I had a TURP last week & I stayed over one night in the hospital. I had spinal anesthetic & I wasn't totally put out. There was absolutely no pain involved in the procedure. I've had cavities filled that were more painful. I get my catheter taken out today & hoping for the best. Thanks for the video.

This is the most detailed and comprehensive explanation on TURP that I’ve seen or read over the past two months. Dr. Lin, I can’t tell you how good you explained everything. I’m having my TURP surgery done August 21 four days from now. I’ve had a catheter in for 58 days. My Urologist said that my bladder was swollen three times the size of normal and if I didn’t have TURP surgery done that I probably have kidney failure eventually. I’m 76 years old. I’ve never been on any medication in my life. I feel great always have now this. Thank you again. and God bless you.
Oh, by the way, he put me on two medication‘s DUTASTERIDE and SILODOSIN. I was so tired. I felt terrible. I lost 20 pounds in 18 days. I took myself off medication. He told me he never heard of that before. Now I’m just waiting. The catheter originally was put in to give my, bladder rest.
    Coram Deo

I had the LIFT procedure. The procedure takes 20 to 40 minutes. No catheter, and no time recovery. Once you live the doctors office. Your way if life is normal, as when you were 30..
I had this proceduredone over a year ago. My two friends had had the same procedure 4 years ago and no problems.
But I know that sometimes you may need other procedures, turp, laser which takes about one hour.
Remember, every doctor know only one procedure, and they don't tell you that are better procedure around.
I know that once you get turp or laser done, and the time recovery has passed. You are good for life

Dr. Lin, I have a question regarding the type of anesthesia used for a TURP procedure. Do you prefer spinal or general anesthesia? Also one question regarding a spinal, how long after the procedure does a patient start having sensation back in their lower extremities, and how long before they are able to be moving around normally again? I realized this might vary from patient to patient, but could you just me some general idea on this. Thank you. Ray

    • @Sunriseurology1
      @Sunriseurology1  2 года назад +2

      Ray,
I prefer general anesthesia. Much better control of what happens to the patient and no less-predictable results with spinal.
Duration of spinal varies depending on dose and patient.

Thank you for this. I had my TURP done 7 weeks ago and wish I had seen this before. My recovery is still in progress. Unfortunately I didn’t have the kind of pre-procedure consultation this video provides. I was not prepared. All I got was “oh it’s great”, “nothing to worry about” , “quick procedure” etc. 4 to 6 weeks recovery they said. Well I’m here to tell you it’s been 7 weeks and it’s slow going. Flow is good immediately, but, for now, urgency is worse, frequency is no better (worse at night), spasms are uncomfortable after peeing, and dribbling after also bothersome. I’m trying to be patient and have a follow up mid July so we’ll see how it is by then. Your video gives me hope.
One thing I will say is the discussion around retrograde ejaculation is downplayed too much. Sure I’m 64 and the only babies I want are grandkids (had a vasectomy 30 years ago anyway). It’s dismissed as no big deal. I say that’s subjective. Unless you’ve experienced it you don’t know. It’s different and I don’t like it. Again, I’ll be patient and I’m sure I’ll get used to it, but I wasn’t prepared for it.
So great video. Guys, if you’re getting it done, just be prepared. In the long run I’m sure it will be worth it, but be prepared and patient.

I'm in 5th week post op, no bleeding but burning when I pee like the hell. And it's also very uncomfortable and painful when I'm standing or trying to sit down. I feel the pain in the prostate and it goes down the penis. Only when I lay down I don't have any pain. They tested me I have no infection. Can you give me some information please, did you have the same. They told me after 4 to 6 weeks I'll be fine. I don't think, it will take more time. And the retrograde Ejakulation is also normal, 100%. But I can deal with it. I really worry about the pain when I stay or walk at work.

    • @Trguy60
      @Trguy60 7 месяцев назад +2

@@Loewenherz6183 sorry to hear you’re having pain issues. I had none. It was relatively pain free, just some discomfort during urination for a while. There’s still spamming and “aftershocks” at the end of urination causing dribbling so I have to be careful and make sure it’s over so to speak. The urgency is also still there which is also bothersome. I think the pain is unusual so keep an eye on it. Good luck.

@@Trguy60 Thank you, it's very uncomfortable when I stay and walk. In the morning no pain, very nice, but after a few hours of work I feel very uncomfortable. In the afternoon I lose more urin than in the morning, at night nothing. It feels like sore muscles. I hope in a few weeks it will be better. Maybe I need a month more time.

Trguy60, that's exactly as you describe it!!
I made the Turis surgery, today is the 5day since catheter was removed. I still pee 3- 4cm while doctor had said for rivers of urine!!!😁
The weird is when the signal to pee comes, i pee only 2-3 fingers most, in the glass. So i can check everything!
I wear pad like women does cause i leaking!! Doc says it's normal.. To be patient! Ok..
Still take my medicines for 2 weeks.
No pain,no blood, but not miracles!
As for ejaculation there are some nerves in the area which get damaged by bipolar electrode and thats why the muscle who controls sperm movement, doesn't work well anymore!
*God is the only One real Artist who makes this complicated body, a miracle..
Be patient! You may need from 2 months to 16 months for complete normal action!!
Another very disturbing thing is the night "sleep" : I pee every half an hour!!! And when I try to walk from toilet to bed, pee starts again!!😁
      We're getting old,mate!

A very interesting and informative video. Two things you didn't address - the side effects of TURP on post SBRT radiation for prostate cancer and TURP vs Green Light Laser treatment. I'm an 67 year old, 18 months post SBRT radiation for favorable prostate cancer. Gleason 3+4=7. Have taken Flowmax for years and have had BPH symptoms for 20 years. Prostate size is 69cc before cancer treatments. Had urocuff and cystoscopy and have an blockage from the mid lobe tissue pushing up into the bladder. Do the risks doing the procedure outweigh the benefits, since my radiation oncologist says my prostate may have trouble healing due to the prostate being radiated?
    Again, great video!

For every 10 TURPS you do, and removal of the catheter, how many patients require additional follow up surgeries? If it's 1 out of every 10, then I'm in that 10% category. It has been 5 days of my second surgery, I had my catheter removed today. Keeping my fingers crossed for a great outcome this time. Hopefully, docs manage expectations.

I had MY bipolar TURP procedure two weeks ago, honestly it wasn't pleasant but life changing.
As far as the possible side effects, well, in my case, erection quality is decreased marginally, orgasms are so much more intense post surgery and, not to be crass, but I've got the opposite of retrograde - if anything it flies out now!
As far as symptoms go, how amazing not to have to go to the bathroom three or more times at night, with all this quality sleep I'm feeling great.
Pain, yes, it hurts. Immediately after the catheter is removed, it is sore. Not going to candy coat it, it's not fun for a week or so. The blood in the urine is a bit concerning, but comes and goes.
All in all, this has been a life changing in a good way.

Hello Dr. Lin. I am a 69y/o retired RN having spent 10 years at Mayo Clinic Hospital, with 4 of those years on the urology floor. I fully understand what it’s like to care for a TURP patient and it sounds like you have the procedure pretty much perfected. Over the past couple of years I have been experiencing increasingly more annoying LUTS. I have been watching your videos for quite a while now and am very impressed with your expertise. I know that all urologists are not created equal and I would be very comfortable being evaluated and treated by you. I have Cigna Preferred Medicare (HMO) which I’m pretty sure you are not a provider (out of network situation). Is it possible that this insurance would cover some or all of your exam and treatment?
Sorry if I’m long winded, but I feel it’s extremely important to find a competent urologist. Any advice would be greatly appreciated. Thank you.

hi! My name is Neil Ortiz, and I just had TURP Surgery on August 29th, so this is Day 7. My Foley Catheter was removed yesterday at 830 am, my medical team wanted me to return at 2pm for a VOIDING TRIAL. (Having the ability to Emptying out the bladder completely when urinating,) since I went to the bathroom several times and I brought it down to 247ML, I ended up passing but I was concerned that they were going to REINSERT the Foley Catheter and I wanted to avoid that Foley altogether. My question is the following: Is it normal to STILL have urinary retention after TURP Surgery? As far as urgency and frequent desire to go to the bathroom, I don't have that problem now like I did before.

Is turp a viable option for a man with cancer in the prostate which has not metastasis?

My history with BPH and TURP is rather unusual, and instructive.
My wife's a Neurologist and Internist of Filipino origin. I'm 77. In 2002 my first inkling of a possible urinary problem arose from a complete lack of impulse to urinate during a 20+ hour flight from Newark, NJ to Manila, Philippines during the Christmas/New Year season. On arrival My wife urged me to be examined by a Urologist in Manila, at Makati Medical Center. The fellow was Chief of Urology, a real pro. A DRI was unremarkable. A Sonogram demonstrated an enlarge prostate of approximately 42 grams, twice normal.
Since our visit was for two weeks only, he recommended my seeing a Urologist upon returning to the US. I HAD NO URINARY URGENCY, FREQUENCY. OR NOCTURIA AT ALL! My normal urinary pattern returned while in the Phillipines.
On returning Stateside I saw a female Urologist here in New Jersey. She confirmed the BPH, recommended a prostate biopsy
The biopsy, done in her office assisted by a female nurse, was performed the following week, in office, without anesthesia, which I declined. I found it uncomfortable, but entirely tolerable. The biopsy was completely negative on all 12 cores. I have a very high pain threshold, having endured 20+ surgeries in past years for congenital orthopedic anomalies.
The local Urologist prescribed Proscar and generic Tamsulosin 0.4 MG OD. That was the sum of treatment for the next 15 years. I did not actively see a Urologist again until 2017. During that fifteen year period I had increasing difficulty urinating, and nocturia.
In March 2017, after taking a prescription antihistamine which I had not taken previously, I ceased urinating. I thought it was a passing effect of that drug. It wasn't. I failed to urinate for two weeks thereafter, all along thinking it would resolve spontaneously. I was then transported to an ER, where a nurse placed a urinary catheter. My bladder had retained 3300 cc of urine during the two week interruption. I remained catheterization, which didn't bother me.
Within the following week I was examined by another Urologist, more senior than the first one. He maintained the catheterization, prescribed Proscar, then Avodart, and Tamsulosin. After several removals and replacements of the catheters, each of which resulted in complete cessation of urination, I was advised that a TURP was indicated to resolve the persistent failure to urinate without a catheter.
The TURP was performed in early July, 2017. It was planned for a "green laser" technique, but the surgeon had to revert to an electrocautery resectoscope because the resectable tissue was "too tough" to respond to the green laser. My surgeon preferred to hold me hospitalized overnight because of the change in technique, and the consequent additional blood loss.
I returned home the following afternoon, catheterized. I remained catheterized for approximately six weeks during epithelial healing of the urethra. The catheters didn't bother me at all.
After final removal of the catheters, I regained fairly normal urination. But, because of loss of bladder muscle wall contractile weakening, from fifteen years of straining, and the two weeks of total cessation of urine flow, the surgeon place me on a generic of uremine (bethanicol 40mg bid) to assist the bladder function.
I am presently on Tamsulosin 0.4mg OD, and the uremine generic.
I still have nocturia, for which I use urinal by my bed. The urinary volume oneach occasion is about 200cc, which I'm told is normal voiding volume.
Repeated ultrasonography shows a typical post void retained volume of 2-3cc, entirely normal.
Given the history as described. I've come through it quite satisfactorily. My surgeon has advised me it's quite unlikely I will ever require further repeat TURP surgery due to accumulating additional prostate tissue in response to DHT. Possible, but highly unlikely.
My wife is homebound, disabled, and in her early 80s. I have no problem with sexual function, as it concerns her--since she's far beyond any continuing interest in that regard. But elsewhere, and with others, I find the condition of retrograde ejaculation to be,

My recommendation after having gone through a 20 year saga with this insidious condition: get tested just as soon as you have a problem. Then evaluate several surgeons, and pick a seasoned pro. Trust your gut on who you trust. Then GET IT DONE PRONTO! Before, like me, you stop pissing for a two week stretch--and nearly blow out your bladders!

i had a TURP done for the second time as the first time the channel for the uretha was narrow. Initially it worked for several months and had the process again by a new younger Doctor who is specialist in this procedure.
My case history is severe chronic pain including chronic pelvic floor pain syndrone, Interstitial Cystitus, Prostititus which I have been dealing with for 30 years. Been in pain management for the same period with little relief.
This current TURP procedure was removal of large prosate stones with the expection this will eliminate my chronic pain due to severe prostititus and prostatestones.
Have you had patients with similar pain issues? Will this process cure my pain conditions? This is my last option to treat

Prostate measurements were approximately 95 grams (mri/us). What’s the difference between your procedure and the one he wants to do? Seems like the robotic one is more invasive and is like an inverted Turp. And like Turp it has a 95% RE like the other Turp procedures seem to suggest. An what’s your opinion on Aquablation?
