I'll have to give this a try. I'll only do leg extension at the end of my leg workout, with light weight, 12-15 reps, 30 second rests. The intention is to fatigue the quads as much as possible, I'm not trying to reach concentric failure with heavy weight, but instead push my quads to the point in which I can no longer tolerate the pain and have to stop.
I believe doing them with heavy weight is where you start putting too much stress on your knees, it's not an exercise meant for heavy lifting.

Thank you so much Ron! I have bone-on-bone osteoarthritis and am on track for two knee replacements. Part genetics, part very active life and part being fat (working on it). I so appreciate your video. My pain doc wanted me to do these youtube video where you sit in a chair. Uh no! I bought a great bench with a preacher curl/leg attachment so I can do leg extensions. and leg curls. I have a history of weight training an am very comfortable with it but was worried about leg extensions with my knees. I first have to practice extending my knees, which I can't now. I am only using the weight of the machine, adding no extra weight. Your method of moving the hip flexor forward is great! I'm not at full extension yet, but I don't feel it in my knees as much. Keep it going!

I have found that the old fashioned leg extension machine that I used in college is best for doing leg extensions. They always had additional support under the knee joint like a roller pad. Never had any problem with knee extensions until I started using the newer style extension machines.

My pain stems from the bottom of my patellar on my right knee but only after 20 mins post workout and can last for a few days. When I bend my right knee over my toes I get this aching pain on the lower half of my kneecap.
I always thought it was my form from Bulgarian Split Squats as I've been doing them a good 2 months and only started feeling this pain over the past few weeks but I've since corrected my form and now it makes sense - the origin of the pain and this particular exercise.
I use an incline bench and lift a single dumbbell with both legs.

Thank you. I have just started again on machine exercises (moved countries, WuFlu and had to wait for my gym to be set up here) about 6 or 8 weeks ago. My outer right knee has been twinging and getting worse in that time but since I have no trouble inside the knee and walking/gym work is absolutely fine I didn't connect the two. I'm going to stop using that machine for a while and start again doing it properly when the twinging has stopped. I've always had a weakness in that knee as I was hit by a slow moving car there as a child, but no problem has ever been found. In a way, I hope it was the machine causing the pain, as otherwise it will entail doctors. Much appreciated.
