      Dr. David Geier today i fell 15 feet or so off a grain buggy and doctors told me i tore my acl and pcl plus had multiple fractures behind my knee cap and they told me i will have to have surgery to repair them some day next week

      35 years complete tear pcl. Followed the rules cycle , leg extensions with weights. No latteral sports. Some arthritis

    i got my PCL torn in my right knee without any other injury in other ligament during a rugby game, my doctor told me to have physio first, strengthening the muscle around my knee and see what will be the result, and make decision whether surgery or not.

      jj jai same her . my left knee form soccer match.. From 8 weeks ago swilling and pain in the bignning I can't move or bend my knee.. But know I can run and bend it 90% I can't full bend my knee.. the swilling and pain is only 20% now, I have to finish 2 months rehap than training .. It is clear that the thigh muscles and knee ligaments have reduced the injury Because it is strong unlike non - athletes

      Maher SG i just got the double bundle pcl reconstruction surgery, the doctor said, i have to walk with crutches for 6 weeks, looking forward to walk without it.

      After injuring my PCL while chasing my two year old. I had swelling and finally found out it was a sprain pcl. Dr. Is having me do pt and check in in six weeks.

    I have a PCL avulsion fracture where a piece of the bone was detached from the main bone. Surgeon suggested reattaching it with a screw through surgery but also explained the risk of this type of surgery, partly because incision is made from behind the leg and the risk of cutting into one of the arteries. He also suggested working on strengthening the quadriceps. MRI did show an 8 mm dislocation, but with the risk of surgery and with no other surgeon who can do this type of procedure in a small town, I was advised to search for a PCL orthopedic surgeon from out of town, which seems like the most appealing thing at this point.

      I have the same PCL avulsion fracture where the chip came out from the lower tibia bone.. I haven’t gone for any surgery and left the knee in rest mode for 4 weeks.. now it’s perfect and re attached at the same place

    Tore my PCL, LCL and Meniscus had to have surgery and feeling great after!! Would highly recommend getting it done if you can!!

    I was told PCL surgery was optional in my case based on the severity of my injury. My PCL was detached from the bone, but the knee is not excessively unstable. This being said, I chose to not go with surgery because it did not give me guarantee that I would not have OA in the future. Also, the possibility that the graft could fail in the future was not appealing in choosing to do a surgery that would require 8-9 month rehab. Instead, I chose to go with a PCL brace as non-operative treatment in addition to PT. This seems to be working for me so far.

    i had to run 3.0km in the military with two PCL injury,and it fking hurts,i couldnt even walk or sit down cross legged on the ground after the run .

      I couldn’t. ): gosh I have a PCL tear and it hurts to stand & I have no balance. Running would make me cry

    I have a partial tear in my pcl and my first doctor wanted to push for a surgery I had to change doctors due to insurance and things but now today I start my non surgical approach to this pcl injury if it heals correctly after 6 weeks of PT I’ll be good but if not doctors says we will talk about surgery.

    So 3 years ago I was wrestling and my drill partner “shot” on my leg when it was completely extended and it caused my knee to hyperextend. I felt a pop and it gave it twice right after that practice so I saw my surgeon who had done my shoulder surgery the year before. MRI came back normal but my knee never felt quite right after that. I did PT and was able to get back into wrestling however there was always instability and a lot of rotation with my tibia. Fast forward two years later, I ended up having two meniscectomies done with a different doctor (team doctor at my college) to that same knee because I tore the lateral meniscus. 6 weeks after the last surgery, I was walking up some stairs after doing PT and when I planted with that foot, I felt and heard a rip and my knee gave out. It didn’t hurt right away but about 20 minutes later I could barely walk. I saw the team doctor and he didn’t want to send me to an MRI or do anything so he just gave me a cortisone shot and it never really did much. It wore off a few weeks ago and I have had an immense amount of pain and my knee has become more and more unstable and I saw the first doctor this past week and he fears I may have either stretched out my PCL or torn it, as well as possibly damaged my ACL, and maybe also damaged the PLC due to the amount of rotation there is. Considering it’s been an ongoing issue for 3 years and I’ve done PT on three separate occasions with really no avail, would surgery be recommended if the PCL is stretched out? I mean at this point I’ll do whatever if it means no more fear of my knee giving out while I’m walking especially going down stairs, no more pain after walking for more than 10 minutes, and no more pain when kneeling down. I can’t even bike anymore because it hurts and feels like my knee will pop out the back and I tried swimming and I could actually feel my tibia moving around every time I kicked. So yeah I’m awaiting an MRI on the 2nd of April and a possible visit to a knee specialist.

      It sounds like you may have injuried your pcl initially. Once the pcl is injuried/weakened there is a greater chance of injuring the surrounding ligaments. I am not in medicine but I did have pcl reconstruction surgery 6 months ago. I had similar pain that your having now, prior to surgery. I especially felt it when I tried to squat down. Mine was a grade II tear. Which means it was torn about halfway and lose. My surgeon recommended surgery due to my job requiring me to be physically active.

      Jj City I ended up finding out that I have ACL insufficiency which explains why I might have so much instability. While my ACL is there, it is so thin that it basically isn’t doing its job when weight bearing. My surgeon was going to wait until the end of this month to improve with PT however, my knee hyperextended the other day while I was moving my phone and towel off a table while at PT and I was twisting my knee at that moment. My knee started buckling and I caught myself before I fell and it started hurting to the point where I could barely breathe. My PT tested my knee and he noticed that my knee is now much more loose. I actually felt like there was not an endpoint when he would do the anterior drawer test and I felt my knee slipping back into place. I had to call my surgeon and they bumped my appointment up and chances are I am going to need an ACL reconstruction since I have not had any improvements with being in PT for a month already.

      Jj City thanks! I appreciate it, I’ll definitely keep you updated. Next Monday is the moment of truth. My surgeon may want to do the surgery soon too which means I’ll spend my summer this year possibly recovering. It’s a little bit of a burden because I’m in my undergrad portion for physical therapy so I need as much money as I can get over summer but hey, health is important!

    "non-united fracture fragment is seen at the tibial attachment of pcl with laxity and few intact posterior most fibres." This statement is written in my MRI report. Can my fracture heal? How much time will it take to gain my pcl back as it was?

    Got a double bundle Tibialis Anterior allograft reconstruction with a Kevlar taper. Of course, you need reconstruction to shore up your leg in the future and protect it from wear and tear. Although they say if you aren’t active you don’t need one, but that's false ethics in my mind as that just means they put you on a not-needed shelf, what about if you want to be active in the future? Get it shored up instead of leaving that doubt in your mind. Partial tears might heal if braced but not full tears.

    tore my pcl in my left knee, 20-dec-15, skiing, mri showed complete tear. possible meniscus damage, possible slight mcl tear, discussion ongoing between radiologist and orthopedic dr. to determine severity of those injuries. Orthopedic dr. has won in my opinion so far as I'm not excited about surgery. starting 6 weeks pt this week 1day/week. back to see the initial orthopedic dr at the end of 6 weeks. we are going the non-operative route first.
    have an appointment with another orthopedic dr in about 10 days, for a second opinion.
    right now just R.I.C.E. and ibuprofen.
    swelling is subsiding, knee pain is still well noticeable and have issues finding the correct position for rest.
    perhaps the diagnosis of the medial meniscus and the mcl may vary as the swelling goes down, and range of motion is becoming greater? (yea there's a sharp pain in that area at times, and in certain positions, just doesn't seem consistent though)

      My experience is same. I got my pcl ruptured completely alongwith the high signal seen in posterior horn of medial menisci. No pain, no instability. 2 orthopedic surgeons have asked me to strengthen the quads only. What is your take dr?

      I would like to know how are you doing now? did you get the surgery for PCL or PT was enough for you.
      Last week I injured my knee with fracture, complete PCL tear. the Doctor is saying no need of surgery , only PT is enough and will be recovered in 6-8 weeks. and I am not sure what to do, if I will have my complete knee movement back and would I be able to get back to my physical activities, like ski, running, soccer etc.

      Hi I also had a near complete PCL tear . One doctor told me that surgery is required for reconstruction but the other orthopaedic surgeon said that it do not require surgery. Can anyone help me out here . Those who have not went for surgery , is your leg recovered completely.

    I did no surgery but I’m starting to think I need something. Every time I sprint my knee swells. Did a complete pcl tear with damage to the other ligaments and I fear I need more support. I was a college athlete that tore after my college years and I have lost normal functionality.

      @@juniorowusu_ well first off, if you haven’t had a scope to clean things up I would recommend that before starting physical therapy. I think physical therapy should be enough. Also keep your weight down and limit the amount of high impact activities on that knee

    My daughter suffered a partial pcl tear a year ago. She also suffered hairline fracture to the tibia. Initially we did 5mo of our pt, although there has been some on going instability issues. Now thirteen months later she re-injured her knee, had MRI and X-rays tonight wish us luck tomorrow have a strong feeling she going to be looking at surgery in her future.

    I have avulsion of pcl and complete tear of lcl ligament.. I am not opting for surgery.. Would i be able to recover and walk again??

    My MRI impression , Avulsion fracture of the medial condyle at the MCL origion, with MCL intact.
    Avulsion fracture at the lateral colleteral ligament origion with low grade partial tearing .Avulsion of the posterior crusiate ligament.Complex tear of the medial meniscus body and posterior horn .Large Baker's cyst with posterior rupture .Large joint effusion . The orthopedic doctor sent me home said see you in 4 weeks then physical therapy . I only can bend my knee 30% .This accident happened 3 weeks ago . I am 64 years old and because its my right knew I can't drive . I am by myself I am going to get second opinion . I have the complex meniscus tear i read online that does not heal by itself .I really would like to hear your opinion Dr. David Geir. Thank you .

    Dr, I have an isolated partial to complete tear on my pcl grade 2 and my mental situation is very bad because I am just 23 and I am afraid I won’t be able to play football again. Doctors are saying surgery is not needed but i think it’s because they don’t care if I will have a knee that does not support me on sport activities as long as I can live very basic life which is not fair considering I am 23 only.

      Often grade 2 PCL injuries can heal without surgery assuming there are no other injuries.

    so when I was young I tore my right mcl and meniscus dancing ballet...well...fast forward about 20 years and two years after a back surgery to remove 30% of my L4 disk...well I decided I'd try some ballet again...I landed on my knee wrong yesterday and I'm told it could be PCL...however the medial side of my knee is sore to the touch, my whole back of my knee and hamstring are also tender to the touch...my last knee injury was not tender to the touch...and it did not feel like pressure...does having pain to the touch mean it could be worse than if I weren't having pain to touch?

    How do you guys fix the posterior sagging? Does it go away by itself over time? I'm 4 months in after injury and still in pain and can't bend my knee fully

    Hi Dr.David Geier,
    I have seen your video, it is like a Booster for patients who is suffering with PCL problem. I am one of them. My MRI report says,
    1) Tear of the Posterior Cruciate Ligament.
    2) Small fluid Pocket is seen in the tibio-femoral joint space along the anterior aspect of the ACL. Thin rim of suprapatellar bursal fluid noted.
    can you please suggest am I suppose to go for surgery?
    I like Horse Riding, Swimming, Trekking and Dancing, I am bit worried now will I be doing all this activities after my rehabilitation of PCL!!!!!!!!

      I think many people can do well without surgery for an isolated PCL injury. If they still can't play sports or exercise they way they want, then they can undergo surgery.

    I tore my acl mcl lcl volve tendon and broke bone and its a year after the axident and I tore all like to talk to doctor aok at the energesy room and he will tell you the store but mainly it was a sleding acident and I got my leg stuck at the bottom of this moution and all 3of my cizions hit me at a high speed and my cuzion in the middle hit here leg ob my knee and it launched me into the air and my cuzion broke her feamer and its been 1 and a half yeats ever sence so ya I can run and jump but I dont have a pic with I'm geting one in the summer

    Hi Dr.
    acl pcl ligament involving most of thickness suggestive of complete tear .Dr.has advise me surgery for Acl & said no need for pcl surgery so please advise only acl surgery sufficient.

      For isolated PCL injuries, we often treat them without surgery. Combined injuries often do need both.

    I had MCL +PCL Contusion 6 months ago and was not able to undergo reconstruction. ...Now my MCL has healed completely and also the pcl , I have full range of motion ....my only problem is that I have slight tibial sag ... what is ur advise to me pls

    I met with accident and for 1day I did not had any pain .after that I could not move my leg back (bending) i don't have any pain while standing or sitting but when my leg goes back I have severe pain .I contact doctor he gave a exercise to lift my leg from edge of bed or table. it been 2weeks I still have pain

    Hey doc I've recently had a complete tear of my PCL and partial tear of my posterior horn of my meniscus. I've been in physical therapy for 5 months and it hasn't improved, my orthopedic surgeon recently said that surgery isn't needed but referred me to another surgeon for a second opinion. What are your thoughts?

    so I'm always getting joint pain or breaking bones in the leg area, I have a patella that shifts out of place... I was offered surgery or to do rehab. I chose a few times doing rehab... years later I fell went to the hospital wore a brace for a week, then went back to work. everything seemed normal until today, I was just watching tv, where now I can't put any weight at all on my leg. I'm scared I may need surgery. i'm not sure if I should just skip work and wrap it up myself and give it time to heal or to go see a dr. I had xrays done. nothing is broken soft tissue is normal but the swelling is obvious again.

    doctor please help i have a pcl avulsion fracture, the bone where pcl attach is broke, other ligaments are ok, some meniscus damage. should i get surgery, its been 3 weeks, should i just wait until there is no pain, i dont know what to do, this type surgery is not common

      There is no need for surgery if only the PCL avulsion.. I took only physiotherapy on regularly and continue the excercise after one year of continuous excercise I feel better and I danced agin and not feel very serious issue in balancing

    Almost a month ago I tore my MCL and PCL (via MRI). I just met with my doctor and he indicated no reconstruction at this time. He said both of I’ll heal on their own. If after a month, my knee is still unstable then he’ll conciser surgery. Thoughts?

    Have a question, ruptured my Patellar Doc said we should try and avoid surgery. I can walk with crutches, put full weight on knee. I did notice that when I was swapping out immobilizer braces my calf muscle was sagging, there was no resistance, and hard to flex, but when I'm laying down I can flex it. Does the PCL have anything to do with the Calf Muscle?

      I had a similar accident where i fell off the bike on my right knee. I had physical therapy for 4 months and im on month 6 since injury. My knee has gotten better i can bend knee and walk comfortable. Running not yet. Doctor told me to do exercices till 9 month and then see how it goes. According to my therapist i dont need surgery. I can walk up stairs now and going down stairs is getting better. Ligaments usually take a year to fully heal without any injury. Hoping i can run again!

      It depends on the status of the knee, but physical therapy can usually still help restore knee motion and strength.

    I spoken and discuss with my doctor's after the MRI, my PCL is fully tear and ACL partial, I'm having instability by walking causing my to trip my leg, so we finally discuss for the reconstruction and I just discharge last two days, the brace really limit my daily activity very much as it can only be straighten the whole time... I bet the surgery is just step one... Long recovery... 😭😭😭

    Hi Dr. David,
    thank you for your video. My MRI report says (S/O High-grade PCL sprain) & (Medial Meniscus Tear) . I did the surgery for both and by today I'm completing 9 days after the surgery. No swilling and no much pain so far. But i have been advised not to bend the knee for at least 4 to 6 weeks and not to put weight on it. Do you think bending it will damage the PCL or the meniscus ?
    i have read on some website that after the surgery they need to bend it at least for 60 degrees to make sure it is working fine, but in my case they didn't.
    I'm afraid that i will face difficulties to bend it after 4 weeks!

    I tore my MCL and have an avulsion fracture of PCL ligament. The injury occurred while playing hockey. I've been do physiotherapy twice a week for 12 weeks now. My surgeon wants to see if I can heal on my own, if I'm still unstable after 6 months I will undergo surgery. Do you think this is the best course of action Dr? I thought a multi ligament injury always required surgery. Do you think I could potentially heal on my own? You said in the video if it's MCL, PCL it could potentially heal. Is this right?       @@osamaelbehery yes, I had surgery last January to fix my PCL, MCL AND POL. Definitely happy that I went through it, knee feels much more stable now and I'm back doing everything I was before injury. I thought my MCL may heal on it's own and I would be able to get away with a loose PCL but after a year and a half of trying to get away without surgery and strengthening muscles around the knee it still felt loose so I opted for surgery.       @@osamaelbehery I don't think it will completely heal on its own if the PCL is fully torn. I do think that if you put in alot of work getting your muscles strong around your knee ( quads, hamstrings ) that you can get away without surgery and have a knee you feel confident playing sports with. You may need a knee brace for some things tho. I used an amazing brace that helped me pre and post op called the PCL OSSUR brace. Highly recommend this brace for a PCL injury. It's quite expensive but worth the price if you want to feel like a normal knee without your PCL ligament. Hope that helps, will answer any and all questions if you have more.

    Hi there,
    Thank you for your informative video. I've injured the back of my knee almost 6 months ago. The doctor wouldn't grant an MRI , they said the CT showed inflammation of the tendon and that ice or heat and limited use would improve the injury. Down the track and yes the inflammation and use has improved and pain is less, but anytime I do exercise or even light walking, pain and tightness occurs. It feels if I were to step wrong that something very painful would occur. The pain also started in back of my other knee about a month after initial hyper extension. Is this from over compensation? I'm sick of feeling like the basic movements, standing, sitting, laying straight are painful... What's more , I really want to be able to work out again and feel safe that my legs won't just give. I'm only 39! This is really upsetting me. Do you think it is a PCL tear?
    It popped and stung, when I was leaning into back seat of a coupe car to lift a heavy basket of washing. I kept my legs straight and ever since, the legs have gone from bad to worse :( now when I sit for too long or cross my legs I also feel a lot of pressure and bad blood flow. Maybe if it isn't PCL, it could be the artery or something? It's driving me crazy trying to work out why my body isn't working right anymore. I never had leg problems in my life. Now it's pain and problems everyday. Stiffness and locking knees and joints even foot pain ... I read somewhere that sometimes with hyper extension , you can overstretch the tendon/ligament? And only surgery can fix it? Pls help? Thank u so much !!!!!

      I did the same as well as meniscus tear, and large bone break off tibia. I have not had surgery but once past the 4 - 5 weeks stage improvements started to appear. Now can extend fully. Just today allowed out of the brace.

      Gloria Zermeno I’ve had the same issue. I’ve torn my PCL and MCL and Dr says it will heal on its own. I can straighten my knee but I cannot bend it more than 50-60 degrees until it’s too pain to go any more. No pain otherwise. It’s been 8 weeks now, and slowly (very slowly) feels like I’m getting better. I’ve been told it’s very important to straighten the knee so keep trying!

    1-High grade partial tear of mid third of PCL.
    2- Sprain of the ACL.
    3- Contusion in the posterior horn of medial meniscus.

      @@osamaelbehery yes it was near complete tear . For complete tear, surgery is the only option as it will bind your ligaments and you will recover on an around 80-85%.

    hi doctor, i have had avulsion # of PCL at the site of attachment of tibia.. i went through conservative treatment for 1.5months but it didn't heal.. in review 3D CT reconstruction avulsed fragment was displaced caudally by 5mm, Finally m undergoing Arthroscopic PCl reattachment, n has been adviced 2 months of Non wt bearing.. pls suggest during rehabilitation can i continue my work which requires little running around.. thanks

      It depends on the demands of the patient's job. Generally we limit physically activity that can be harmful to the graft in the first few weeks after PCL reconstruction surgery.

      Some doctors adviced me to surgery and other doctors say conservative... what is ur opionon?

      DrAkrm oqbah My advice u should undergo surgery..displaced # do need sx, conservative mx is being done by old practitioners but results r not definitive with conservative..

      +Diego Ramos Generally multilligament injuries need surgery, although partial tears sometimes don't. It would really depend on the stability of the knee. In your case, your orthopaedic surgeon can examine your knee to help make that determination.

    Dr. My mri report is that pcl tear complete but when I met with my surgeon he told me that as per physical examination it's not tear it is injured dr told me to take 1 months and if I fill any problems than he suggests me what to do . Now I have no any problem I can walk . No any pain in my knee . What should I do.

      @@osamaelbehery most doctors adviced me to do the operation excet one told me that it may heal without operation since the seperation space is less than 5 mm. If did not heal spontaneous it will not that much effect your routine life activities as the ligment participate by 20℅ in knee joint stability. I did not do the operation and i am living now normal i even do not know if it healed or not. I hope you got my point.

    Why doctors dont care about PCL? Because they dont get them as much money because it is less common? Money will always matter, even for doctors, its a business at the end

      The PCL is only injured a small fraction of the time compared to the ACL. And many of those injuries heal without surgery. It has nothing to do with money.

      I received three opinions about my high grade PCL injury and all three doctors says the surgery is more complicated so I doubt money is any issue.

    Sir my problem is .. mid - substance interestital tear of ACL with mild PCL bulking...
    Horizontal tear of the porterior horn of medial meniscus ..
    Now surgery is correct or not for me.

    Hi David, I was recently diagnosed via MRI with a grade two isolated PCL tear. I had the injury 2 months ago and I'm still unable to bend my knee or kneel.
    Is this normal? I'm not seeing any improvement and im concerned something was missed in the mri.
    Could this be a possibility?

      Those can still be tough injuries and often require a lot of work with a physical therapist.

      @@SabbirAhmed-ks2ou I had a surgery when I partially torn my PCL. Method used is called thermal shrinkage which is basically burning the ligament.

    Hello can someone interpret my MRI results, I cannot walk properly and instability issues for 2 months after the incident that happened (May 23, 2024). So 2 months ago I accidentally fell off the stairs from 3 steps above to the ground both knee first, after that, I had pain at the back of my left knee, it was really painful I cannot bend it that much. My other complaint was the instability issues, I cannot walk properly and I was limping. I decided to go to the doctor both Ortho and PT, they told me to go have an MRI scan and after that, I showed them the result and the doctors told me it was a PCL sprain/partial tear. Doctors told me that I only need to rehab this and no surgery needed so I decided to a PT clinic, it improved my pain and flexibility, but I still have trouble on walking and slightly limping but I still do the exercises that my PT gave me.
    So here is the MRI result:
    Impression:
    • Patella is laterally tilted.
    • Small subchondral cyst in the lateral patellar facet.
    • Hoffa’s fat pad mild edema.
    • Femoral trochlea dysplasia.
    • No meniscal tear.
    • Consider posterior cruciate ligament sprain and/or partial tear.
    • Intact anterior cruciate ligament.
    • Minimal suprapatellar joint effusion.
