  • Dr. Martin Gorman is a highly trained dental practitioner in the treatment of temporomandibular joint dysfunction (TMD), sleep disorders and Mini Dental Implants. He has spent 30+ years learning and practicing the most advanced technology and research to bring relief to his patients. The heart of his practice is aimed at treating headaches and facial pain related to TMJ disorders, and to provide an effective alternative treatment option to the undesirable CPAP device for patients with sleep disorders. Additionally, Dr. Gorman provides a unique solution to patients who are missing teeth and struggle daily with their prosthetic devices. This denture stabilization system is in demand because there is no need for incisions or stitches. More importantly, within one visit the patient can be on their way with a secure denture, able to eat and speak with comfort.
    Dr. Gorman offers a comprehensive array of preventive, cosmetic and restorative therapies performed with compassion and an eye for detail. He treats the underlying causes, not just the symptoms, of dental problems to create beautiful, healthy smiles that last a lifetime.
    TMJ problems may cause head and neck pain, ringing in the ears (tinnitus) and jaw pain. Most headaches are caused by (1) tenderness of muscles and joints of the head and neck and (2) clenching or grinding of teeth (bruxism).
    Because of the multifaceted experience he has gained while working with experts in TMJ treatment, including physicians, ear nose and throat doctors, dentists, chiropractors, physical therapists and natural health practitioners, Dr. Gorman brings a unique, multidisciplinary approach to help patients find true relief from their TMJ symptoms. Dr. Gorman believes that equilibrium between all oral structures allows the creation of a natural, youthful looking smile and promotes the maintenance of a healthy chewing system that will provide a lifetime of optimal comfort, function and beauty.

      How do you determine what treatment is required. I’ve had 3 specialist tell me 3 different things. One said to get invisiline. Another said sleep apnea due to my tongue being swollen. Another said mouth appliance and cpap due to restricted air way.

    Teresa, sorry you're having so many issues. chronic headaches, clenching, facial tension and twitching are all symptoms/causes related to tmj. clenching can be caused by stress, or possibly a sleep issue. i think the first step is control the clenching with a small anterior appliance...like the NTI. this appliance separates the back teeth, making it much more difficult to activate muscles and clench. this will also change your bite, but in a more biologic, natural way. martin gorman,dds

      Hey Dr, I'm a Scottish student studying in Glasgow Dental Hospital and School! They recommended I use a splint but I did sustain a neck injury in my C1 region which has affected my hearing, TMJ and I believe my occlusion all of which contribute to dizziness and headaches! It sounds like alot of stuff but I think I can pin it down to the one neck injury.

    • @gormandental
      @gormandental  8 лет назад +2

      +Martin Laird With a C1 issue you should think about seeing a chiropractor, specifically an atlas-orthogonist. If you google Dr. Chris Chapman, you can review his chiro/C1 relationship to mandibular position in the fossae. With the appliance I use, its the continual adjustments as the jaw moves that eventually stabilizes the condyle and the bite. Remember as the mandible moves, all the teeth move with it. In regards to your accident...what I do is the same...move the mandible to its "stable condylar position" and in over 90% of cases you will get an excellent result.

    Dr. Gorman, the thumbs were placed at the edge of the entrance to the ear canals.... just in case you cannot visualize well the action that took place on my TMJ. The right condyle showed osteophites, fossae was normal (CT and MRI).My common sense tells me, now that the internet has helped me to learn more about the subject, that the clicking and crepitous sound I hear would have been more that enough to DX "TMJ problem" so anybody would keep away from touching or put pressure in an area already in trouble. Many thanks again.

    The plastic appliance in this video is an emergency appliance to separate the teeth so that the jaw muscles relax and the condyle seats or starts to seat in the anatomic socket. We use 3 different upper appliances depending on history, severity , exam results and longevity. The piece of plastic is adjusted until pain is resolved and the jaw is no longer moving; the jaw is in a stable condylar position. This can take a few days or many months. Once phase 1 is complete we then develop a treatment plan to have the teeth do exactly what the appliance has been doing. This can involve bite adjustments, ortho, bonding, veneers , crowns and even jaw surgery. Yes, sometimes , the cause is faulty dentistry. The issue is, if I just start adjusting somebody else's work it becomes my work and if the adjustment doesn't work I'm responsible. So by using and properly adjusting appliances I know what l am doing, and know when I adjust somebody else's dentistry my adjustments will work. Martin Gorman DDS. 818-995-1891

      Wish I was closer... I'm in North Carolina. The pain and discomfort have continued for years and I never knew what was going on with me. I thought it could have been related to Lupus or stressed. I was an ice cruncher and bubble gum chewer. The pain is unreal. After several dental appointments once a week for 6 weeks even getting second opinions, my new dentist and Endodontist discovered it was TMJ. I plan to make an appointment with the specialist (chiropractor or facial pain center)....

    Dr. Gorman last year my mom brought me a big home made sandwich from her friends house and I opened my mouth more than usual and felt a lot of pain and couldn't eat the sandwich till 10 minutes later. Next week in my AP chemistry class suddenly my ears clogged and then never uncolleged. Now i have reduced hearing from perfect hearing (could hear things from my room to other rooms). When I put my hands on both ears to stop any sound waves from entering. I noticed I have a constant ringing in the right ear. I strongly believe all of these stemmed from opening my mouth too much and dislocating my jaw. Yes, currently I have clicking and popping noises when I open or chew.

    simply said...YES. We have had many cases over the years with a variety of ear issues. Sadly, there has been way to predict success. Results have varied, but there is always some improvement with our treatment. Normally, the sooner you act, the better the result, most times. Good luck with your health issues. Thank you for your very much appreciated video Dr. Gorman. Quick question -- assuming appliance therapy is the suggested treatment, what are your thoughts in general on using botox injections into the jaw muscles in conjunction w/ appliance therapy? What about excluding appliance therapy and using botox injections alone (again, generally speaking)?

  • @gormandental
    @gormandental  11 лет назад +1

    I believe the CAUSE of TMJ is based on jaw location and how the teeth function. We make an upper appliance that will allow the lower jaw to seat in the base of the skull in its natural anatomic position. The appliance also is formed and adjusted so that your teeth function correctly. Medication only treats the symptoms not the cause. Finding & treating the cause usually will lead to long term results. Please contact our office at 818-995-1891 to learn more about our treatment. Hope this helps.

    This is BS -- if her previous dentist had properly aligned her teeth when he redid her silver fillings she would be out enjoying her life right now. I had a crown that was put in that never fit and now I have to wear my orthodontic retainer to prevent pain.

    • @gormandental
      @gormandental  9 лет назад +17

      citticat2 Very astute observation. One of the main causes of TMJ is the teeth not fitting together optimally. Since the teeth are connected to the jaw bone, and the jaw bones connected to the...(you know the ole song). If the teeth are not aligned properly, and in a harmonious position with one another, the jaw will not be in alignment either as it moves around, which of course can lead to wear and tear on the TMJ. As a Bioesthetic Dentist, I specialize in helping patients achieve the optimal biological alignment, allowing the best of function, and aesthetics. This approach (combined with some other non invasive techniques and practices, has led to incredibly high rates of success (long term) amongst my patients who were suffering with TMJ disorders. I would be happy to go over any of these treatment options over the phone, I can be reached at my office at 818.995.1891

    I have been dealing with facial twitching on both sides of face (no pain) when I smile,frown, for over two years now. I clench my teeth at night. I've tried several mouthguards including one purchased online where they sent me a kit and I sent back the mold and they made it for me. Each mouthguard seems to change my bite and my teeth hit weird after. My teeth are all straight but slant inwards. The tension in my face is really bad and I have chronic headaches. What do you think would help me?

  • @ThatOneScienceGuy

    Put putting a plastic piece ien her mouth doesn't fundamentally address the problem. If the problem is her bite, won't she need braces to fix hr bite? Or some kind of repair surgery to adjust the teeth so that the bite is fixed? She said she first had problems after having cavities filled, which means the dentist did something wrong, in which case shouldn't it be corrected rather than her having to hold a piece of plastic between her teeth all the time?

  • @gormandental
    @gormandental  8 лет назад +1

    Maria Magurno, you seem to be on the right track. I'm sure when you delivered the appliance it was in balance. But once patient starts to use 24/7 you get healing in joint, muscle relaxation. Therefore the mandible moves along with all the teeth. That's why the appliance must be continually adjusted until the jaw becomes stable. I recommend the appliance be built at the patients correct VDO and adjusted accordingly. Either add or remove acrylic to maintain the VDO you want. There are less invasive restorative procedures available for increasing VDO but this is not a classroom. Best of luck Martin Gorman DDS 818-995-1891

    Hi Dr Gorman. I would like your opinion on which is better between hard and soft mouth guards. I wore a soft mouth guard custom made by a dentist for 6 mouths during sleep and my TMD symptoms didn't improve at all. Recently I started wearing a hard mouth guard made of acrylic and I was told by a few other dentists that is much more effective at healing TMD symptoms than a soft mouth guard. Thanks.

    Dr. Gorman,Could you please picture the following and explain at the end what went wrong:1.- I had no pain and problem at all while eating, masticating, moving my tongue, yawning, etc2.- I was experiencing every so often sharp right ear ache for one 2 seconds.3.- Somebody behind my chair placed his thumbs at the edge of each of my ears and it started pressing and pressing forward, against the normal position of the TMJ area.4.- I arrived home one hour later and whenever I tried to eat something I just could not do it normally for the next almost 60 days due to the terrible pain, grinding and destruction sounds of what I later learnt was my TMJ area.The 3 D CT and MRI showed "anterior disc displacement without reduction and osteo arthritis.Was the excessive pressure placed against my TMJ by those thumbs a no-no action?Would you call that action as "part of any testing to check for TMJ disorders to give answer to the only present problem of what could have been a simple "rea ache" which, unhappily , went wrong?I will appreciate your input in the above.Many thanks for your attention to this. Today I cannot enjoy a bagel o baguette because of the chewing painful that start coming up and building up.

    • @gormandental
      @gormandental  8 лет назад

      +kil hefner Sorry your having a difficult time. From your history, thank you, the sharp right ear pain is the major fact. That means your disc is displaced anteriorly and the bone is pressing on the live tissue that has also moved forward. So, you had an issue and didn't realize you did. Over time, the bone will damage the live tissue and it will become scar tissue...the pain will resolve. But, that will not address WHY is it happening. If the WHY is it happening is not address the same can happen on the left side along with other TMD issues. Best of luck, Martin Gorman,DDS gormandental@gmail.com

    Gosh I wish I had seen this years ago and lived in America. I am in the UK and have complex pain and TMJ issues which has amounted to multiple surgeries inc five total jaw joint replacements. Now I have a chronically infected right joint which has degraded and needs removing and a spacer putting it. I had every Faith in the medical profession when I started this journey and now I am scared of it. They have now found problems with my immune system and I can't have my surgery until they are resolved. I wish they had simple fixes like this over here. Here they do rubbish splints and they failed for me. I still clench. I have massive spasm neck pain etc a bad back. I am fed up!!

    • @gormandental
      @gormandental  9 лет назад

      +Pobsbabies3UK Wow, sounds like you've really been through the ringer. I guess the UK isn't known for its teeth. It is surprising how often TMJ issues are misdiagnosed or mistreated as neck or back, or neurological issues. People underestimate the importance of a stable oral structure's role in balancing the alignment of the whole back and neck. As for Immune system issues, this is by no means "medical advice", but in my personal opinion, Colloidal Silver can be one of the most beneficial things to use to support your immune system. It can kill over 200 different bacteria, while leaving all the healthy probiotics alone. I've seen serious infections clear up at astonishing speeds with the use of colloidal silver. Just a tip.

    Curious about TMJ. I have a form of it. Ever since dental work last July, my body has had nothing but issues. Ear problems, tinnitus, balance, neck and back issues, etc. Issues I did not have before. I actually switched dentists during this year and the new one has been gret to me, and made me a nightguard for relief since I now clench my teeth, which I never did before my dental work (crow on tooth #31, rooth canal and crown on tooth #30). He also replaced my crowns an adjusted my bite to his best abilities. Things have been "better" but nowhere near to where I was before all of this. Any reccommendations?

      Dan L I get the feeling your dentist was onto the right path in looking to the bite as the possible source of your problems. One technology that I employ at my office in Enino CA, is called the TekScan. Its basically paper thin sensor that you bite onto, and it tells us where your teeth are meeting in your bite, and with how much pressure at each point. This allows us to determine problem areas with terrific accuracy and detail. I would be happy to invite you in for a free consultation in my office, I can be reached at (818) 995-1891

    Wish I had saw this sooner. l woke up one morning to something pulling in my inner check area on right side. Had no idea what it was, really weird. This was happening off and on for two days. After that I noticed on right side of my face looks swollen, not by much but I can see the difference when looking in the mirror. So far I have off and on tingling sensation on right side of my face, headaches, right side neck pain, right side ear pain and sometimes weird sensation on right side of throat. It has persisted and I now have sensation in my right arm as if I laid on it wrong or its sleep, sometimes travels down my right leg. Also, right eye twitches from time to time.
    I have a retainer but try not to sleep with it in my mouth because of grinding my teeth at night.

    Thank you Doctor for your help, just one more question, you told me ¨Some form of oral appliance to separate your teeth will usually help as a temporary treatment¨
    Does it help to fix my teeth and my jaw come back to normal place? or it can not help permanently. Thank you.

    How long does it take for that pain to go away once you are wearing the final appliance and how cost effectively can you get a client to that stage of having that appliance? Also, what about trapezius trigger points and whether or not they could be causing the temporal is and neck pain and some secondary jaw pain or whether it is the other way around with the jaw causing it all?

    Hey if have tmj we should not touch the teeths of back while chewing food? (Teeths which we chew food). We should chew softly?by doing this It will not make lower jaw weaknes and get longer? How to chew if we have tmj? Pls reply

    • @gormandental
      @gormandental  3 года назад

      Hi Nasreen, if you have TMJ, the first thing is to see a TMJ specialist near you or you can visit me just give the office a call at (818) 995-1891. Second, if you have TMJ, soft food to chew is ideal and back teeth can touch to chew food properly. The only reason the back teeth should not be touching is to prevent the teeth from grinding and/or clenching that can exacerbate your TMJ conditions. It is very important to take care of your TMJ to have a correct functional chewing jaw. I hope this helps. Stay healthy.

    Hi Dr Gorman, I have a very similar issue which also started after getting multiple cavities filled in by my old dentist and i'm also under the impression that my teeth have been filled down too short. I have been having almost identical symptoms, very tired jaws, sore facial muscles, headaches and the discomfort of an irregular bite, i have been having these symptoms for over 7 months (since i got the cavity fillings done) and my symptoms seem to be getting worse by time. I have gone back to my dentist who did not have any solution or treatment for my symptoms. I then saw an orthodontist who recommended to possibly see a Prosthodontist to see if he can possibly help. Is that the right person to see regarding these TMJ symptoms, or is there a more suitable dental specialist that i should see? I live in Sydney Australia btw.
    I am in urgent need of some treatment as this pain and discomfort is really affecting me physically and mentally.
    Please reply

      +Len Edra Hi there Len. Unfortunately, many dentists attend dental school, and then somehow believe that there is nothing new to learn beyond their area of speciality after they graduate. The key in my opinion is to not only develop a solid foundational understanding of the teeth, but also of the whole oral structure, and how it works in conjunction with the rest of the body. This is one core element of Bioesthetics, the study of the optimal natural biological form, and the practices that best allow me as a healthcare professional to best help my patients achieve this optimal model of oral health for themselves and their families. Improperly affixed or shaped crowns can absolutely affect your bite. If you would like to discuss your case in more detail, I would be happy to offer advise or suggestions over the phone at 818-995-1891, or at gormandental@gmail.com.
      Help is available, TMD is curable!

    Hello Doctor Gorman, I am a dentist myself. I have a Patient with diminished VDO. I explained she needed a full mouth reconstruction to augment VDO, but she can't afford it at the time. She was having pain in jaw and clicking, I gave her an occlusal appliance with anterior guidance and did proper adjustments for the splint to have contacts everywhere in centric. She presents 2 weeks later and states she doesn't have any more pain and her clicking went away, but now when she doesn't have the splint, she deviates to the right upon closing and occludes only on that side and no contacts towards left. Why can this be happening? any suggestions?, we're on Florida. Thanks.

I don't have gum recession and I'm trying to figure out why my teeth feel a little loose. Could be from grinding but I don't have headaches and don't have symptoms of grinding. My dentist told me it's normal for a little teeth movement which is crazyness in my book....they've never felt loose before. I'm 37, is this normal? My teeth have been aching for a couple weeks.

I don't think I clench but I have a clear invisalign retainer so wouldn't that keep my teeth separated? My dentist said they could make something for me but if I forget to wear my retainer one night the next day its so tight. Its a long story but I think I have TMD from all the dental work I have had in my life( I am 20). My mom and I tried to find someone in my area but they wanted to do a whole body muscle thing which was going to be $200 then see if they could even help me. Theres no one else near us.

    Hello Dr Gorman. I am going to be getting fitted for my nightguard appliance tomorrow for my tmjd. I am scheduled for botox injections on the 25th of this month. I am afraid and wondering if I should get botox. the dentist and oral surgeons said I am not a candidate for surgery at this point. but a side note is I have suffered from migraines for over 20 years, I had 2 car accidents back in the early 1990s with whiplash. I also have c2/3 ddd, bulging disk at c6 lower back disk probs, and narrowed foramen, and a few more spinal issues in which i regularly see chiro for several years (im a gulf war vet with ptsd as well so i clinch, grind, chew on inside of jaw and dont realize it) bruxism too. what are your thoughts on the botox injections?

    • @gormandental
      @gormandental  8 лет назад +1

      +Mrs. D Smart , to be honest, not really sure.. Botox injections work by killing the muscle for a period of time, so that muscle will not refer pain etc. But, if your nightguard is designed and adjusted correctly, all muscle pain should subside. Since you do have multiple issues, adjustments will probably be challenging. I would think 15-20 over 2-3 months would be necessary. In my experience, very few dentist/oral surgeons really understand the adjustment philosophy and that's why the success rate is low. Best of luck, Martin Gorman, DDS 818-995-1891

    I saw my dentist about jaw clicking, and ache/pain today. All he did was file down a couple of my molars. Is this regular practice? I've had these symptoms for 2-3 years and they have become worse in the last 3 months.

    • @gormandental
      @gormandental  9 лет назад

      ***** Hello, of course I cannot give a very complete diagnosis on a youtube thread, but I can say that one of the major causes of TMJ disorders is when a patients teeth are not fitting together properly. If done properly, and carefully, a good dentist will be able to help adjust your bite (the way your teeth fit together), by trimming/filing down tiny areas of your teeth until the "tips" of one tooth, fit nicely into the grooves of the other, allowing for the jaw to open and close harmoniously. One piece of technology I use at my office is called TekScan, it's basically a thin strip that a patient bites down onto, and it measures (in great detail), what parts of their teeth are touching together, and with how much force. This enables me to very accurately adjust a patients bite down to the optimal position. Think of your jaw as a door, and the doorframe as the teeth. If the frame is not the exact same shape as the door, the door will not close properly, eventually wearing out the hinges (your TMJ) from all the improper movement. I hope this makes sense. I would of course be happy to speak with you about your case at your earliest convenience at 818-995-1891.

    Hi Dr. Gorman! I'm going to the orthodontist tomorrow (Friday, November 13th) and I have TMJ. it was caused apparently by my very bad grinding my teeth while I sleep. The reason I am going tomorrow is to get a mouth guard while I sleep to wear to stop the grinding. My doctor suggested it and I feel like it's the right thing, is that a good thing to do? Will it stop pain?

    • @gormandental
      @gormandental  8 лет назад

      +The Abby And Jojo Show. Hi. hope your guard is working. Sometimes they do, but usually hit and miss. If you are grinding because of a sleep issue or stress it may not help. Also, if your posterior teeth contact the guard you will probably still grind, but maybe slightly less.. The real issue is WHY are you grinding. When that question is answered, the proper treatment to address the CAUSE of your symptoms can be implemented. Visit our website Gormandental@gmail.com. Best of luck, Martin Gorman, DDS

      +Mischely Gomez Hello, I would have to know a little more about your case to make a real diagnosis, but from what you describe, a bite adjustment could be a part of the solution. Often times the teeth not fitting together properly is indeed the cause of TMJ pain. When treating my patients I almost always try the least invasive treatment first before moving on to more intensive treatments. What other procedures or treatments are being suggested to you?
      Also, several members of my amazing staff here at the office are from the Philippines as well, and they would be happy to explain anything in tagolog if needed as well. Our office number is 818-995-1891, or of course we can email as well at gomandental@gmail.com.

    I suffer severe depression as being a former TMJ sufferer imagined I would undoubtedly die with TMJ.

    Hello Dr,
    I have severe gum pain (denist says my gums are fine), sore neck, and get tension headaches where the women in the video gets it.
    I have seeked many other remedies but have had no success with relieving pain.
    How long does a mouth splint take to work?

      @gormandental  9 лет назад

      +Leigh Spada Well first off, if your gums are in pain...they are not "fine" as your dentist has told you. Gums pain is not usually a symptom of TMJ, although the neck pain and headaches certainly can be. To learn more about your gum pain, I would recommend you either come into my office for a free consultation, or locate a periodontist (gum specialist) in your area. I can be reached in my office at 818-995-1891, or via gormandental@gmail.com.
      As for the mouth splint, it can take from a few wakes, to a few months depending on the severity and complexity of the case of course. All my patients have reported feeling some degree of immediate relief from the splint, and gradual improvement over the course of treatment.
      Call me any time and I would be happy to go over your case and help you find a solution!
      Dr. Gorman

    Why do you put the appliance on the upper teeth. I have an overbite and my dentist made my appliance for my bottom teeth. Also how do you know when you are ready for the restoration process and what can happen if you make the restorations and the the muscles are still not relaxed/ Will the new teeth all break ? Thanks

      CJ Smith Great questions, every case is unique. Often times I find that once the muscles have re-learned the correct bite position etc, the TMJ symptoms reduce dramatically, as does the body's tendency to resist any bite adjustments. This is one of the key purposes of the appliances. Once the muscle memory is trained in the correct natural way, bite adjustments hold much better, and the risk of chipping teeth during normal activity or at night is reduced dramatically. In many cases, the issue is actually an underdeveloped lower jaw, not an overbite in the traditional sense. Alternative therapies such as Myofunctional Therapy can gently (and non-surgically) expand these areas which (among other things) will open up your airway and increase your ability to breathe while you sleep. Often the chief cause of TMJ is actually a sleep disorder such as Obstructive Sleep Apnea, which is often caused by the tongue falling back into the airway during sleep. When you can't breathe well at night (one big sign is snoring), your body will often unconsciously grind your teeth at night in order to tighten the throat muscles and keep your airway open. But all this grinding (bruxism) causes tightness and soreness and often instability in the TM Joint, leading to TMJ Disorders that cause the kind of pain you've been suffering.

    Hi! I am experiencing TMJ for a couple of years now. Was able to see a doctor last week and undergone xray. Do you think braces can help?

      sorry to hear your having TMD issues. It's been my experience that it's better to find the natural, biologic jaw position first. By doing that, now you know where to move the teeth so you can maintain that ideal healthy position. My video with the Denar Board explains the philosophy. Best of health, Dr. Martin N. Gorman, DDS
