Should I keep the fire burning while I pour the coconut milk in the caramelized sugar?

Hi, yes.. you can keep the heat on whilst stirring in the coconut milk. But once it comes back to a boil, it's then time to take it off the heat to let it cool down.

Is this on low heat?

It wasn't really low, just high enough to have the water boiling but not too vigorously.
