  • Why do you gain weight with antidepressants and mood stabilizers?
In this video I discuss two way. One way is by blocking histamine 1 receptors. We usually think of histamine as something that is associated with allergies and your immune response. If you’re exposed to something your body is sensitive to, you get a histamine response that can look like itching, runny nose or even shortness of breath. And if you take an antihistamine like Benadryl, it slows the allergic reaction and you feel better. That's how it works from the neck down. Histamine acts differently in your brain. In certain parts of your brain, it’s responsible for wakefulness or your ability to be alert. That's why if you take an anti-histamine it may reduce your allergic reaction, but if it crosses the blood brain barrier, it can also make you sleepy because it's blocking histamine’s stimulating effect in your brain. There are four histamine receptors, I’m going to talk about H1 receptors.
A second mechanism is blocking the 5HT 2c receptors. This is just a type of serotonin receptor. It’s been shown in mice that if you activate their 5HT2c receptors, they lose weight because they eat less. And if you block the receptor they became obese. Many of the newer antipsychotic medications block this receptor. Of all the antipsychotic medications, the worst offenders when it comes to weight gain are olanzapine, clozapine and quetiapine. And all three of these medications block 5HT2c.
In this video I also discuss what you can do to offset the weight gain.

The worst part is that doctors don't bother telling you about this side effect and then act like you're crazy when you bring it up. Rapid weight gain makes me depressed and ruins my self-esteem, is that not important to consider when treating my mental health?!?!

What is really messed up is that every single doc who prescribed these meds just says, "oh, just eat healthy, walk, you won't gain weight", that is such a load of crap. It’s dishonest, inexcusable, and inadmissible. And stupid people who vomit thought-terminating clichés and say to just think positive, are equally stupid. Depression doesn’t go away with exercise or positive thinking or a healthy diet. Don’t confuse normal sadness with depression.
So, a depressed person needs these meds, but then they put so much weight on that they start becoming depressed over their weight. And doctors are dismissive and ignorant.

I gained major weight very quickly, and am now 55 pounds over my starting weight (which was in normal range) even though I've been off the med for a year now (and have lost about 10 pounds since). He always denied that the weight gain had anything to do with the abilify even though when I was on cymbalta alone, my weight was stable. The lack of validation, and frankly respect, made me give up on psychiatrists altogether for a long time. It was especially horrible because I have other serious medical problems that make weight gain dangerous for my health. I am now, finally, seeing another psychiatrist. They seem to think developing "Metabolic Syndrome" and subsequently becoming diabetic is an acceptable trade-off to being insane. They seem to think that just eating less and exercising more is enough to counteract the drugs side effects. It's no wonder then that studies have shown that people with severe mental illness like Schizophrenia, Schizoaffective Disorder and Bipolar Disorder have a life expectancy of 15-20 years less than the general population!

During my anti depressant taking I experienced this side effect as well, and even though it'd been my most active period in terms of working out, it literally changed nothing in terms of body fat in my body.
Yet, one month after quiting the anti depressants, and through whole last year I've "returned" to my natural skinny state, despite me literally giving up on workout entirely.
So this side effect weight gain was so strong, that even intense workout couldn't have outdone it. While simply knocking it off entirely made all the difference.

So not only did I gain weight, the standard remedy no longer worked.

@Robin Tiefel a person can be sore for a number of reasons, not just because they're out of shape. I think exercise is beneficial to almost everyone though, although it doesn't always make much of a difference with severe depression once the temporary endorphin high wears off. I know of a number of ppl who feel worse after exercising because the very short endorphin 'hit' gives them a taste of feeling better, only to drop back into the severe depression a short time later. One guy I knew played competitive basketball, & by the time he would be back in his car after a game he would feel worse than ever because of the sudden mood change. My fiancé has military PTSD & feels immediately depressed if he works out. I myself have ME/CFS which is made worse by exercise. So I guess everyone's different, although of course for most ppl it's beneficial, for the body at least.

This was interesting, but didn't address people like me. I'm not depressed, I was put on Prozac because I have trouble sleeping. I ended up gaining 60 pounds - 125 to 185. I was already working out hard at the gym and had excellent eating habits, nothing changed when I went on the drug, just tons of weight gain. Not everyone over-eats and gains weight, sometimes it's all caused by the medication. Cutting back on food seemed to make the weight gain worse.

I came off hormonal contraception and gained 30kgs. Was told by a doctor that I must have significantly increased my food intake and decreased my activity levels. Neither of those things happened. Over 10 years later I meet a doctor that tells me that yes, the pill can suppress weight gain (I was put on it quite young too because of hormonal issues). We are told that if we lose or gain weight suddenly we should seek medical advice because something could be very wrong but when it comes to weight gain, doctors simply don't care.

I have bipolar 2, and am on quetiapine, and I gained 103 lbs in 8 years. I cannot begin to tell you how that has affected my self esteem. I am so glad that someone is presenting scientific reasons for what I have been claiming for years. When I first started taking quetiapine, in about 45 minutes I would enter a hazy state, and would find myself wandering into the kitchen and eating whatever I could get my hands on. I had, and still have intense cravings for carbs and sugars, and never really feel satiated when I eat a normal portion. The only way I have been able to start losing weight is with the ketogenic diet. I've lost 30 lbs so far. It kills the physical hunger, but I still have the mental cravings, and have fallen off keto more than once, so losing weight has been slow, but I guess it's better than not at all. I don't want to take any medications to make me lose weight. I just don't want any more drugs in my system. I actually feel really good on the keto diet, and plan to stick with it for the long term, and maybe even be able to reduce some of my meds.

i've been on Klonopin long-term, and it gave me my life back. Without it, i wouldn't be able to sleep, eat, or go to school or work because i'd be having panic attacks 24/7. it's truly the best for anxiety, and i've tried countless other medications throughout my lifetime including medications in the same category (Valium, Xanax, Ativan). I keep my dose low and skip days or take half pills sometimes to prevent tolerance from building. (i used to be on a high dose, and it caused really bad rebound anxiety, so i had to wean myself to a lower dose). it's probably best if you use it sparingly like 2-3 days a week. you need to find a doctor who is understanding of your situation. i don't care if i'm dependent on it for the rest of my life because it allows me to have a very good quality of life that other medications do not. i don't drink alcohol anymore either because i'm no longer anxious to the point where i'd want alcohol. i know this is an unpopular opinion and probably not like others' experiences, but that's what my experience has been

Your doctor sounds real obnoxious haha. I'd be frustrated too.
Strangely enough, I have lost some weight (though only some, about 17 lbs on the scale but about an 3 inches on my waist; and what's really bizarre is that recently I lost 1 and a half inches on my waist so much my clothes fit loose but no weight on the scale) on zyprexa and I get plenty of praise from my doctor for that and she always points to it as proof that you don't *have to* gain weight on zyprexa but I don't think she realizes how hard I had to work just to lose 17 freakin' pounds.
Sometimes I think (even though she's basically a nice person) that she's more concerned about the money than the well being of her patients. I don't think that about all doctors but some I do.
I had to work out about 150 minutes to for a while 12 hours a week, walk the dog 6 days a week an hour each day, eat fairly small amounts (for a while 800-1200), and, drink coffee on a regular basis. About 2-4 cups a day. Sometimes more.
And even though I have a somewhat slender waist, my weight is still pretty high (though not overweight) which is super frustrating. >:(

This explains so, so much. I have adhd and PCOS, which already makes impulse control extra difficult and makes weight easier to put on and harder to lose. Knowing that my anxiety medication is possibly making weight loss even harder is frustrating, but helps me to focus on finding a solution instead of blaming myself. I can't take bupropion because I had a febrile seizure as an infant, but I'm hoping to find help from a nutritionist or psychiatrist who has experience with these extra complications.

I also changed my plates to smaller ones so I couldn't overfill them. I was feeling a bit hungry after finishing the meal but it was a bit different. A bit like regular and diet soda. Both are sweet but you taste the difference. I did get munchies so I never purchased junk or if I didn't resist, I'd had to walk to the furthest store and back to buy it. Didn't always work of course but I didn't gain much at all untillmy doc told me I didn't need the meds. Best day of my life and best choise I did was starting the treatment. Hello. Thank you for another educative video. I'm a big fan I usually watch all your videos every Wednesday. I been on mood stabilizer and antipsychotic drugs for quite a long time and the questions has been answered. Thank you! Now it make sense why I gain 30lbs for the last 7 years. Although the effects of medicine outweighs the gain weight. We all know how important it is to take medicines on a daily basis. This has been an important issue for many patients thank you for making a video on this. ❤️💙💜 💕

This is so helpful. I went on Paxil last August and I've gained 23 pounds since. I'm super active, but still gaining weight. I'm going to start tapering off this week because I can't deal with what it's done to my weight. I'm a fitness professional and this is unacceptable! Time to be natural again. My life circumstances that contributed to the anxiety and panic attacks to begin with have changed, so my prayer is that I can come off of this medication and go on to live a normal life!

I’m in the same exact boat as you were. I went on Paxil in May 2021 , it’s now December 2021, and to date I’ve gained 25 pounds. I’m tapering off pAxil and want to be natural once again. How did you end up? Did you lose the weight coming off and how long after you stopped did you start to lose ? Thanks !

@@JB-sj8sj I tapered in May and was totally off within 5 weeks. The tapering went well! But I'm still struggling getting the weight off. It's really frustrating!

Thank goodness!! This is exactly how I described it as blocking my bodies ability to know when I am full. I reduced my meds for a while and my appetite shrunk too. I went back up to full dose again and have to eat a lot more to feel satisfied. I even tracked my weight when first going on the meds and the first two months was significant weight gain then slow gain for the rest of the time but I was told but the nurse it wasn't the meds.

Thank you so much for this video! I was on Olanzapine and gained 20 pounds in a blink of an eye. Which i think made me feel worse. I’m now adjusting to Vraylar and hopefully I can find the will to shed the lbs via exercise and healthy eating, but those simple tasks are really hard at times. I wish my “care team” would’ve explained these meds like you did in this video. Thanks again! 🤗

Same here! I gained 35 lbs in 3 months and I eat like a bird. I eat healthy but could exercise more (but I have back issues so it's hard). I am normally 115 and weigh 140 now (only 5'3"). I am stopping that med and just staying on my Lamictal for a mood stabilizer and one .5 Klonopin split into two doses as needed for anxiety/panic attacks which are disabling for me. I hope to lose the weight (so far strict dieting has done nothing which is unusual for me) very soon. I have read positive reviews for people once they stop taking it it just falls off on it's own (about half of it within three months) I sure hope so! I will still continue to watch my diet etc.

Holy hell it’s been such a screwed up year that I had completely forgotten about this side effect, thinking it was just the lockdown or increased drinking. I stopped drinking and still haven’t noticed much difference, and didn’t even think of this possibility until this video popped up on my recommended feed…
