Thank you for the video. I had some questions regarding TIL counting: 1. What would you do if there are several tumor islands and TILs are found in between them? Are they not counted? Or will they be counted as being within a larger invasive margin? 2. Does omitting intratumoral TILs not drastically affect the count? (Is there a biomedical reason why intratumoral TILs need not be counted, i.e., are they thought to be deactivated or immunosuppresive?)
Hi Matthew, many thanks for your questions. On your first question, indeed, f these are within the invasive margin you count the TILs. It is not necessary to limit yourself to the TILs that are near or touching the cancer cells. On your second question; the only reason why we ommitted intra-epithelial TILs is because on an HE these are poorly reproducible. I and many others are very convinced that these are indeed very important, and emerging evidence suggests also that this is the case. How are TILs reported in medullary or medullary like breast carcinoma with no or little stroma ?
