure with no roof, a music video crew strapped a balloon to a small video camera and filmed while it slowly floated upward. They lost control of the balloon and it floated away, landing in a field.😅 The film was odd enough for police to get involved when found..

How did the footage make it to the music video if the balloon got away?
My guess is that they used other footage on other cameras or re-shot the footage with a new balloon, or maybe they recovered this footage and used it? I can't remember if this is actually in the final video
They did not only have one camera. They rerecorded it after the one who flied away couldn't get back.
