- All dentists miss their IAN blocks from time to time and for some it can be a major struggle! This video shows you an alternative way to deliver the block that may be more predictable for you!
* See 00:30sec for the ideal needle position during injection*
1. Use a 27 gauge short needle.
2. Palpate the coronoid notch.
3. Enter the tissue just above the thumbnail and advance to hit bone.
4. Deposit some local anesthetic to numb the buccal nerve.
5. Re-orient and advance the needle leaving just a few mm out from the tissues and your needle tip will be positioned directly above the lingula.
6. Alternatively, you can use a long needle and advance it to about 1/2 of it's depth if you're not comfortable inserting a short needle so deeply.

But won't the bone contact be very painful for the patient?
You’re right so one thing you can do is use topical and once you barely get through the mucosa, inject a small amount very slowly and wait a couple seconds before you advance. This often helps! All the best!
What’s the length of the needle being used here
This is a standard short needle. I believe they measure to be about 21mm. All the best!
But this distance of length of needle every time near to Target area? Because it may vary patients to patients
There can certainly be variation between patients, but palpate the anterior/posterior width of the ramus and you’ll see that the short needle will be sufficient for most any patient. The exception may be if there is excessive soft tissue and you don’t want to risk inserting the needle fully. In these cases or even routinely you can use a long needle and just adjust your depth of insertion using the same technique shown. All the best!
