    I have been on this job until I retire as a nurse with this kind of specialty here in Uk . Been caring on patient with tracheostomy and everthing you can see when you have been caring a comatose patients . I have noticed that his wife was always on the bed , she should be sitting on a specialised design chair recommended by physiotherapist . Our muscles when not been used for a long time tends to have a muscle wasting or contracted . I’m sure she had a Peg tube where the food is given. I saw one woman giving her a purée food via the mouth and that’s a No No . Why , because her swallowing reflex was already compromised . Based in my experience , I’m sure she develops aspiration pneumonia 1. Constantly lying on her back wherein the secretions is pooling around the throat and deep chest , constantly suction every 2 hourly and changing the inner tube of tracheostomy . Caring for this kind of patient is physically and mentally exhausting . In philipines , nurse can’t suggest to the doctor . In the Uk , nurses can suggest on a polite way since nurse know the patient more than the doctor . She cannot go back to her baseline which is totally independent with activities of daily living like , washing , walking ,dressing , cooking and all the things we do normally . Just only a bit of sharing of knowledge with you guys .

      same thought po.i dont know if it was not recommended by her physio or they dont have that chair..but here in singapore those family who cant afford to buy the geria chair.they would recommend the helper or the family to sit the patient in the bed for few minutes with monitoring the BP... i noticed that his wife is kinda always in a flat position its better for them to atleast put in a sidelying position and do some backtapping right?

    • @thesimplekitchen59
      @thesimplekitchen59 Год назад +14

      @@enitsirkgemps6266 , if they can’t afford that specialised chair , the therapist would make the bed to a cardiac position or if she’s lying in bed at least the nurse has to tilt the bed to 30 degrees angle especially if she’s ongoing the feeding .

    Hi Doc Alvin, same situation po sa mama ko, sobrang biglaan, sinugod namin sya sa PGH after 24 hours sa ER biglang bagsak ng oxygen level nya at unresponsive na yung mama ko at kaya need na nila irevive, tapos need ng intubation,naghintay pa kami ng 3-4 days bago sya nilipat sa ward kasi unstable pa si mama nun, kaya lang po after 2 weeks di pa din nagigising si mama at inadvice din ng doctor need sya ilipat sa neck vent kaya naka trache si mama. Then nilipar ulit sya sa ICU, then after a week nag stable na si mama pero coma pa din. After 1 month, biglang nag conclude yung isang doctora na brain dead na si mama at pinipilit nya kami na iistop na yung gamot(IVs, at gusto nya na i-off na namin yung ventilator machine ni mama). Pero mabuti na lang talaga hindi ako nakinig, Kasi after 2 month ng coma biglang nagresponse si mama sa pain at slightly naoopen ni mama yung mata nya, hanggang sa unti unti natataas nya yung kilay nya as response. after din noon, mas tumaas yung chance na maalis si mama sa vent machine kaya after so many attempts, successful syang nalipat sa oxygen support hanggang sa unti unti din sya sinanay ng walang support na oxygen, Then miraculously after almost 4 months naiuwi namin si mama, kahit vegetative state na, bedridden, at na diagnose din po sya ng locked in syndrome after nyang magising, pero talagang lumalaban si mama hanggang sa nakaya nyang iopen yung both eyes at nasusundan nya yung direction ng doctor. Pero gusto ko lang din icredit yung mga student doctors na naging way para magising si mama, talagang effort talaga sila magising lang si mama.
    And yun, after almost 4month naiuwi namin aya sa bahay, nagawa nya din makita ulit yung papa ko, kapatid ko at ibang kamg anak namin,
    kaso talagang prone na si mama sa pneumonia kasi after 9 month simula ng nauwi namin sya, di na talaga kinaya ng katawan ng mama ko, pero salamat na din kasi di na sya maghihirap🙏🙏🙏🙏🙏🙏 kaya laban lang sa buhay. Hanggat di pa kinukuha ni God.

    I agree with you regarding the cause of the increase in NA+ My first guess when you said she had a fever and then you showed the color of her urine is perhaps UTI. Many bed bound patients are at risk of UTI, not to mention pneumonia, nutritional challenges and skin breakdown. We see it a lot in nursing homes here in the USA. I admire Andrew Schimmer and his family's dedication and love in caring for his wife. They're going to need a lot of strength and support. Thanks for sharing!

    I think Andrew and the kids should brace for the worst. While miracles do happen and it is good to have hope all the time, I feel it is best to leave everything to God and accept His will in time. He knows what is best.
    It is just good that she was able to go home and be with her family for a week. That window was a blessing from above.
    Wishing Andrew and kids more of God's strength at a time like this. May the Lord's will be done in Jesus' Name.

    I feel for Andrew, but I think that he should let her go. 2 cycles of CPR that led to anoxic brain injury is irreversible. The way I see it, and this is just me: she’s being kept alive bcoz of the ppl who doesn’t want to let her go. This doesn’t benefit the patient: she’s suffering. I know that she’s being loved and all, but this is just futile. In this kind of state, Ano ba talaga ang goal: to pacify the living (Andrew, kids and parents), or to make the patient comfortable and go with dignity? I don’t question his love, but loving means letting go too. The kids should have good memories of their mom: not memories that they’re constantly in the hospital with their life revolves around mom. Look towards the future. That’s my 2 cents. I don’t expect u to agree with me.

      I AGREE WITH YOU. WHEN MY HUSBAND WAS IN AGONY I WHISPERED TO HIM "I LET YOU GO. I CAN TAKE CARE OF MYSELF" and pray. Seconds he closed his eyes. He suffered so much of lung cancer.

    Been trying to avoid informative videos like this for awhile now due to my own health issues that has not been properly diagnosed for two years. Been feeling alot of pain lately and I do not even know what is going on anymore. However today, I had the courage to watch this video. Ofc not in one sitting because I am too weak to watch til the end. But I hope and pray she gets the miracle she needs. Thank you Doc for being an instrument to people and giving us informations about medical stuffs. God Bless.

    From experienced, wala po akong natatandaan na pasyente namin na status asthmaticus na nakarecover to normal na kagaya ng case na ito ( cardiac arrest, CPR, comatose, brain hypoxia - sa tagal na walang oxygen ang brain) reality po iyan. Most of them, sa katagalan pa organ failure na sila, multiple organ failure - bumibigay na kidney, heart, etc. I am certain na naexplain po iyan kay Mr. Schimmer ng mga doctor. Sana na explain para hindi sya nag accused ng negligence.

      Same here, wala pa akong naging pasyente na nag recover na may brain hypoxia na pag uusapan. Hindi lng maintindihan ng marami at pag nag real talk ka sasabihin mind your own business o kaya masakit na magsalita. Si Andrew kung mag accused akala mo well versed sa medical field, i feel bad for his wife who is suffering.

    Yes you’re right Doctor saw this before and had same thought sodium is bad but I don’t think that’s direct culprit of what happened to her. Feel bad pointing the fault at the Nutritionist. There’s more to it for sure.

    Firstly, I symphatize Him in what they are going through. But, personally I did not like that he pinpoint right away someone and blaming with what is going on to his wife. From day one the medical team worked collaboratively to save Her.I do not know if he has medical background, and if He has, the more that He should understand what is going on. He is just looking on one point which is high Na.He should look at the whole picture. Just saying....Hoping for her recovery
