  • When, Why and How to Use a Walking Cane or Quad Cane www.AskDoctorJo... Knowing when and how to use a cane can help prevent injuries from falls. There can be many different reasons you should use a cane and they are outlined in this video. A cane is mainly for balance, and should not been used for complete support. More details about this video:
One big reason why people don’t want to use a cane is because they don’t want to look old. My response is always, well if you fall and break something, you will need a lot more than a cane, and then you will definitely look old.
So how, when, and why should you use a cane? Canes come in many different shapes and sizes. Today we will talk about a standard cane and a pronged/quad cane. A standard cane should really be used to help with balance and to help you stand more upright when you walk. A pronged or quad cane has more support at the bottom, and can help stabilize you more if you need more help when walking, but it still should not be used if you are putting more than about 20% of your body weight on it. If this is the case, you should look into getting a walker or crutches.
One of the big things I see is furniture walkers. These are people who use their furniture to hold onto when they are walking at home. They will grab onto the couch or counter top, or even the walls. Since they are at home, they know where everything is, and that makes them feel safe. However, when they get out in the community, they don’t know where things are to grab onto for balance, and then they risk falling and getting hurt.
The other issue can be your pet on the ground. If you don’t have a lot of balance, you might trip over your pet or they might walk in front of you and make you fall.
Sometimes people also drag their foot, which can cause someone to fall. You might also walk differently with a limp or favoring one side. Even if you can walk without falling like this, you risk causing muscle imbalances, and that can make other things hurt.
Once you pick out the cane that is right for you, make sure it is properly fit. The most important part to start with is to make sure your cane is the right height. Most canes have a pin you can push in to change the height. On your upper leg there is a bone that sticks out called your greater trochanter. It is just below your hip. This is where you want the top of the cane. You should have a slight bend in your elbow about 20 degrees. If the cane is too high, you might irritate your shoulder, and if it is too low, you might lean over too much.
Now for the walking part. The cane should be in the opposite hand of the injured side. The cane should always move with the injured side. If the injured side goes forward, the cane goes forward for support. Again, this is how our bodies naturally move; so don't think about it too much. Just move how you would normally move. The cane should be for balance and safety, and if you feel like you are pushing really hard on it, or if you can't walk smoothly, then you probably are not ready for a cane yet.

Thank you so much for this video, the information is invaluable. I was given a cane for severe back issues a few years back, my cane was just delivered to my door, so, as a result, i was never told how to set it to the correct height or indeed use it correctly, which has unfortunately caused new issues in itself and i actually found it more painful to get around in some scenarios with the cane, than without. So, again, thank you for not only showing me how to use my cane correctly, but have it at the optimal height. A great video!

@@AskDoctorJo I'm looking for confirmation that I'm using two canes (simultaneously) correctly. I have been using a single cane for years, and I have a rollator, but it's very bulky and cumbersome. I have come to need support on both knees and I find using two canes both easier and less unattractive (than being a 40-something on a walker lol.) I've been using both the same way I use a single cane, but when going upstairs or uphill, I find planting both canes and "walking to" gives me more support. Is this fine?
Also, BIG THANKS for introducing me to the small-footprint quad tip!! I will be ordering some as soon as I'm done with this comment!!
P.S. I invented a "fashion cane" cover (I can't disclose publicly how it works because I don't have a patent... too poor!) so one can change the look of their cane everyday, but I have no idea whom to contact to get the ball rolling on said invention. Do you have any ideas who might be interested in producing such a thing?

    • @AskDoctorJo
      @AskDoctorJo  4 года назад +1

You're welcome! I hope the quad tip helps. Yes, the way you mentioned should be fine. It's really about how stable it feels for you, so if it feels safer/stable going up and down stairs/hills that way, it should be fine. Unfortunately, I don't know who to contact about that.

I've been living with this disease for 16 years and I find that a cane really helps with the pain while walking and that I can walk safer. Some days I have to use my walker, which was recommended by my physical therapist. Before I got a cane, people thought I was drunk and falling was a problem. There is no shame in using a cane.

Rarely does an individual speaking into a camera actually project "connected cognitive presence" as if they were directly conversing with the viewer. I have found DrJo at a time in life where my future mobility is in question and I am in great pain walking these days. If you don't use your leg muscles you begin to lose your leg muscles. I must have developed "terminal curmudgeonitis" so that I dismiss "experts" too frequently and too quickly, yet DoctorJo has such warm, friendly presence in her presentations that I have become a "patient(a) patient(n)." Her personality and persona should be used to educate physicians on bedside manner. She confirms my philosophy, Eve was not an afterthought, Eve was an improvement!

Thus I am up for surgery, L4/5 prolapse and L drop foot, I fall often...painfully...so I am off to get some titaniun and dead man bones in my back lol! I am a funiture walker both at home and at my studio... with stragitically placed chairs for myself...I have avoided a cane, but bought one last night and actually went to a green house today...first time in years! Same reason I'm here! Have been using a walker for two months but I feel I have so much more mobility, less pain (lower back). I do however, feel that because I've been home mostly, I've done the "furniture walk" She mentioned 😆. The few times I've gone out with the use of the walker, most recently, it's become somewhat of a limiting tool that I feel I have to carry to feel safe, which I do, and it does but then it's hard to reach out and open a door or go up/down the 4 steps outside my apt. So in the hopes of trying to get back to normal, I'm looking into buying a cane 🤔

I say go for it and get one anything that helps can only be a good thing, even if it only lessens the pain a little it will be fantastic. I hope you find something that works for you. 29 just broke down and got a cane last night. Took a steering column to the side of my left knee almost 9 years ago and have been limping off and on ever since. This time it has been much worse than usual enough I went to the doctor saw an orthopedic and am scheduled for MRIs on both knees. I'm hoping my bad knee between a brace and the cane will have less of a chance of giving out on me.

Used a single point cane after hip replacement. It worked but I was always having to figure when/how to park it. Two years ago, after a shattered femur and transitioning from wheelchair and walker, I used a quad cane with wider feet than the one in this video. They recommended that style when I was in in-Patient Rehab. It was so much easier to walk with. Granted, I was using it differently than I did with hip replacement, but it was still easier to use and I felt much safer. Plus, as I was getting closer to stop using it, I could let it go while standing at the sink and it ‘parked’ itself.

I have a lot of pain in my knees and sometimes my ankles (and other joints, but these are the relevant ones), and I can't handle standing on the joints that hurt when they hurt a lot. I often limp and such, should I talk to my doctor about getting some sort of mobility aid? I'm worried because i'm only 14 and would look weird with a cane, but I really can't walk normally most of the time anymore.

you talked about balance a lot, and my balance is perfect. I'm an occasional skateboarder and ice skater, and do yoga with my mom. I'm not sure if that means I should be asking for something other than a cane, since it's only the pain bothering me.

    • @AskDoctorJo

I would talk with the doctor about this in general. If you have pain in multiple joints, it could be a whole body issue that the doctors need to figure out with testing. Definitely go talk with your doctor. Good luck.

I used a cane for standing, lower back issues. Then I discovered using it to walk took the pressure off my arthritic toe. I use it on the same side as the toe because it's the bent toe push I need the help with. I've tried the opposite side and it's not as much help. GP was OK with it, haven't talked much arthritis or PT with Rheumy yet.

I really like my that cane cane with both types of feet. I can easily switch it out for what I need and the cane itself folds (4 prices). It was hard to find a high quality cane like that but worth it the time and money put into it. I have a cane for 2 reasons:
    -Permanent damage from old injury
-Severe pain from standing for long periods of time, making walking painful and difficult.
I don’t need it 24/7 but I do have to carry it in my bag with me so it’s there when I need it.
The quad foot is a little taller than the regular one so I do have adjust the height when I switch it out

Hi Dr. Jo, Thank you for your quick response. I am seeing a PT now. I just got clearance on Monday from my surgeon to start bearing weight. So when I went to the PT on Wednesday they started working on it. They said there is also some kind of bar that can be attached to my walker and extends upward so I can hold on to the bar instead if pushing my full weight into the side hand grasps. Thank you for your encouragement. I am really hoping to feel better soon.

I've recently had surgery on my left foot. Haglund's Resection w/reattachment of Achilles tendon. I'm post-op only 10 weeks, and am still in an air cast, but have been advised by my dr. to start walking some without the aircast on. I have been trying to use my mom's cane. I keep it on my right side, and just like you said, it moves with my left food, the injured foot. However, I still feel like I'm putting as much pressure/weight on my post-op foot regardless of the cane. Am I doing something wrong? Does the cane not try to take some of the weight off of my injured foot? It feels like if I had it on my left side, that when I walked, I can put most of the weight on the cane, rather than my foot. This feels better. But using your recommendations, I am doing it wrong. So I'm still trying, keeping the cane on my right side, for better outcomes on my left post-op foot. Thanks, Dr. Jo!

Unfortunately, I am not sure what would help without getting to see you. That's why it would be best to go see a PT in person. It might be an easy fix, but I really would just be guessing without getting to evaluate you. Do you have access to a pool? It is a great place to get in and exercise both your leg and arm without the impact, not swimming, but actually doing exercises in the pool.

Thanks for the video! I'm finally getting to address my hip dysplasia, and just finished some PT. I'm waiting to see an orthopedic surgeon and get imaging of the hip joints, as I still have pain especially with extended periods of walking and standing. I'm thinking of getting a cane for times I go out and know I'll be walking a lot, for the extra support when I start to hurt. I'm going to talk to the orthopedic surgeon about it, but do you think using a cane before I get sore would help me keep going longer? And with bilateral hip dysplasia, which side would I keep the cane on? I assume opposite of the worse hip/leg.

    • @AskDoctorJo
      @AskDoctorJo  7 лет назад +1

You're welcome! Yes, I think it will help keep you going longer. You are right, if you have a side that's worse, use on the opposite side. Otherwise, I would say use it in your non-dominant hand, so you have your dominant hand free to use for other things. Good luck.

@AskDoctorJo ~ do you have any videos on using a cane for Spondylolisthesis? I don't need support often, but the pain gets so bad that my legs give out & my balance is off. The leg pain that occurs is on the right side. Should I support from the left? Since there isn't actually a "left leg / right leg" issue. It's just pain on one side or the other. I haven't found any info on this type of question. Using a cane DOES take some of the pressure off my back. Thanks for any help you can give.
    I LOVE YOUR VIDEOS!

    • @AskDoctorJo

Thanks! Yes, it would be the same as in the video, so if the leg pain is on the right side, you would want to hold the cane in your left hand. If you don't have one side that hurts more than the other, I usually tell people to hold it in your non-dominant hand, so you have your dominant hand free to do things. I hope it helps!! Good luck.

I can’t think you enough. I’m 17 and trying to figure out what mobility aid will be best for me (EDS). I don’t necessarily have a weaker side, so I don’t understand what I would personally do about what hand. For this reason I am also looking into forearm crutches (smart crutches) due to their angle adjustability. I get worried about a cane causing my wrist to dislocate even more from the pressure.

Hi, I have seen other physiotherapists, and they have adviced me to use the cane on the same side of the weak leg. Can I know what the rational is?

Sometimes there are exceptions, but not many. I don't know why they would tell you that. You would have to ask them. Unless you have an injury on the opposite arm, or it is painful to do the way shown in the video, there really isn't another reason. Did you have an injury?

Hello Dr Jo! I’ve been doing so much research to find a perfect cane. Let’s go back 3 years ago I got into a very bad motorcycle accident that put me in a induced coma for a few days. Ever since then my walking and balance has been terribly bad. So I was curious what cane should I look into? I’ve tried the 4 point long legged canes and I did terrible at PT. Then I used a one point cane and my walking was great! But I still have issues with my balance. So is there any cane that’s good to use for balance?

I am recovering from a Torn ACL. Do you feel that I would benefit from walking with a cane? I'm currently "one-crutching it" while out in the community, and pretty much walking on my own around the house. The crutch is starting to annoy me! :)

+hauntedtank75 If you are using the crutch just for balance, I think you can use a cane, but if you are still on weight-bearing precautions, I would stick with the crutch. Good luck!

Thanks! I can get around without the crutch, but look a little funny doing it. Think I'll try a cane. Thanks!

Great video. I haven't been able to find how to use a cane when both knees are bad.

Hello, I just had quad tendon repair of a knee, which will require straight leg brace for 2 months. Weight-bearing will be OK in later weeks; my issue is (unrelated) Achilles tendinosis (runner) that's really aggravated with any weight-bearing. As an MRI tech who runs all over the place, crutches when I return to work won't cut it. I'm considering a (quad?) cane and heel lift to take the weight off. Thoughts on whether this will suffice? Thx

I have POTS and OH (autonomic disorder) and just got a quad cane because I have issues with feeling dizzy especially when standing in line somewhere. I don’t limp, so walking around with a cane in my hand and not limping makes me look weird. If I’m in a store walking around and then stop, then I just use it as support. I feel like an idiot with a cane and not limping. How do I get past this fear of being judged? How do I even walk with it when I don’t need it when I’m feeling fine but then all of a sudden need it because I’m upwards for too long? Do I just hold the cane over my shoulder and wait til I feel weak? I know this is a weird question, but I’m 39 and not quite ready to be seen with a cane but know I need it from time to time....especially if I’m anywhere where there’s no seating. I’m just embarrassed I guess.

I can imagine that's hard, but if you need it, you need it. I often see patients "dress up" their canes with different colors or styles to make them more fun. When I had to use crutches for my surgeries, I found duct tape that had the Gamecocks logo on it, and wrapped them up (I even put Christmas lights on them when I had to use them in December). When people see a design or a pretty color, they might be more admiring it than judging you. I had so many compliments on my crutches.
