My Thoughts on the First Postpartum Depression Pill Zuranolone/Zurzuvae

  • The US Food and Drug Administration has approved the first oral medication for the treatment of postpartum depression. It will be sold under the brand name Zurzuve and the generic name is zuranolone. We know that postpartum depression is a serious and potentially dangerous condition that puts both the mother and infant at risk.
    Prior to this the only treatment for postpartum depression was a 60-hour infusion that is only available at certain centers and carriers a premium price tag $34,000. Zuranolone uses the same mechanism of action but offers the medication in a convenient pill form.

    Progesterone, (100mg) which is cheap and readily available is very rapidly converted to Allopregnanolone by the body, and IMO would have the same end results of working on gaba receptors for PPD.
    Somone I know was prescribed progesterone 100mg x 3 doses at night for PPD / insomnia when they also had hpa axis issues due to drug induced hyperthyroidism (over medicated hypothyroidism post partum) it was very effective for this person.
    I am not asking for medical advice, but I'm curious your thoughts on knowledgeable providers using progesterone (off label) as a cheaper alternative to this drug with is basically allopregnanolone. I understand the financial motivation for pharma to liscense new meds for profitability, but in theory how would high dose progesterone be any different than this new drug?

    Thank you for the video. 2 questions, why did it fail for MDD? Second why is the preposed dosing intermittent as opposed to every night in order to sustain the benefits. Presumably dependence is a concern but that problem already exits with lots of current treatments.

      It did not result in a clinically significant improvement in depression over placebo. Essentially it did not beat placebo in a phase III study. If you think about it we know postpartum allopregenolone levels are low and this medication is acting like a replacement for that neuro steroid where in depression we would not expect that and no one as far as I know has looked specifically at those levels in depressed patients but I would guess they are likely to be normal. This medication severely limits your ability to drive or operate heavy machinery. You have to wait 12 hours after taking the medication before driving. I think they were hoping for a short course of treatment that results in prolonged depression improvement. Thats my speculation

      It's a very interesting medication and mechanism of action. I think the rapid onset of action and short duration of treatment are most appealing. The cost will probably be prohibitive for community psychiatry but we will see

    Zuranolone has the same abuse potentional as Xanax, not a very neuroplastic molecule😅
