Is it possible to apply without LOR.? What if you have a USCE from 2016, but haven’t asked for a letter yet?
The doctor had promised a good letter then for my performance though
You need to apply with at least 3 LoRs to have a complete application. You can still request LoRs from experiences in the past, but having them from 2016 is too old, generally speaking. LoRs should ideally be within 1-2 years.
