Per the continue: the designers have to add the 'No' option for legal reasons. Hypothetically, Little Johnny could continue to play until the end of time, with unlimited always-affirmative continues. Hence the 'No' option is there, in the event Little Johnny dies from too much NES, the designers could always say 'We gave the option to quit playing'.

On the orange cog wheel boss all he has to do is hold box overhead and let the wheel hit him with box overhead and cog freezes and you can than grab another box/crate. It’s a timing thing it takes time to beat that boss
