I think the defender should have a hand on the terrain, since he is the one choosing where to fight. He should be able to put some hills and woods wherever he likes, at least.
That is accurate, which is why its so hard to create a good terrain setup system. These days i allow one player to setup terrain and the other to pick the edge, but as the attacker/defender has not been determined yet it does not deal with the issue you raise. I still have not found a good and practical system yet. Perhaps one day i will.
