Most of the times, the people died as a consequence of them not caring about their surroundings. During the Shibuya trials, Sukuna was fighting someone else and just didn't care there were people being sliced up. Hiro seems to find some more satisfaction in individual killings, the others seem to be more into mass murder and property damage lol. Imo Sukuna would probably kill more people in the same neighborhood of those families Hiro killed, but he would probably do it by leveling the entire town. They all have similar levels of human indifference but they have varying degrees of satisfaction or motivation for killing. One does it for fun or boredom, for another it's probably like killing ants, and the other was probably more into eating the candy than the actual killing imo lol
