You need a lot of rocks, a star, and an atmosphere.
Well first you need a nebula and then compress it to form a star (G type or K type is good. Aka Sun like stars) than you need a bunch of materials to make rocks that clump together and form planets. Wait for them to cool down and there you have it

1. Get/create enough matter to create your planet. As a reference, Earth has a mass of about 5,972 * 10^24 kg, but you can make your planet smaller with no problems.
2. Get all the matter you need and reunite it all near, but not too much, a star. It'll start attracting itself.
3. Wait for a few million years until the planet is stable enough.
