format USB as exfat, if it does not work then try a different USB

Have you tried pppwn lite? And make sure you have the payload on the USB and the PS4 sees the USB
Whats the benefit of using this ppwwn other than ppwwn lite?
you can use this one to check if the connection is okay, then you can use the lite loader instead.
