    I got up to 174 lbs on Abilify (antipsychotic) and then went down to 134 lbs on Vyvanse (amphetamine). I’m 5’8” and settle at 157lbs or so unmedicated. I own three sets of clothes.

      Tay! I just saw you in the comment section on some guitar channel recently lol I gained 70 lbs in 3 months while on doxepin, bupropion, gabapentin, quetiapine, and eventually buspirone and I think escitalopram. I was severely medicated. The weight dropped as fast as the meds though, I'm back down to my normal 180ish. Those drugs made me feel extremely hungry all the time. I didn't taper off them slowly and the withdrawal was HELL. I definitely don't recommended quitting cold-turkey.

      I went from 140 to 220 at one point... some of the same stuff. I now struggle and go in between 180 and 200. 5'9" cant get under 180. And its been 14 years

    Quetiapine nearly destroyed my life! The weight gain made me more depressed. I told my consultant I stopped it myself out of fear my eating disorder would resurface. Best decision I made!

      Yeah quetiapine isn't for everyone. I had a few psychotic BPD episides and ive got chronic insomnia. I'm don't mind my extra weight i feel protected. Unfortunately it has begin to impact my joint s at age 42 so I'm like to keep as mobile as possible. Not quite ready for a cane or a walker yet..

      You listened to the wisdom of your intuition and body. You are after all the expert on your brain and body. You live in it 24\7. Although I always advice people to consult their prescriber before stopping a medication, however there are situations when that is precisely the right thing to do. Especially if you're experiencing adverse effects and toxicity.

      @@marleyhill34 That's a tough situation. I've noticed some people struggle with memory and cognition with treatment with Quetiapine as well. Medications can be helpful in acute crisis and stabilization phase, but no one knows for sure the full long term impact. Have you explored any holistic lifestyle interventions to help you with weight loss and your joints? What's helping you cope now?

    My aunt gained a lot of weight on antipsychotics and she craved high carb foods like candy. As a med graduate, it broke my heart as there wasn't anything I could advise her. This possible solution is relatively risk free and easy to implement. It gives some hope that her life may improve🙏💖

      Me too, gained 70 lbs in 3 months... I went from being a healthy weight and in pretty good shape to being clinically obese and not fitting any of my clothes. My docs couldn't fathom why extreme rapid weight gain is a bad thing, zero concern whatsoever... I craved carbs and sugar CONSTANTLY. I was ALWAYS hungry, could never satisfy my hunger. Now my teeth are fucked too from the sugar and constant dry-mouth and I have extra skin from rapid weight loss after I got off of them.

    Makes so much sense. Since I’ve started medication I tend to be constipated. Never used to eat yogurt, now I feel like it’s the only way I get relief

      Interesting I've always ate a wide range of fruits, vebegtable and nuts. I think you are suppose yo eat about 500g a day of them but most people eat 500g of carbs and red meat instead. I've always eaten yogurt from childhood..

    I can honestly say this is the greatest piece of advice regarding weight gain on antipsychotics. Been taking probiotics for almost 3 weeks now. I weight myself every Sunday morning. After two weeks I was already down 1,3 kg (around 3 lbs). A big thank you to Dr. Marks
    EDIT: So I stopped taking them because on their own they couldn't do that much. But now 2 months ago I started again but now I also track my calories. I lost 12 kgs (around 26 lbs) since then. If you have to take these types of medications or know someone who does definitely give it a try. You won't regret it. Literally life changing.

      Doctors often don't have enough time in the day to delve in to looking at deeper underlying issues. They're overworked and witness so much human suffering, at the end of the day it is easier to see someone for 7 minutes and hand them a script. What do you suspect is going on with you? You are the foremost authority in your own brain and body. What were you hoping you could get support with?

      @@NeseretBemient I was put on a sleeping pill qnd gained 50 lbs. Now I'm pre diabetic, but I've been vegan for 8 years. Qll of a sudden I have to change everything I eat. Along with being anemic because of menstrual cycle, I don't know what to do anymore. I started eating fish and eggs and only bread once a day. Put on Vyvanse, haven't lost a pound. Replaced breakfast with protein shakes. I don't understand

      ​@@LynshereeEastman what kind of protein whakes? whey protein specifically spikes insulin very much, so it's really only advised around workouts. try to get some other kind of protein if you are using whey protein.

    I’ve never been told by a doctor the down side of antidepressants. I’m on Paxil and have just stopped taking mirtazipine 8 weeks ago. Whilst on these meds I put on 50 kilos, in 8 weeks of not using mirtazipine I’ve lost 12 kilos without exercise. Which I plan to do, now I’m working on getting off the Paxil slowly. These meds have made my life hell ! ❤to all still suffering

      I gained some weight on Seroquel but I was way too skinny on Prozac. I had to be taken off Prozac because they thought I was anorexic. I gained weight as soon as I was taken off it.

    I'm injected Invega and I take Seroquel among other meds. When Seroquel kicks in the food cravings can be wild. I am sometimes able to do 16/8 or 18/6 intermittent fasting, but cannot do it for many days in a row. Training willpower is key.

      Intermittent fasting is one aspect of a holistic lifestyle intervention but there are many others you can add that will optimize brain and body functioning. When was the last time your treatment with Seroquel was reviewed/re-evaluated? Do you need to be on the same dose? As well, have you explored, similar to intermittent fasting other lifestyle interventions?

      Psychiatric medications do not work for everyone - some people can't tolerate them, other have horrific side effects (like feeling numb or suicidal, or a subdued version of them) and others prefer not to take medications, and still other self medicate, and some people can't access them. So there is a wide range of situations. Have you explored any other therapies or holistic lifestyle interventions?

    Thank you Dr. Marks for all your amazing videos. You discuss topics that psychiatrists and psychologists don’t discuss normally with patients. My guess is they don’t have the time per appointment and the patients don’t know what questions to ask. Your videos are so enlightening and informative. You say so much in just a short video which is a testament to your vast knowledge. I have a lot of respect for you and I think you are truly passionate about your profession. Such a rarity these days.

    This is extremely helpful, thank you! I've been taking Seroquel for many years, and I feel like a constant battle is going on with my binge eating disorder and the medicine exacerbating it. I just don't have the willpower to fight the addiction while the meds are making it worse. What I'm doing now, and it does work, is intermittent fasting, but what you mentioned about probiotics is something I will definitely try! I just happened to start taking probiotics for a different reason, but I'm hoping this will help.

      They call it the "seroquel sweets" for a reason, you WILL crave sugar and carbs and often right before you sleep which is even worse. Eating healthy and a daily excersize routine definitely helps, but it won't stop the constant craving caused by the drugs. These things affect your insulin production, metabolism, your gut, some can cause heart and other organ damage, and some can make a person very impulsive, at least partially from screwing up your brain's reward system. I was able to lose some fat by eating less junk and doing cardio and lifting weights, but i didn't lose all the fat until I taperd off my meds.

      Sometimes medications do make things worse and that is a really tough place to be. Taking care of your gut is one of the best things you can do for your overall health as well as your mental health. They say more serotonin is produced in the gut than the brain. They call the gut "the second brain." Part of a holistic approach in healing mental disorders is to approach it from many perspective. Food is a major aspect of that and it has one of the most powerful impacts on mental and physical health. I encourage you to continue to explore. Has your treatment with Seroquel have recently been reviewed/reevaluated?

      @@mr.giggles4995 Excellent observations and kudos on taking charge of your physical and mental health!!! That is awesome! There's a huge correlation between metabolism, gut microbiome, and insulin resistance and mental health. Diet is one important area that can have powerful impact. Glad you see the connections and are implementing lifestyle interventions. I also have been able to do the same for myself and put a 13 year antidepressant induced bipolar disorder in to remission. Thank you for sharing.

      I was on seroquel for many years, and I finally got off of it about a year ago. It made me so sick and I gained so much weight. Make sure you're getting your labs done often with Seroquel, so you don't end up like me.

    Thanks for all the information!! It has helped a ton to get an idea on what to talk to my doctor about.
    Used to have a problem with weight loss as a teen and young adult. Then it swapped as soon as I moved and changed up my lifestyle to be more vegan (then) and heightened exercise. Then lost my gallbladder 3 years back after several years of episodes and a huge upswing on gut bacteria like c.diff (oh goodness not fun with Clyndamycin). That was when I was told I had a problem with gluten. So it was cut out and then the gallbladder went. Wasn't able to gain weight or muscle mass. Huge mood swings and overall, got called many things that I wasn't during the time of health crisis. Went from 220 lbs to 128 in less than 6 months. Continued on a path of inability to eat food. A friend from Austria told me about nutritional biochemistry. Been obsessed with it since. Cut out liquor, most processed sugars, no more red meat or gluten either. Gaining weight and moods stabilizing. Listened to my body and learned alongside the care team that an increase in vitamins b, vitamin d, calcium, magnesium, and fats (like natural pig lard or salmon) does make a massive difference on clarity and even motivation. Food does a lot for the human body and mind.

    With Seroquel I don't feel hungry a lot between meals but once I start eating I don't want to stop.
    For me menopause caused more weight gain than the medications but everyone is different.

      Increased appetite is one of the many effects of psychiatric medications. How long have you been on the Seroquel? Also have you explored additional lifestyle interventions to help you with your nutrition and for hormonal balance?

      @@NeseretBemient I am pretty up to date on nutrition. I have POTS which causes balance issues. This has made exercise difficult.
      Edit: I have been on a low dose of Seroquel continuously since 2009. I had been on it before then as well.

    Zyprexa's impact on my appetite felt so bizarre. The compulsion to eat and eat and eat, and not feel full at all was so disorienting.
    And I'm someone who's also had binge eating disorder (at another time in my life). But with binge eating, I'd be uncomfortably full yet unable to stop. On Zyprexa the amount of food I'd have to consume before feeling full was insane.
    Honestly the weight gain itself didn't bother me, but the way my appetite felt was so disruptive. At any given moment I felt hungry, even if I'd eaten more than a full meal an hour earlier.

I hear a lot of people say that they gain weight on APs like Quetiapine, but some of them say the weight comes off when they stop taking them, others say the weight stays and it's very hard to get rid of.. the latter might be my case currently, what should be done in that case?

    This is helpful. I gained 15kg due to migraine prophylaxis drugs. Some of them were anti depressants but I also had beta blockers and anti epileptic drugs. The worst one was a drug called Flunarazine . Slowly loosing with Mediterranean diet and restriction of simple carbohydrates and also limiting fruit to 2 pieces a day.

    literally just scheduled an appt to talk to a PCP about going on Ozempic, encouraged by my psychiatrist, because the only meds that work are mirtazipine and trazodone but i refuse to take them because of the absolutely ravenous and miserable appetite they cause. i gained 30 pounds and it ruined my life. you like can’t understand it unless you experience it

      Have you heard the affects of overmedication? Psychiatrist prescribe and diagnose. I cam off the quetiapine. It's been 90 without it.

      @@brunettemoondoll4JAW Ok. I have Borderline/Emotionally Unstable personality disorder with autistic traits and complex childhood trauma. I've been dealing with my mental health since I was 8 years old. I'm 42 now. I've only ever taken one anti-depressant and one mood stabilizer at the same time. I'm 90 days off of Quetiapine/Seroquel and I am taking zopliclone every other day. I won't take zopiclone every day as it's addictive. I'm hoping to do CBT-I for insomnia to re-wire my neurodivergent brain to relax and sleep. I've had general CBT, Dialectical Behavior Therapy (DBT), system training for emotional predictability and problem-solving (STEPPS) and Mentalisation-Based Therapy (MBT). I'm waiting to get assessed for my sensory processing/autistic traits and my childhood trauma. Then I will look into therapy and coping specific to those. I currently use BetterHelp online groups and an individual therapist. Individual therapy, group therapy and support groups keep me off the meds. I weigh 200lbs but I don't have any obesity illnesses or body dysmorphia. My joints hurt a bit but that's normal for my age...even in thin people. Exercise, massage and going to the spa helps my joints. Definitely a case of use it or lose it after age 40. I hope you find solutions that you can cope with.

      @@marleyhill34 Overmedication is a common practice in psychiatry. Many people are put on a cocktail and left without periodic review/evaluation. They may not need the same dose as they did during the crisis and stabilization phase, they may after a time taper and get off of the medications after implementing additional lifestyle interventions. But often these things are overlooked.

      I gained 20 lbs on Mirtazapine and decided to taper. Many psychiatric medications cause severe weight gain, and some are more notorious than others. Mirtazapine is one of those medications. If you're extremely underweight, it could be helpful for short period of time but otherwise, no one knows the long term effects. Have you explored other lifestyle interventions to help you with your weight? What have you tried?

    After taking them for more than a year, I stopped taking antidepressants (escitalopram), anxiolytics (xanor), and antipsychotics (abdin) last december 2023.
    I have been gaining weight and currently i gained 40lbs since my last weight pre-medication.
    Are the effects (insulin resistance) still on despite being off-meds for almost a year?

      That is definitely one of the possible effects of treatment with antidepressants. It's extremely difficult and impacts your sense of control, self image, relationship with yourself and partner. It also can limit you from fully engaging in exercise and other activities. It can be incredibly distressing for some people. How do you go out and do the things that bring you satisfaction or joy when you just don't have the energy or motivation?

      Seconded, I tried vraylar and it worked amazing but my insurance denied it. Hopefully will have luck with something else. To me, the only side effect I got was dry eyes 👀

    Thank you! I have bipolar, Add, Pmdd and was recently diagnosed with chronic fatigue syndrome. The only thing my doctor could help me with (concearning CFS) was advice to try probiotics. It reduced some symtoms of depression so less tired now tbh. Other than that i dont know yet 😅

    The problem is these meds permanently change some people's set point. My aunt was very slim her whole life at 110lbs until she went on anti psych meds and gained 45 lbs. Five years later it just won't go away despite her having a verty active job and a healthy diet.

      I have mixed feelings about it. I think it depends on how much you need to lose. Some people are stuck and the only way they will lose weight is with this kind of help and they need to stay on their meds. The potential downside is possible side effects from the drugs (like pancreatitis) and needing to stay on the med to maintain weight loss. I'm just now hearing more about the downsides of these meds so that's why I'm a little iffy now. But I think if a person drastically changes they way they eat, then when their appetite comes back when they stop the med, maybe they can maintain the weight loss. We don't have enough long term data on this to know if the diet change is the answer to regaining weight.

    I have gained a lot of weight from many different antidepressants since i have been changing them because nothing was working. Now i am on Zoloft + Wellbutrin and i eat half the amount but i can't seem to lose weight. I wonder if probiotics will help to lose the antidepressant weight (not just prevent it)

    my previous psychiatrist lied to me about olanzapine having less side effects such as weight gain compared to other ap medication. i switched to another psychiatrist that actually wants a successful medicinal treatment for his patients so i switched to seroquel and not only all the worst side effects from olanzapine faded, i stopped gaining weight all the time. it feels like i am not taking any medication and schizophrenia doesn't exist with seroquel. main one is the flat affect; i was struggling with it for 4 years and didnt know what was happening, was lied to by psychiatrists that it is because of my diagnosis, not the aweful medication.

    Can I ask for a link to the study you mentioned around improving the gut biome and its link to weight loss? Also I’d like to start taking probiotics to test this but wondering what doses are required for change?

      Just an update: I started taking this combination of prebiotic, probiotic and postbiotic a month ago and there hasn’t been any noticeable improvement.

      Fair enough, it is a tough situation when you have to choose between your physical and mental health? It's not a choice at all, because they're both interconnected. Have you explored additional lifestyle interventions along with your medications? As well, do you regularly review to see if your doses can be adjusted?

      I did mentalisation based therapy for 18 month and now am 90 days off of quetiapine, so it is possible.

    I take a pro-biotic from Amazon and it hasn't put a dent in my gut health. Is their a really good pro-biotic that's not just trash with a fancy label? Or super expensive because a famous RUclipsr puts his face on it? Is there a more medical grade version? - Thank you.

      Probiotics in pill form are a scam. Any beneficial ingredient is destroyed by stomach acid. Cabbage and fermented food increase the biome.

      There are many alternatives eating patterns that could be very helpful in your situation. Certain elimination diets can be an option. However, it would be very important to know more information about your overall health before making any dietary changes. How long have you noticed issues with fiber? Do you currently follow a particular diet?

      @@ryarya3291 As human beings the only essential foods we require are protein and fats. We cannot live without those. Fiber and carbohydrates are not a requirement. For some people they feel better having a little bit of carbs, but it is not a necessity. You can always significantly lower your fiber intake and not completely eliminate it. I also find fermented veggies are easier to digest than any other types of vegetables. What are the list of carbs you consume right now?

      But you don't have to buy any supplements. You can alter your gut bacteria with food alone.
      I eat yogurt and sour kraut every week and I drink kefir every day.

    Took several months to realize trintellix was making me gain weight and causing me to crave sweets and high carbs. My waist was 27 inches and now it’s 30. Not happy at all about this especially after reading a majority of people on trintellix never experienced this.

    Nope. The meds are the problem. You could have the healthiest gut in the world. Those meds are more powerful. They massively damage your sleep, and sleep disturbance will almost always result in weight gain. Get to the lowest dose that stabilizes you as fast as you can, and build your non pharmacological toolbox. NOW. Today. When you think you don't need to use routines, stress reduction, and daily mental health management skills, you are at risk for a crisis.

      There are lots of ways to approach mental health, and a holistic approach with multiple lifestyle interventions as you listed is a great way. Often it doesn't involved a lot of terrible side effects, and most of the time it improves your overall quality of life. Medications can be helpful in acute crisis and to help stabilize, but long term they can cause serious issues. They end up creating problems, that are sometimes worse than the conditions the client was put on medication in the first place.
