  • @edwinot
    @edwinot 10 месяцев назад +23

    Hello! I’m an MD/MPH. First of all, you guys are awesome. Second, here’s some unsolicited advice/points; Careful w iron, every supplementation/ medication has side effects. Iron is extremely constipating and will change stool consistency and color. Also, evidence based add on to increase iron absorption is vitamin C. You can get both over the counter and the iron that is more easily absorbed is ferrous sulfate. In men, very low levels need to be worked up and could be related to GI issues or Bone marrow abnormalities.
    Other point; altitude camps. It takes approximately 3 months for the nephrons in your kidneys to adapt to altitude, afterwards they become optimal and produce more Epo etc. What happens is after the adaptation, if you take that blood and save it and then later go back to sea level and inject it ,technically it’s your own blood, but it’s “optimized”. Alright, keep up the good work.

    • @HairyPantaloons
      @HairyPantaloons 10 месяцев назад +1

      Dylan warned about supplementing iron in his video as well. The conclusion was to get tested if you think it might be an issue and only supplement if advised to do so.

    • @edwinot
      @edwinot 10 месяцев назад +2

      Routine iron checks are not recommended, especially for men. Only time to get “tested” is if you are having symptoms, and you’ll know if you are. If you are iron deficient, you are most likely anemic as well and those are the symptoms we are looking for. Things like pale conjuctiva, hairloss, changes in skin color, etc.
      Low iron in men is not typical and needs to be worked up thoroughly. The real question is what is the cause of the low iron and not the low level itself.
      Hope it makes sense.

    • @carlosgaspar8447
      @carlosgaspar8447 10 месяцев назад

      @@edwinot a simple iron test (not ferritin) is done every time you donate blood which can be as frequent as every 2 months. not a big deal. nothing to do with hair loss. all you need to do is sweat like crazy over many months and you soon find trouble recovering or doing simple interval training.

    • @carlosgaspar8447
      @carlosgaspar8447 10 месяцев назад

      @@HairyPantaloons not a big deal to supplement unless you have a genetic disposition for iron retention. my doctor recommended 300mg of ferrous sulphate 3x a day. i asked if one a day would suffice and the doctor said, if you want the recovery to take 3x as long. within 2 weeks i could notice the change and was almost back to normal.

    • @edwinot
      @edwinot 10 месяцев назад

      @@carlosgaspar8447
      Free iron levels are clinically useless. My friend I look at labs daily for a living. To assess accurate iron levels you need at least:HGB(Hemoglobin),MCV,Ferritin,TIBC and RDW. Those are just a few. A truly useful iron panel is expensive, which is not what you get when you donate blood.
      Also what I said is that the most common cause of iron deficiency in men is microcytic anemia. If you have significantly low iron levels you’ll have symptoms and one of them is called“telogen effluvium” which does cause hair loss. Hope it helps.

    One of the easiest way to track your iron level is to donate blood regularly. I go 4 times a year and I get a free haemoglobin test every time. They don't let you donate if the haemoglobin levels are too low. Measurements of Ferritin can be tricky. If you have an infection or some inflammatory thing happening in your body (an elevated CRP - C Reactive Protein), the body can lock up iron in Ferritin so the iron level appears to be high, but the body cannot use it.

    • @tommenke836
      @tommenke836 10 месяцев назад

      That is how I found out about my low iron levels. However, giving away red blood cells hinders performance the same way as having low iron. It takes the body around three weeks to replenish red blood cells after donation.

    Agree about 24h Worlds
    Competition is always stiff. Pacing but also hydration and nutrition are key as well as experience... It's also race of eating. But when the Danish racer put and incredible pace in the beginning and was ahead of Phillip Kider , Stanko and Marko , it push all chasers to the limit. In my opinion the high pace blew up quite a few people. I feel like some could have done better with a different pace instead of trying to keep up. It's non drafting and it's long so you can't expect to feel always as good as the others for the entire 24h. But those with great pacing ( your own pace that you can sustain ) did better and even made up time after flat tires or other problems. Takes a while to figure Ultras. Fitness and fancy bike alone is never a guarantee for success. I know Tyler is even more motivated and everyone has lots of respect for him. Doing ultras is tough. Nice pod , cheers Boyz

    Chris regarding your ability to put more power out on the wider tires when riding at Heffron, i think it was Peak Torque or perhaps the Marginal Gains podcast folks who said that every little bump that the tire doesn't absorb and is sent up into your body, is power that your body consumes. So the combination of better rolling resistance on the larger tires [to a point] and the less fatigue from vibrations being sent up into the body, then you're able to put out more power..
    I've probably made a dogs breakfast of it and so i'll try and find the discussion original discussion.
    Great show this week!

    The pacing thing in a ultra event would be different to shorter race as your dealing with mental fatigue sleep deprivation and different sleep strategies. Some lien pushing harder and sleeping longer. It’s also about the mental pressure on opposition and trying to brake them early

    Please invite specialist if you talk about Roubaix (some things are not correct, on some things I have an answer to being Flemish and having ridden a lot of cobbles, etc). An other thing Jumbo and Dan were riding with a system to inflate and deflate their tires. Also they hit the cobbles, coming from tarmac at 37-40 m/h (55-60 km/h) I doubt you can do that on 40+ mm tires that have a low pressure etc

    This was all good to hear on the doping front because my hunch over the years after the whole EPO era is the testing has either really fallen off the radar or as some here said what they are testing for may or may not be what racers are currently experimenting with etc. But just from the reporting there really isn't much being written on positive tests these days and to think it's because of a whole bunch of negative tests seems naive at best. I'd have to guess like in the US with Pro and Master's events the testing just isn't great. The charging members bit is pretty bad too if indeed the testing is the same or even worse than during the EPO era etc. As opposed to them testing more. I really don't hear this conversation enough anymore.
    As an aside, the whole debate about doping controls reminds me of Jeff Evanshine during the US National Team selection back in the early 90's with Armstrong, Hincapie and Julich. Jeff was removed from that squad simply because he didn't show up to a doping control and after said he hadn't even been notified that he needed to be there for the test. That was when Carmichael was coaching these guys so there's that issue too. But it's crazy how old that story is and how it might still be happening these days too. Jeff was Junior World Road Champ back then and only second to do that since Lemond until recently. In other words, it's pretty crazy that his whole career got derailed by possibly missing that one single test.
