Final Results of the ASCEND Study on Social Risk Screening Implementation in Community Clinics

  • Event Description: This presentation will provide an overview of findings from the ASCEND study. ASCEND assessed whether providing six months of adaptive implementation support yielded increased clinic rates of social risk screening and referral-making, and what factors drove these outcomes. The study also explored: patterns of social risk screening in the OCHIN network overall and by patient race, ethnicity, language, and encounter type; how providers use social risk data at the point of care; and the relationship between social risks and diabetes care quality and outcomes.
