- Tripod Lite: a lightweight investigation tool to improve learning from incidents and near misses
Large organisations typically experience thousands of near misses and low level incidents each year. However, only the most serious are typically investigated in depth. Unfortunately, many investigation tools are either too complex, take too much time to use, or require expert investigators to be used for every incident or near miss. Organisations therefore struggle to learn from near misses. Weak signals are being missed.
What we need is a ‘lightweight’ investigation tool that can be used by anyone to quickly investigate incidents to the required depth. This is exactly what the Tripod Foundation and Energy Institute set out to create in 2021. Tripod Lite is a tool that can be used with minimal training by anyone who needs to investigate a simple incident or near miss - including supervisors, managers as well as trained investigators. The goal was to create a method that will allow anyone to investigate an incident in just a few hours, and uncover failed barriers, human factors, and organisational factors. The tool was developed and tested in 2022, and published free of charge in 2023.
During this webinar, the Tripod Lite development team (Stuart King, Tony Gower-Jones, Dr Robin Bryden, Dr Andrea Parker and Charlotte Mitchell) will explain the development of the tool, demonstrate how it works, and how it can be used to improve near miss reporting. This will be followed by a Q&A panel session.
