Which Antipsychotics Cause The Most Weight Gain?

  • Antipsychotic medications are powerful drugs that are used to treat more than psychotic conditions like schizophrenia. Many of them are FDA-approved to treat bipolar disorder and depression. Even though they work very well and fast, they have some big side effects like weight gain and insulin resistance, some more than others. A new research study has given us some insight on which ones are better and worse.

I'll take mood swings over weight gain. The fatter I get, the less inclined I am to exercise due to joint pain, which then leads to a downward spiral with meds just accelerating the problem.

Abilify for my depression/anxiety made me gain 15 lb in a couple months. It was a huge stress to me, especially since I already have body image issues (including gender dysphoria). But I looked online, and all the official sources said it was weight neutral, so I felt like the weight gain was somehow my fault. Now I'm switching to lamotrigine years later and hoping it'll go back down.

    • @lucakat9262
      @lucakat9262 Год назад +8

My doctor put me on Abilify and I gained 70 pounds on a 15 milligram dose a day. I asked my doctor about this and all he said was that I needed to exercise more. Finally, I decided to research it and found out that, yes, it can cause weight gain. So I slowly weened myself off of it. After a month of not taking it I lost 16 pounds. I'm continuing to lose. Plus, I found out it can cause you to overspend money and gamble a lot. So I also noticed that not only was I not hungry as much but I quit going on horrible spending sprees. For some reason, I lost my desire to buy things all the time. I was wasting thousands of dollars a month buying things I didn't need. I had to file bankruptcy once and I was behind on my rent and I owed my mother money. So I wouldn't recommend Abilify at all. In my case I changed for the better. Another person online said they had gotten in so much gambling debt that they lost everything. But, when they stopped taking Abilify, they didn't feel the need to gamble anymore.
Personally I think these and other antidepressants need to be researched more. Please, look at the side effects of anything you are taking. Plus, it may also help to further research the drug for any further or worse symptoms it can cause. So definitely be careful.😳

This is a great video. I would love to see a similar one in regards to SSRI’s.
I have been on Trintellix for several years at 20mg (the highest dose) and even though the FDA states it’s the only SSRI that does not cause weight gain, well, that is a myth. I was about 145 lbs. when I started and have creeped to currently 195 lbs. I have a small frame and am 5’ 9”.
I get so angry that most insurance companies (including Medicare) does not cover any weight loss drugs. I also suffer from hypertension and the weight I carry is not good at all.
My doctor attempted to get me on Wegovy but there really aren’t any insurance’s that cover it. The OOP was approx. $3,500 for one month (4 injections).
Even though Ozempic is the same exact drug, and only $10.35 co-pay per month, because I don’t have a diagnosis of diabetes I can’t get it. Phentermine is also out of the question because of my severe GAD anxiety.
There’s just ZERO solutions out there for people who struggle with drug-induced obesity.

I've taken literally all 10 of the medications on this list, and 9 of them caused me to gain a significant amount of weight. (80-100 pounds!) The only one that did not affect my weight, and I was actually able to lose weight while on it, was Latuda. Unfortunately, I eventually developed a sensitivity to the akathisia caused by Latuda and had to stop taking it. Since then, I've regained all the weight I lost, plus 20 more pounds from trying other antipsychotics. After years of this, I am finally OFF antipsychotics for good (with my doctor's approval). I am currently morbidly obese and am seeing a bariatric doctor to help with weight loss. Hopefully I can get back to a healthy weight. I couldn't while on antipsychotics. It's kind of pointless to be on a medication that's supposed to help with depression, only to gain 100 pounds from it and become even more depressed because of your weight.

I experienced the same thing to a lesser degree (“only” gained 50 lbs, only had to try 4 or 5 meds, and I was able to be off the meds in a year so most things weren’t permanent for me), and it really ruined my life, so I have a lot of empathy for what you are going through.

This caused noticeable bloating and weight gain, not too much, but enough to make me feel insecure, which affected my mental state and made my depression/hypomania worse, and because of this, my doctor increased the dose of quetiapine. And so on. Now without quetiapine I am trying to lose weight and it is quite difficult. I'm afraid that my body was ruined.

My worst nightmare is telling my doctor I'm doing better and them taking me off or lowering my dose. Yes they are doctors, I get it. They aren't in my shoes though. Finally getting to a good place and then having the stress of possibly being taken off the medication is super counterproductive to me personally. One size will never fit all with mental health. It's not like the medication is doing all the heavy lifting. Im still in my body experiencing the excruciating pain that is living with MDD and working with the medication to have a morsel of what a regular life could look like, BUT with the notion in the back of my head that my doctor can decide that the protocol is that " you arent supposed to be on it forever." When its keeping you alive, only to offer mediocre group therapy services that make the situation worse.
At the beginning of the year I started taking naltrexone to control my alcohol use. I have lost 20 pounds since from A. Not drinking alcohol. B. The combination of naltrexone and Biupropion is a weight-loss medication that is out on the market under a different name. Contrave.
Advocating for yourself when it comes to mental health can be a struggle when you're side eyed for doing your own research. The google doctor gets a bad reputation but there are things like naltrexone that I found through doing my own research that my docotor would have never been on her radar. My psychiatrist tried multiple different antipsychotic medications to try and manage my unipolar psychotic depression when I was 18… the only one I could (kind of) tolerate was risperidone. I started on 1/2 of the starting dose because my body is extra sensitive to most medications. On this dose over about 4 months I gained 50 pounds, had elevated triglycerides, and was pre diabetic (no family history or risk factors). I weigh roughly 100 lbs usually, so that was an insane and life changing weight gain for me. It endangered my life. At the fastest period of weight gain I could not even roll into fetal position because my skin was so taut… it hadn’t had time to stretch…. and once it did I got stretch marks from hell.
On risperidone I also had increased prolactin levels, so I was producing breast milk, and it would leak constantly through my clothes.
I felt like I had extreme low blood sugar (faint, light headed, dizzy) if I didn’t eat every 3 hours.
To be honest, even with all of that said, it was better to be on the medication than not, because psychosis was the scariest thing I’ve ever experienced. I did not have any quality of life at all with my psychosis. I was able to get off of risperidone after about a year of managing my psychosis, and I have learned how to manage it with my other meds and monitoring my anxiety… but it no doubt changed my body forever.
My weight gain never plateaued, I kept gaining until I was off of it. When I stopped taking it, my body shed the weight immediately (although of course my skin was still stretched out).
I hope that we can continue to develop more options for people experiencing psychosis so that they don’t have to make the choice that I did. The hell that my body went through on risperidone was actually the better option because I actually had a life.
The scariest thing I can think of is having the psychosis return and having to go back on the risperidone…

I was first prescribed olanzapine during a lovely stay in a psych ward while I was experiencing acute mania, and two subsequent psychiatrists kept that prescription active for no other reason than that it had already been prescribed to me (even though my mania had subsided -- now I know this was a mistake). Months later I pointed out how it seemed to have a noticeably negative effect on my metabolism. After quitting the olanzapine I did feel slightly increased symptoms of anxiety, but the tradeoff was worth it overall.

I just watched your video with Lauren on her Living Well with Schizophrenia page. I have this video pulled up on my TV and computer. That way I can watch it on TV but comment here. I know I have some things to share and maybe even some questions based of the video with Lauren only I want to watch this and consider anything you may share here.
I had an eating disorder a large part of my life. It was anorexia with bulimic tendencies for the most part although there was a time where I definitely had a lot more the the bulimic behaviors. I had gone in to some form of treatment many times however I am going to share about the last time I went to a treatment center.
I went into the treatment center at a low weight and had stabilized at about 115-120 before they put me on Zyprexa. Now, at this time I had never shared any of what I dealt with that I now know to be psychotic features. I continually told them Zyprexa wasn't working for me and they insisted it was. People talk about it making you tired however I never experienced that. The only change I experienced was gaining lots of weight super fast. One day while in the program when we shared what we had to eat over the weekend I shared that my mom had wanted McDonalds so I had a burger and fries from there. As the weight gain started they began to place the blame on me shaming me for having eaten at McDonalds telling me I shouldn't be eating at those places so much. I literally ate there that one time. What they didn't know is I was still skipping meals and such and I definitely wasn't eating enough food to cause me to gain that amount of weight. After graduating from the program while secretly still using behaviors, I went off Zyprexa and soon after my eating disordered behaviors stopped. (I am not correlating stopping Zyprexa helping me with the eating disorder.) Sometime down the road I was put in a psych hospital where they put me on depakote. I then gained a lot more weight again. In less than a year I gained from about 115 pounds to 212. Depakote hadn't helped me either. Although when I went on depakote I don't have any memory of who knows how long because I suddenly woke up one morning after having a dream that these people were doing crazy things to me in a tent and they had people surrounding it to watch me and make sure I didn't get away. When I woke up there was a staff following me for some reason I had been put on one on one and had no clue what had been going on. I didn't have any answers.
I am one of those patients that seem to be treatment resistant as far as medications go. Anyway, So... 5 or 6 years ago my mom and I went on a plant based diet and I lost some of the weight finally even though we weren't always making the most healthy choices. I did gain some of it back at first because it was halloween time and I couldn't seem to stop eating chocolate. Once I stopped eating all the chocolate I stayed at that weight. As the years passed there was no more weight gain but there was no weight loss until last year. I went on a whole food plant based way of eating for probably 6 months and lost a great amount of weight. I was also able to start going to the gym. After some things took place it affected me being on the whole food plant based way of eating however I am doing my best to make good choices that are beneficial to my physical and mental health issues.
Now that my weight is at a healthy level and I can exercise, which I love, I absolutely have nothing in me that is willing to take a medication that's likely to cause weight gain. They tried me on Abilify only it caused me to become out of control with anxiety. Anyway, I am thankful for this video.
I don't remember the full point in me making this comment. I do want to ask.... What do you do when you have someone who is medication resistant?

I gained 80 pounds on quetiapine. And man is it depressing. I'd been slim my whole life up to this point. My weight stabalized after switching to rispiradone. But I still couldn't make much progress getting it off. I was finally switched to lurasidone. Which has also been more effective at managing my bipolar symptoms with the least sedation. But I'm still struggling to lose this weight. And because of the weight gain and how bad it makes me feel physically, I've started skipping doses of my medication. I know that it can get bad. Bipolar 1 is no joke. But I can't live like this. It hurts to be this heavy. I have a small bone structure. I'm not meant to have so much weight on my body. My back hurts every time I walk for more than 5 minutes now. And that makes exercise so much harder. I'd always been fairly fit. Usually doing about an hour of exercize 4 to 5 times a week. And now it's so hard to get the exercise in that I want to. I feel lucky if I can push for 3 hours a week. And the exercise helps so much with the deoression.
I've been carefully monorting my sleep and mood daily though. And trying to be real mindful of my symptoms. I've been trying to be mindful of when I start to feel more paranoid. (A common symptom when I'm starting to become manic) And I've taken my meds when I do see the symptoms coming on or if I start to sleep less than I should. And I'm focusing on keeping a normal social rythem. I have alarms set for everything I need to do, including my go to bed alarm. And honestly, I think that close monotoring and medicating only when symptomatic could really be a good option for treatment. But I do plan to be on my meds all the time when I'm done getting most of this weight off.
I'm just so afraid of ending up diabetic or with heart disease. I'm in my 30's now. And that's the age when struff starts to catch up with you. I have a family history of diabetes and high blood pressure. And I feel like obesity could also make glaucoma worse. And I'm predisposed to that as well. We gotta start treating the whole person while managing psychiatric illness. I wasn't even warned of the weight gain side effects. How can I combat something when I don't know it is going to be a battle?
I do feel like lurasidone should become one of the first line medications now that there is a generic out of it.
I just want to feel healthy, mind and body.

I really feel for you. I am also a small person and had been slim (even underweight naturally) my entire life and I gained another half of my body weight on risperidone. I can relate to the back hurting/bones hurting feeling. I felt like my frame couldn’t take the extra weight! And I did become pre diabetic from the meds (no risk factors) and my triglycerides were very high. The drs had to pull me off the meds at that point for the sake of my physical health.
There is no easy path through this. No one understands accepting such damaging side effects unless they’ve been here. It’s not a real choice.
I hope you can find a solution, and that medicine comes up with more solutions for your disorders soon!

@@tjw9985 yeah. I still deal with symptoms while fully medicated. They're just more manageable. It's just exhausting to deal with for so long. And it never goes away. I've learned a lot of good coping skills with therapy. And I'm rather stubborn. So I'm gonna keep on keeping on. I just want there to be more available for us to get to and stay in good health. I think we deserve as much normality as anybody else.

@@fieldofreeds8581 Thank you for your kind words. A lot of people don't seem to get how painful all the extra weight can be for those of us not built to be thicker. I literally have a tinier frame than a lot of people who tend to be heavier. While it's important for us to have our sanity, we still need to care for our bodies. And the current med loadout and way our psychiatric care is often handled doesn't factor in the whole person. I miss the feeling of being physically healthy. And I feel like I've been floundering trying to figure out this weight. And any time I go off meds I tend to drop a few pounds. So it doesn't make medication very palatable. If it were 5 to 10 pounds, I'd just deal with it. But last time I weighed myself, I was up past 150% of my original starting weight. And I hear this same story over and over again. We deserve better.

Same story with Quetiapine.. I've also always been rather slim/fit or just normal for a guy. I now gained enough weight that i sometimes feel like im pregnant 😅 or like i have a beer belly when i literally never touch alcohol...
It also gave me stretch marks on my belly and thighs which made me even more depressed about it, like i now ruined my body irreversibly and the marks will always be there to remind me.. i know there are worse things but with depression it's hard to brush these things off.. but i do tell myself it's no big deal, and that there's nothing to fix it anyway, and it's not like I'd have surgery for that if it even exists..
I just feel like i tried so hard, finally took the steps to seek medical help, and just got disappointed in the end.. i know it's a journey that can be long, but sometimes it's just too much and life/deadlines etc dont always allow to just spend months or years trying to find the right thing, trying meds that can sedate you so hard you have to stop work/go on sick leave and can't even wake up, end up needing 12h+ of sleep, gain massive weight, AND its ineffective for both my sleep issues and depression in the end... it was initially helpful sometimes but not anymore.. just ugh.
Now my pdoc suggested switching to lithium.. i guess i have to try although im just over it and scared of side effects a this point tbh.. also very scared of agonizing Quetiapine withdrawal that i had the misfortune to experience after i was told by my doctor to reduce from 400mg to 300mg in 1 day..
Its just all too much sometimes, and it feels like it just added shit on top of the depression.
And as a masters student i've had to go on sick leave from my work-study program which just sucks.. especially as a foreign student i might just end up failing and getting sent back.. idk
Sorry ended up rambling 😅 eh what can you do though.. i just have to keep trying and hope to recover just enough to bounce back...

I gained 90 pounds in 6 months on mirtazapine without any diet change. Even though I stopped taking it immediately after those 6 months, now it's 7 years later and I'm still trying to lose even a fraction of it (diet, exercise, endoctrynologist - nothing works). It's horrifying

Abilify and Rexulti kicked my ass after loosing my dad young and then having post partum a year later it was added to my Zoloft gained 40 lbs in about a year and tried for 2 years spinning for 1 hour 6 days a week and eating healthy and my weight WOULD NOT BUDGE. Finally put it together and the weight melted off literally within a couple months. Blew my mind and was also irked that I was never warned about it or educated about the possibility that I would literally be completely unable to loose a single lb. while on it. It also does something to the H1 (I believe) inhibitors (metabolic) in the brain which makes the body hold onto the weight. Just educate us.
