We were unable to remove or break off the lockwasher, even with pliers, hammer, etc. I called the place that had recently sold me new tires for my car, and they said Come On Over. I brought them my hand truck and they had to use a sledgehammer(!) to break off the old lockwasher (and had to mangle the old tire in the process, but that was okay). This "how-to" video should at least show people which way to put the lockwasher on, since it's not symmetrical... luckily I looked carefully at the old wheels' assembly before the destruction phase. :)

fairly useless - doesn't have close-ups of how he did what he did, of when to use a spacer, of what the ideal fit on the axle is, on which side of the wheel faces inward, on how to use the axle lock washer - explains nothing that would confuse and send one to look for a tutorial in the first place.
