This is sad, I don't know why fancy goldfish hobbyist in the US always have substrate like sand and gravel for goldfish and they said it's natural for the fish. These fancy are not natural, they don't need sand or gravel. It's only make it worse for they as sometimes they can't sieve through all of sand/gravel and will have swim blader, bloated, and dropsy. If you keep fancy goldfish, keep it bare bottom and upgrade to large cannisters/sump but reduce flow

Well... Hobbyists in Japan also keep fancy goldfish with substrate to and sometimes outside too!

If you think it was disease, wouldn't it be prudent to get it out of the tank promptly instead of allowing harmful microorganisms to spread to the other fish?

I did remove it. The tank is medicated anyway.
