Kalluto is also referred to as a 'brother' despite looking like a young girl. And if we take Zoldyck's family tradition into account, Kalluto might very well be a girl and not a boy.
No both alluka and kalluto were born male alluka is trans and that's why killua (the only member of their family who loves her) calls her a girl while the members of the family who don't like her refer to her as a boy. Kalluto is just cross dressing though if i remember correctly their mother likes to dress the children up in her old kimonos when they're young (the last part could be me misremembering though).
