Had you considered Rod of Ages instead of heart steel? Same slow stacking type of item but it affords you healing through the passive by spending mana constantly, plus AP I'm not even close to your elo but ROA, boots, unending despair/ spirit visage, into riftmaker has felt really great for me. 3 healing sources including conqueror and you are still ending at 6 items with like 350ap
It's always fun to see builds that I used in the past start working out better in the future. I played drain tank swain during 2023 2nd half and had 16w/6l and had fun doing that.

I play swain grasp, heartsteel , rylais, despair, spirt v and jack show. This build is the best for tank swain or rift maker if they are more melee and they will not try to run.

You must've not played/watched league for quite a long time! TP now has to versions/phases - leashed TP: where you can only TP to structures - Unleashed TP: Where you can TP to allies and structures That was done a while back when bot lane dives were the meta and some teams even took 3 TPs
